Which version of Dead of the Brain should I play?

Choose the planned Steam edition for the easiest modern access

Choose MSX2 with a translation patch if you want a documented English or Spanish route now
The MSX Resource Center reported on July 2, 2020 that English and Spanish IPS patches were made for the MSX2 game. The patch text is based on the PC-98 English translation, includes some bug fixes and expands the text area to three lines of 32 characters with a new font.
This gives the MSX2 version a documented translated path, but it does not prove that the MSX2 release is otherwise identical to PC-98. The report also does not specify source-image hashes, emulator settings or a universal installation sequence. Read the patch’s own instructions and use software obtained lawfully; do not rely on an unverified download or a random pre-patched image.

Is Dead of the Brain available in English?
MSX2: yes, through documented patches
The strongest currently documented original-hardware answer is the MSX2. The July 2, 2020 MSX Resource Center report describes both English and Spanish IPS patches. It says the wording is based on the PC-98 English translation, and that the patches also make functional and presentation changes: some bugs are fixed, the text window grows to three lines of 32 characters, and a new font is added.
“Available in English” here means that a patch exists for the MSX2 game; it does not mean that every MSX2 copy is already translated or that the patch works with every disk image. Confirm the patch’s target version and instructions before applying it.

PC Engine: translation work is documented, but status needs checking
The PC Engine translation repository describes a project for translating both games and lists project files for English and French patches. - Hit boxes: optional visibility or assistance for interactable areas.
