Short answer: Cuffless blood-pressure devices are closer to a common validation framework, but they are not yet established substitutes for a validated upper-arm cuff when diagnosing or managing hypertension. New FDA draft guidance, IEEE 1708-2025 and European recommendations make testing more explicit; they do not turn regulatory clearance into proof of clinical accuracy.
What changed for cuffless blood-pressure devices
The field moved forward in 2025 and 2026 because regulators and standards bodies are describing more specific ways to test devices that estimate blood pressure without an inflatable cuff.

FDA draft guidance published in January 2026
The U.S. Food and Drug Administration’s January 2026 draft guidance recommends submitting clinical-performance data from Static, Stability and Change tests when those tests apply to a device’s intended use. It also directs sponsors to use the clinical-investigation methods in ISO 81060-2. The recommendation is for accuracy consistent with the device’s intended use, including measurements taken while users are seated and resting.

It describes what sponsors should present to support a submission; it is not a declaration that every cuffless product already meets those methods.

IEEE 1708-2025
IEEE 1708-2025 supplies a normative definition and an objective evaluation framework for wearable cuffless blood-pressure devices. It covers intermittent readings, continuous monitoring, beat-to-beat measurements and blood-pressure-variability outputs. The standard therefore treats the claimed operating mode as part of the validation question, rather than assuming one test can support every type of result. The complete standard may require purchase to access.
ESH recommendations from 2023
The European Society of Hypertension (ESH) set out six tests for intermittent devices:
- Static accuracy
- Device position
- Treatment-induced decrease
- Awake-versus-asleep change
- Exercise increase
- Recalibration stability
Together, these tests reflect the conditions in which a wearable might be used, not just a quiet, seated reading taken under ideal conditions.

Why “FDA-cleared” does not answer the accuracy question
An FDA clearance indicates that a device met the requirements for the particular submission and intended use reviewed by the agency. It does not establish that every reading will be accurate in routine life, or that the device is suitable for diagnosing and treating hypertension.
The American Heart Association’s scientific statement puts the distinction plainly: “FDA clearance of cuffless devices does not mean they will prove accurate enough for clinical use.” Clearance and clinical usefulness are related but separate questions. A reader should still ask what population, operating mode, reference method and conditions were actually tested.
Validation depends on how the device is meant to work
A device that produces an occasional spot check has a different evidentiary burden from one claiming continuous or beat-to-beat blood pressure. A credible evaluation should match the claim being made.

| Validation question | Why it matters |
|---|---|
| Measurement mode | Intermittent spot checks, continuous monitoring, beat-to-beat values and blood-pressure-variability outputs require different evaluation approaches. |
| Intended output | Absolute blood-pressure values are not the same claim as detecting a rise, fall or other change. |
| Calibration and drift | The evidence should state whether calibration is required, how it is performed and how stable results remain after calibration. |
| Reference method | The cuffless result must be compared with an appropriate reference method under a defined protocol, including ISO 81060-2 methods where applicable. |
| Position and activity | Body position, exercise, sleep and wakefulness can change performance; a seated-resting result cannot automatically be generalized to movement or sleep. |
| Population coverage | Age, health status and other characteristics of the tested participants determine how broadly the findings can be applied. |
| Motion robustness | Movement can degrade a signal or alter the estimate, so claims should identify whether motion was tested. |
| Evidence quality | Peer-reviewed, protocol-compliant results are more informative than an unspecified internal test or a marketing accuracy number. |
What the newer tests are trying to catch
Static accuracy
Static testing asks whether readings agree with a reference when conditions are controlled. It is necessary, but it does not show how a device behaves when the user changes position, exercises or goes to sleep.
Stability and recalibration drift
A cuffless estimate may depend on an initial calibration. Stability testing checks whether performance remains acceptable over time rather than quietly drifting away from the reference. ESH specifically lists recalibration stability for intermittent devices.
Physiological change
Change testing examines whether a device tracks a meaningful increase or decrease, including treatment-induced decreases and the rise associated with exercise. A device can show a plausible average while still missing the direction or size of a clinically important change.

Position and sleep state
ESH’s device-position and awake/asleep-change tests address situations that a seated office measurement does not cover. Results from one body position or state should not be presented as proof for all others.

Not for routine diagnosis or hypertension management based on the evidence summarized by the AHA. The statement notes that the field still lacks evidence connecting the very large volume of cuffless readings to better outcomes or to treatment decisions that clinicians can act on. More readings are not automatically better readings, and they do not by themselves show that treatment should be started, stopped or changed.

A smartwatch or other wearable can be useful for research, trend observation or a device-specific use that has been properly evaluated. It should not be treated as a universal replacement for a conventional cuff simply because it displays a number, carries a health claim or has received FDA clearance.

What to use at home while cuffless devices are evaluated
For a physical monitor, the practical reference remains a validated upper-arm automated blood-pressure monitor. The searchable product phrase “upper arm blood pressure monitor” describes the category; it does not certify any particular listing.
- Choose an upper-arm model whose model-specific clinical-validation evidence is available, rather than relying only on a storefront description.
- Use the monitor according to its instructions, including the correct cuff size and a seated, resting measurement when you need a comparable reading.
- If you are checking a cuffless reading, record the date, time, posture and activity so the two measurements are not compared as if they were taken under identical conditions when they were not.
- Do not use an unexplained cuffless result alone to diagnose hypertension or change medication. Discuss repeated or concerning readings with a qualified clinician and use the conventional monitor as the comparison device.
How to judge a product claim
Before accepting an accuracy claim, look for a published protocol and answers to these questions: Was the device tested in the mode you intend to use? Were absolute values or only changes evaluated? What reference method was used? Were position, exercise, sleep, motion and recalibration included? Which participants were studied? Is the work peer-reviewed and consistent with the relevant standard or guidance?
If those details are missing, the claim may describe an engineering demonstration rather than clinical validation. The absence of a detail is itself a reason to avoid assuming that the result applies to you.
What “one step closer” really means
The important advance is methodological: sponsors now have clearer expectations for matching tests to intended use, and standards provide a common vocabulary for comparing devices. The unresolved step is clinical proof that cuffless readings remain accurate across real-world conditions and lead to better, safer decisions. Until that evidence exists, use cuffless measurements as an emerging technology under evaluation, not as a replacement for the upper-arm cuff used for dependable home monitoring.
