Custom Orthotics vs. Over-the-Counter Hiking Insoles: Which Should You Choose?

If you have no diagnosed foot condition and no persistent pain, a prefabricated over-the-counter (OTC) insole is a reasonable, lower-commitment place to start—provided it fits your hiking boots. Custom orthotics are not automatically more effective: studies in people with plantar heel pain have not shown consistent advantages over prefabricated devices, and those studies do not establish which option works better on hikes. If you have a diagnosis or ongoing or worsening pain, seek an individualized assessment rather than treating an insole as a prescription.

What is the difference?

Both are removable devices worn inside footwear. A custom orthosis is individually contoured from an impression of your foot, such as a cast or 3D scan. A prefabricated OTC insole is mass-produced in foot sizes. Either may cushion or off-load parts of the foot; the label alone does not establish how well a particular device will feel or perform in your boots.

Evidence for plantar heel pain No demonstrated advantage over prefabricated orthoses for pain reduction or functional improvement at 6 weeks, 12 weeks, or 12 months in adults with plantar heel pain; evidence quality ranged from very low to low. What the clinical evidence does—and does not—say

Plantar heel pain findings are not hiking trials

A 2018 systematic review and meta-analysis found no difference between customised and prefabricated orthoses for pain or function in people with plantar heel pain. It also found moderate-quality evidence that orthoses reduced pain more than sham supports in the medium term, defined in that review as 7–12 weeks. That comparison with sham does not show that custom devices outperform OTC insoles, and it does not measure trail performance. British Journal of Sports Medicine (2018)

In a separate randomized trial of 185 people with plantar heel pain, referral for custom insoles did not produce better outcomes at 12 weeks than sham insoles or GP-led usual care; 176 participants completed the 12-week follow-up. The trial concerns that condition and those treatment comparisons, not hiking. STAP study (2020)

Results differ by diagnosis

The 2022 Cochrane review covers several foot-pain diagnoses and reports condition-specific findings rather than a universal benefit from custom orthoses. Its summary includes evidence on painful pes cavus, rheumatoid arthritis, juvenile idiopathic arthritis, plantar fasciitis, and hallux valgus; results versus non-custom supports vary by condition. The review’s evidence base comprised 11 trials involving 1,332 participants, not a hiking-specific sample. Cochrane (2022)

Reported adverse effects include additional foot pain, ankle instability, and skin irritation, but the evidence about complications is imprecise. A device that causes new or worsening symptoms deserves attention rather than being assumed to be a necessary adjustment period. Cochrane (2022)

Hiking Boot Insoles for Men & Women - Arch Support Inserts
  • Hiking Insoles for Men and Women: The arch is medium and cushioned, not stiff. They are soft enough for comfort over ten miles, but firm enough to keep your foot steady on rocks and roots. You will really notice the difference on long downhills
  • Up to 56% Shock Absorption: When you carry a full pack, every step can feel heavier. Our shock layer takes in up to 56% of impact before it reaches your knees and lower back, so you stay comfortable all day
  • Up to 47% Energy Return: The arch returns up to 47% of the energy you put into each step, rather than just absorbing it. It also helps take pressure off your knees when you walk downhill. By the end of a long day, your legs will feel the difference
  • Dry Feet on Tough Climbs: A breathable cover and non-sweat layer pull heat and moisture away from your skin, keeping your boots drier on steep climbs and in warm weather. Wet feet cause blisters, and this helps prevent them
  • Sized for Men and Women who Hike: Use the size chart to find your fit. Place your boots' insoles on top and trim around the front for an exact fit. These insoles are light enough for trail runners and tough enough for backpacking boots

Which should you choose?

No diagnosis and no persistent pain

Consider a prefabricated OTC insole as a lower-commitment first option if it fits your intended boots and feels comfortable. The evidence does not justify choosing a particular arch shape, firmness, or brand as a way to prevent hiking injuries.

Diagnosed condition, persistent pain, or worsening symptoms

Ask a qualified foot-health clinician for an individualized assessment. The evidence varies by diagnosis, so findings for plantar heel pain—or for another condition—should not be treated as a prescription for every hiker. The Cochrane review describes the evidence for clinical decisions about custom-made orthoses as limited. Cochrane (2022)

Trying an insole in hiking boots

  • Evaluate the insole in the actual boots you plan to wear; comfort and fit are practical considerations, not proof of improved trail performance.
  • Do not assume that a custom device will work better simply because it is individually made.
  • The cited clinical sources do not establish a hiking-specific break-in schedule, product ranking, or validated boot-fitting protocol.
  • If an insole brings additional foot pain, ankle instability, or skin irritation, stop treating discomfort as evidence that the device is helping and seek appropriate advice if symptoms persist or worsen.

The practical answer

For an adult without a diagnosis or persistent pain, start by considering an OTC insole that fits the intended boot. Choose custom orthoses through individualized clinical assessment when a diagnosed condition or ongoing symptoms make that appropriate—not on the assumption that custom means better. Neither option has been shown by the cited evidence to be superior for hiking itself.
