A useful daily health log is small enough to keep and focused on a goal you care about. In about two minutes, note a relevant behavior, symptom or observation, and—only when useful—one measurement. Tracking can help you review patterns and progress; it does not diagnose a condition or replace medical advice.

What should you track each day?

Start with one goal, such as noticing how a routine relates to your energy or keeping a record of a symptom to discuss at an appointment. Choose only entries that help you assess that goal.

  • A behavior: something relevant to your goal, such as whether you took a walk or followed a routine.
  • A symptom or observation: a brief note about what you noticed, using your own words.
  • A measurement, if appropriate: include one only when it supports your goal or is part of a clinician-directed care plan.

NICE advises that activity trackers and food or physical-activity diaries can help people review progress toward diet or activity goals. It also says digital interventions are an adjunct to existing services and that their effectiveness varies. A log is a reflection aid, not proof that a particular behavior caused a change. How to make a two-minute daily check-in

The two-minute limit is a practical habit-design choice, not a medically validated duration or protocol. Keep the routine deliberately brief:

  1. Choose one goal. Decide what you want the log to help you notice or review.
  2. Record a few relevant entries. Note a behavior, symptom or observation, and a measurement only if it serves your goal or care plan.
  3. Use a consistent time when it makes sense. For measurements, follow the method and schedule your care team gave you rather than improvising for convenience.
  4. Review periodically. Look for questions or patterns worth discussing, rather than treating an individual entry as a diagnosis.
  5. Share a concise summary if useful. Bring relevant notes or questions to a clinician.

This is a low-friction personal workflow, not medical advice. If you have prescribed remote monitoring, the clinician’s measurement schedule and alert plan take precedence over this routine.

Paper, app or wearable: which format fits?

Pick the simplest tool that captures the information you actually need. A paper health or symptom-tracking journal, a digital log, or a wearable can each serve different purposes; no particular product is endorsed here.

Consider What to check
Daily effort Can you record the entries without adding a burdensome routine?
Fit for your goal Does it capture the behavior, observation or measurement you intend to review?
Sharing a useful summary Can you export, show or summarize the information for a clinician if needed?
Privacy controls Who operates the service, what does it collect, and can you control sharing, deletion or export?
Clinical suitability For medical monitoring, does the device and measurement method match your care team’s instructions?

This comparison is a practical way to choose a format, not a formal checklist from NICE or HHS. Wearable data also should not automatically be treated as clinically interchangeable or dependable: a 2026 U.S. Government Accountability Office assessment describes potential clinical benefits alongside varying reliability and integration challenges.

When a health measurement is part of your care plan

Home remote monitoring may involve measurements such as blood pressure, heart rate, weight, oxygen level or blood sugar. The equipment and schedule depend on the care plan. HHS advises patients to ask how to use the equipment, how often to measure, what readings are concerning, and whom to contact. Clarify these details with your care team before relying on a log or device alerts. HHS guidance on remote patient monitoring

Do not change medication or treatment based on a self-created tracking routine. Use your clinician’s instructions and contact plan for decisions about readings and care.

Important warning about glucose claims

The FDA’s February 21, 2024 safety communication says not to use smartwatches or smart rings that claim to measure or estimate blood glucose without piercing the skin. The agency says it has not authorized, cleared or approved a smartwatch or smart ring intended to measure or estimate glucose on its own. That warning is different from an app displaying data from an authorized glucose-measuring device. FDA safety communication

How to think about privacy before using a digital log

Check who provides the service, what information it collects, whether sharing is optional, and how to delete or export your data. Legal protections vary with the type of service. The Office of the National Coordinator for Health Information Technology explains that records offered by providers or health plans may be covered by HIPAA rules, while some standalone personal health record services may instead fall under the FTC Health Breach Notification Rule. Do not assume every consumer health app is covered by HIPAA. ONC guidance on health information security

When tracking stops being helpful

More entries are not automatically better. NICE notes that digital tools can contribute to anxiety, unhealthy behaviors such as excessive exercise or disordered eating, and negative mental-health effects for some people. If tracking becomes distressing, simplify or stop the routine and discuss your concerns with a professional. NICE recommendations
