Data center transformer failure: warning signs, causes, and fixes

A failing data-center transformer usually gives evidence before it trips. The most useful clues are rising or unstable temperature, moisture, vibration or partial-discharge trends; repeated protection alarms; unexplained breaker-operation anomalies; abnormal waveforms; and, for dry-type units, blocked cooling paths, contamination, unusual noise or loose terminations. Treat any new trend as an engineering issue, not a nuisance alarm. Make the installation safe, preserve relay and SCADA records, reconstruct the switching or loading event, test the transformer with qualified personnel, and correct the underlying insulation, thermal, fault or transient problem before re-energizing it.

Insulation is the central failure path. Schneider Electric states that transformer failures result from compromise of internal insulation caused by cumulative aging, mechanical damage, or design and manufacturing faults. Data centers add unusual exposure: high power density, compact electrical rooms, redundant feeds and frequent primary-voltage switching. Those conditions can turn an otherwise manageable transient into a winding, bushing or voltage-transformer failure.

Warning signs that a data-center transformer may be failing

No single alarm proves a transformer is about to fail. The combination of a changing trend, an abnormal operating event and corroborating inspection or test evidence is much more persuasive. Review values against the transformer’s commissioning baseline and manufacturer limits rather than relying on a universal number.

Observed sign What it can indicate Practical response
Rising, unstable or uneven temperature Overload, restricted cooling, hot connection, degraded insulation or a failed fan Check load, ambient conditions, cooling equipment and connections; trend the data and arrange a thermal inspection.
Increasing moisture or humidity indication Water ingress, wet insulation, condensation or a failing sealing system Inspect enclosure, breathers, gaskets and room conditions; test insulation only under an approved procedure.
Increasing partial-discharge activity Voids, surface contamination, insulation deterioration or an electrically stressed accessory Verify sensor health, correlate with load and noise, and obtain specialist partial-discharge assessment.
New or rising vibration Core or winding movement, looseness, mechanical damage, fan imbalance or an external structural issue Compare with baseline, inspect mounting and fans, and investigate after any fault or transport event.
Protection alarm, unexplained trip or repeated lockout Internal fault, external fault, incorrect settings, instrument-transformer error or a switching transient Do not repeatedly reset. Isolate safely and preserve relay targets, oscillography and breaker records.
Breaker-operation anomaly or abnormal waveform Current chopping, restrike, trapped charge, grounding weakness or ferroresonance Reconstruct the switching sequence and perform transient and ferroresonance studies before changing the procedure.
Dry-type unit: blocked vents, dust, unusual noise, hot spots, loose terminations or failed fan Reduced heat removal, contamination, arcing, loose-joint heating or cooling-control failure Follow the applicable dry-type maintenance practice, including IEEE C57.94 recommendations, and correct the physical defect.
Oil-filled unit: oil leak, abnormal level, moisture indication or evidence of gas generation Seal failure, wet insulation, overheating, arcing or an internal fault Secure the area, inspect conservator and bushings, and use qualified oil-quality and dissolved-gas testing where applicable.

A trend that changes after a switching operation deserves the same urgency as a temperature alarm. Record the exact breaker, phase, time, load and operating mode; transient damage can leave little visible evidence after the event.

Why data centers are unusually exposed

Modern facilities concentrate large loads in compact electrical footprints. Multiple utility services, generator paralleling, static-transfer equipment, automatic transfer schemes and maintenance bypasses create many primary-voltage operations. Eaton’s 2016 white paper identifies high power density, compact layouts, redundant systems and frequent primary-voltage switching as a high-risk combination for distribution-transformer failures caused by switching transients.

Redundancy does not remove the hazard. A transfer that keeps servers powered can still expose a transformer to a different phase angle, trapped cable charge or a breaker restrike. A lightly loaded transformer connected to a long cable may be more susceptible to ferroresonance than the same unit at normal load. Commissioning is particularly demanding because systems are operated in unusual combinations and protection settings are still being validated.

An Eaton case study documented two 24.9-kV voltage-transformer failures during commissioning. The investigation identified ferroresonance when a circuit opened and high-frequency switching transients when it closed as possible root causes. That example does not establish a failure rate for all data centers; it demonstrates why switching studies and field measurements matter.

Root causes of transformer failure

Insulation aging and mechanical damage

Cellulose, oil, cast resin and other insulation systems lose dielectric strength as they age. Heat, moisture, electrical discharges and mechanical movement accelerate that process. Through-fault forces can deform windings or loosen clamping even when protective devices clear the fault quickly. A later transient may then puncture the weakened insulation.

Design or manufacturing defects, damaged bushings, poor joints and incorrect clearances can create a failure path from the first energization. Do not assign age as the cause merely because an older unit failed; inspection and test evidence should distinguish aging from an initiating fault.

Thermal stress and overload

High load raises winding and insulation temperature. Sustained hot operation accelerates insulation aging, while repeated load cycles can loosen connections and stress joints. Data-center growth, high rack density and temporary generator operation can push a transformer beyond its original thermal assumptions.

Use site-specific load profiles, ambient temperatures, cooling configuration and manufacturer thermal calculations. The cited data-center guidance does not support a universal percentage-of-rating threshold. Compare measured winding or top-oil temperature with the nameplate, design documents and applicable standard.

External faults and through-fault forces

A short circuit downstream can impose very high electromagnetic forces on windings and supports. The transformer may survive the first event but suffer hidden deformation that appears later as abnormal impedance, vibration, partial discharge or an internal fault. Preserve fault-current records and relay clearing times for every significant event.

Switching transients and ferroresonance

Vacuum breakers can chop current, restrike or interrupt a circuit with trapped charge on connected cables. Cable capacitance, transformer saturation and an unfavorable grounding arrangement can combine into ferroresonance or high-frequency overvoltage. Voltage transformers and lightly loaded transformers are especially vulnerable in some configurations.

Because these phenomena are brief, a conventional RMS meter may miss them. Eaton case studies used simulations and field transient measurements, then evaluated grounding improvements, surge arresters, RC snubbers and a saturable reactor. The correct remedy depends on the actual circuit, breaker characteristics and measured waveform.

Moisture, contamination and accessories

Moisture lowers insulation margin and can migrate into cellulose or porous cast-resin surfaces. Dust, salt and conductive contamination create leakage paths and obstruct cooling. Bushings, tap changers, fans, temperature sensors, control power supplies and protection wiring can fail independently and precipitate an outage.

Confirm each mechanism with inspection or testing. A failed fan may explain a temperature rise without proving that the winding insulation has failed; an oil alarm may be a sensor defect rather than gas generation.

How to investigate a suspected failure

1. Make the site safe and preserve evidence

  1. Follow the facility’s electrical-safety rules, switching orders, arc-flash boundaries and lockout/tagout process. Do not open an enclosure or approach leaking oil until the hazard has been assessed.
  2. Isolate the transformer and prevent automatic re-energization. Record the asset identification, phase, operating mode, load and exact time.
  3. Export relay targets, oscillography, SCADA alarms, breaker timing, power-quality records, BMS trends and maintenance logs. Preserve the original files and their timestamps.
  4. Photograph the transformer, terminations, bushings, enclosure, cooling equipment and any soot, bulging, oil leak or mechanical displacement before teardown or cleaning.

IEEE C57.125-2015 describes a methodology for determining the most probable cause of a transformer failure and encourages consistent data collection and cooperation between users and manufacturers. A disciplined evidence record is more valuable than an early, unsupported diagnosis.

2. Reconstruct the event

Build a timeline that aligns load, temperature, breaker commands, actual contact travel, protection operation, waveform captures and recent maintenance. Mark whether the event occurred during energization, transfer, de-energization, a fault or normal steady state.

For suspected switching damage, model the source impedance, cable lengths, transformer saturation curve, breaker current-chop and restrike behavior, grounding system, surge arresters and connected voltage transformers. Compare the predicted transient with field measurements. Eaton’s data-center investigations combined simulations with on-site transient measurements for this reason.

3. Select tests that address the hypothesis

Testing should be performed by qualified personnel using the transformer manufacturer’s instructions and applicable standards. De-energize, discharge and ground the unit between tests; isolate accessories that could distort a measurement.

Test or inspection Useful question Important limitation
Visual and thermal inspection Is there contamination, a hot joint, tracking, oil leakage, damaged insulation or a cooling defect? A normal exterior does not rule out internal winding damage.
Insulation-resistance and polarization measurements Has insulation resistance changed relative to the commissioning baseline? Temperature, moisture, test voltage and lead configuration affect results; do not use an invented universal pass value.
Winding resistance and turns-ratio tests Are connections, tap positions and winding continuity consistent? Compare phases and taps with manufacturer data and prior records.
Power-factor or dissipation-factor testing Is dielectric loss increasing in the insulation system or bushings? Interpretation requires temperature correction and suitable reference data.
Partial-discharge testing Is there active internal or surface discharge? Separate genuine discharge from electromagnetic and mechanical noise; use an experienced specialist.
Oil quality and dissolved-gas analysis, where applicable Is moisture, overheating or arcing generating evidence in the oil? The cited material does not establish universal gas thresholds. Use the applicable laboratory and standard, and interpret trends rather than one number.
Transient and power-quality capture Did switching create overvoltage, restrike or high-frequency oscillation? Instrumentation bandwidth, trigger settings and probe installation determine whether the event is captured.

4. Determine the most probable cause

Rank hypotheses against evidence. For example, a temperature rise that began after a fan failure supports a cooling problem; a sudden protection operation coincident with breaker restrike supports a switching event; a gradual partial-discharge increase with moisture supports insulation deterioration. A transformer can have an initiating cause and a damage amplifier, such as a weak bushing exposed to a transient.

5. Report and act

Document the event timeline, test methods, raw results, photographs, probable cause, contributing conditions and corrective actions. Share the report with the transformer manufacturer, protection engineer and facility owner. If evidence is inconclusive, state what remains unknown and apply interim controls rather than presenting a guess as fact.

Engineering fixes and mitigation options

Correct the immediate defect

  • Repair or replace damaged bushings, terminations, fans, sensors, controls, seals and contaminated insulation according to the manufacturer’s procedure.
  • Dry and clean the transformer only with an approved method; uncontrolled heating or solvents can create additional damage.
  • Remove overload or rebalance phases while a permanent capacity or cooling solution is designed.
  • Replace a unit with evidence of winding deformation, severe insulation damage or repeated internal discharge when repair cannot restore the required margin.

Control switching transients

Use a circuit-specific study to select mitigation. Documented Eaton options include grounding improvements, surge arresters, RC snubbers and a saturable reactor. The study should account for arrester energy, insulation coordination, breaker operating characteristics, cable capacitance, transformer saturation and the actual switching sequence. Installing a device without modeling can move the resonance or create a new coordination problem.

Coordinate MV and LV protection

Protection must clear faults quickly while remaining selective with downstream devices and transformer inrush. Schneider Electric’s 2024 data-center application note addresses medium- and low-voltage protection for cast-resin and oil-filled transformers up to 4 MVA. That scope is useful for many distribution units but should not be generalized to larger substations; ratings, grounding and network topology require a project-specific design.

Improve grounding and bonding

Verify the transformer neutral, enclosure, cable shields, surge arresters and building grounding network against the approved design. Measure connections and inspect for corrosion or looseness. Grounding changes affect touch voltage, fault current and transient behavior, so have a qualified engineer review the complete protection scheme.

Build a risk-based monitoring program

IEEE C57.167-2023 classifies transformer types, risk exposures, outage impacts and monitoring parameters to support safe, continuous power delivery. Use that framework to decide what to monitor, how often to review it and what alarm requires an outage.

Parameters to trend

  • Winding, core and ambient temperature, plus cooling-fan state.
  • Load current, voltage, power factor, harmonics and phase imbalance.
  • Moisture indicators for the insulation system and enclosure humidity.
  • Vibration and acoustic changes.
  • Partial-discharge activity where sensor placement and noise control make it meaningful.
  • Oil level, temperature and laboratory oil results for oil-filled units.
  • Breaker operations, contact timing, trip-coil current and switching waveform captures.

Schneider Electric describes a connected probe that measures temperature, moisture, vibration and partial-discharge signals and applies analytics. Such a system is useful only when sensors are commissioned, baseline values are recorded, alarms have owners and data is reviewed by people who understand the transformer. A sensor that is never validated can create false confidence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set actions, not just alarms

Define who receives each alarm, the allowable response time, the conditions for load reduction and the criteria for an immediate outage. Review trends after every major switching event, fault, overload period or cooling-system change. Keep a history long enough to distinguish seasonal temperature effects from deterioration.

Commissioning and switching practices that reduce risk

  1. Validate the single-line diagram, transformer nameplate, tap position, grounding configuration and protection settings against the installed equipment.
  2. Complete insulation, ratio, winding-resistance, control, interlock and protection tests before energization, with results retained as the baseline.
  3. Model energization and de-energization, including cable capacitance, voltage-transformer connections, breaker current chop and possible ferroresonance.
  4. Use a written switching sequence with hold points, communications and a clear abort condition. Do not improvise a transfer during commissioning.
  5. Capture high-speed voltage and current data during the first energization and unusual transfer modes when practical.
  6. Inspect for abnormal noise, temperature, relay activity or waveform behavior after each step before proceeding to the next configuration.
  7. Recheck protection coordination after any grounding, arrester, snubber, reactor, cable or transformer change.

Frequent switching is a design condition in a data center, not an exceptional event. Operating procedures, breaker maintenance and transient mitigation therefore belong in the reliability plan rather than in a one-time commissioning file.

Choosing an investigation and protection approach

Compare alternatives using the actual transformer and facility consequence, not a generic dry-type-versus-oil preference.

Decision axis Questions to answer
Transformer technology Is the unit dry-type or oil-filled? What insulation, cooling and fire-containment requirements apply?
Voltage and rating What are the primary and secondary voltages and MVA rating? Does the selected protection guidance cover this size?
Fault environment What short-circuit current, clearing time, arc-flash energy and through-fault history exist?
Switching environment How often do utility, generator, transfer and bypass breakers operate? Are long cables or voltage transformers connected?
Grounding and protection Are neutral grounding, surge arresters, relays and downstream devices coordinated?
Redundancy and maintenance bypass Can the load be transferred while the transformer is tested, repaired or monitored?
Monitoring depth Which parameters justify continuous sensors, periodic testing or laboratory analysis?
Outage consequence What is the cost and safety impact of losing this transformer, and how much test or spare capacity is justified?
Service capability Are qualified high-voltage technicians, transient-study expertise, test equipment and replacement parts available when needed?

Schneider’s protection note covers medium- and low-voltage cast-resin and oil-filled transformers up to 4 MVA. IEEE data-center substation planning addresses substantially larger-load contexts. Keep those scopes separate when selecting equipment or interpreting recommendations.

Return-to-service checklist

  • The failed transformer and connected circuits have been isolated, inspected and repaired or replaced under an approved work package.
  • The investigation report identifies the most probable initiating cause and contributing conditions, with unresolved uncertainties listed.
  • Required acceptance tests have been repeated and compared with manufacturer data and the original baseline.
  • Protection settings, instrument-transformer polarity, interlocks, grounding and coordination have been independently reviewed.
  • Switching-transient and ferroresonance risks have been studied where the event or network configuration warrants it.
  • Mitigation devices, cooling systems, alarms and monitoring sensors have been function-tested.
  • The switching procedure, communications plan, hold points and abort criteria have been updated.
  • Post-energization monitoring has an assigned owner, defined alarm responses and a review schedule.

A transformer should return to service only when the physical repair, electrical tests, protection scheme and operating procedure agree. Replacing a visibly damaged part without correcting the transient, overload, moisture or grounding condition that caused the event leaves the next failure path in place.
