Short answer: COX inhibitors remain important medicines, and scientists are still investigating more selective COX-2 compounds. But the available evidence does not establish a named new drug, approval, or clinical milestone that would prove a present-day “comeback.” The story is better understood as continued development around an old trade-off: reducing some gastrointestinal injury without removing cardiovascular and kidney risks.
What “comeback” means in this context
“COX inhibitors” are not one medicine. The term covers nonsteroidal anti-inflammatory drugs (NSAIDs), including nonselective drugs that inhibit both major cyclooxygenase enzymes and selective COX-2 inhibitors designed to act more heavily on COX-2. A 2024 review in RSC Advances describes a long history of NSAID development and continuing work toward an improved COX-2 inhibitor lead.

That continuing work is not the same as a confirmed return of a withdrawn product or a new wave of approvals. The evidence available for this topic does not identify a specific candidate that has recently returned to clinical practice, nor does it document a new approval or other milestone that would justify presenting a comeback as an established event. “Comeback” therefore describes an expectation or framing, not a demonstrated change in medical practice.

Cyclooxygenase enzymes help convert fatty acids into prostaglandins and related signaling molecules. Those molecules contribute to pain, fever, inflammation, protection of the stomach lining, regulation of blood flow through the kidneys and other processes. NSAIDs reduce prostaglandin production by inhibiting cyclooxygenase.

Nonselective NSAIDs
Nonselective NSAIDs inhibit both COX-1 and COX-2 to varying degrees. Ibuprofen is a familiar example in this category, and it was one of the three medicines compared in the large PRECISION arthritis trial. Naproxen is another nonselective comparator used in that study. The balance between pain relief and adverse effects depends on dose, duration, the patient’s baseline risks and the other medicines being taken.
Selective COX-2 inhibitors
Selective COX-2 inhibitors were developed to inhibit the enzyme more closely associated with inflammatory signaling while sparing more COX-1 activity. Celecoxib is the selective COX-2 medicine tested in PRECISION. Selectivity is relative rather than absolute: dose can change how strongly a drug affects each pathway, and no selectivity profile removes every prostaglandin-related effect.
Why COX-2 selectivity was pursued
The gastrointestinal rationale
Some COX-1 activity supports protective functions in the stomach and intestines. Researchers hoped that reducing COX-1 inhibition would lower ulceration and gastrointestinal bleeding compared with older nonselective NSAIDs. That potential tolerability advantage was a central reason for developing selective COX-2 drugs.

A selective drug can still cause gastrointestinal complications, especially at higher doses, with prolonged use or in people who already have ulcer or bleeding risks. Individual outcomes also depend on concomitant medicines and medical history.

The cardiovascular and kidney trade-offs
COX-2 selectivity does not eliminate cardiovascular or renal concerns. Changes in prostaglandin signaling can affect vascular balance, fluid retention and kidney blood flow. Cardiovascular events, elevated blood pressure, edema and kidney injury remain relevant safety outcomes for the class. The risk is shaped by the particular drug, dose, treatment duration and patient population.
A 2022 review by Carl-Olav Stiller and Paul Hjemdahl, titled “Lessons from 20 years with COX-2 inhibitors: Importance of dose-response considerations and fair play in comparative trials,” stresses that conclusions depend on dose-response relationships and genuinely fair comparisons. Treating selectivity as a simple safety label misses that context.
What the PRECISION trial found
PRECISION was a randomized trial in patients with arthritis that compared celecoxib, naproxen and ibuprofen. In the intention-to-treat analysis, the primary cardiovascular composite occurred in the following proportions:

| Assigned medicine | COX profile | Primary cardiovascular composite |
|---|---|---|
| Celecoxib | Selective COX-2 inhibitor | 2.3% |
| Naproxen | Nonselective NSAID | 2.5% |
| Ibuprofen | Nonselective NSAID | 2.7% |
The trial reported celecoxib as noninferior to naproxen and ibuprofen for that primary outcome comparison. These percentages describe the study’s intention-to-treat population, treatment conditions, endpoint and noninferiority design. They are not individualized risk estimates and do not mean that any of the three medicines is risk-free.

What the comparison can and cannot show
- It provides a large randomized comparison of cardiovascular safety among the three assigned treatments in an arthritis population.
- It does not prove that celecoxib is the safest NSAID for every patient, dose or duration.
- It does not establish that naproxen or ibuprofen has the same risk profile at different doses or in a different population.
- It does not demonstrate that selective COX-2 inhibitors have returned as a new class or that an unnamed candidate has gained approval.
The result is therefore relevant to the historical safety debate, but it is not evidence of a blanket clinical victory for COX-2 selectivity.
Why dose and trial design change the interpretation
Comparing NSAIDs fairly requires more than listing drug names. Dose, exposure time, adherence, background treatment and the outcome definition all influence the apparent balance of benefits and harms. A cardiovascular composite may combine several events that have different clinical importance. A noninferiority result means the prespecified margin was met under the trial’s design; it does not mean the treatments are identical in every respect.
The 2022 review’s emphasis on dose-response is especially important. A drug’s effects at a lower approved dose cannot automatically be generalized to higher doses, chronic use or off-label regimens. Nor can one trial settle gastrointestinal, cardiovascular and renal questions simultaneously. Each outcome needs its own analysis, with the relevant comparator and follow-up period visible.

What has not been established
The available evidence does not name a current candidate that has returned to routine clinical use, received a newly documented approval or reached a clearly defined milestone that would substantiate the headline’s comeback claim. Current regulatory status and geographic availability for any hypothetical returning candidate are therefore unresolved here. A future announcement would need to be assessed on its own clinical data, dose, indication, regulator and region.
In practical terms, the present story is evolution, not a verified reversal of the concerns that surrounded earlier COX-2 drugs. Any claim of a comeback should specify which compound, what indication, which regulator and what evidence changed the risk-benefit assessment.

Gastrointestinal outcomes
Selective COX-2 inhibition may reduce some gastrointestinal injury relative to a nonselective NSAID in certain settings, which was the original development rationale. The size of any advantage depends on the comparator, dose and use of protective medicines. A history of ulcers or bleeding, older age and concurrent drugs that increase bleeding risk can materially change the decision.

Cardiovascular risk remains a central concern for all NSAID decisions. The PRECISION rates—2.3% for celecoxib, 2.5% for naproxen and 2.7% for ibuprofen—come from one randomized arthritis trial and one prespecified composite. Renal outcomes and fluid balance
NSAIDs can affect kidney blood flow and salt and water handling. People with reduced kidney function, heart failure, uncontrolled hypertension or conditions that make them volume-depleted may face greater hazards. Monitoring needs depend on the person’s condition and treatment plan; symptoms such as swelling, sudden weight gain or markedly reduced urine output warrant prompt medical attention.
A clinician’s decision framework
For someone considering an NSAID, the question is not whether COX-2 selectivity is universally good or bad. A clinician typically weighs the following in sequence:
- Confirm the indication. Identify the pain or inflammatory condition and whether an NSAID is necessary at all.
- Review baseline risks. Discuss prior ulcers or gastrointestinal bleeding, cardiovascular disease, high blood pressure, kidney disease, heart failure and relevant allergies.
- List interacting medicines. Anticoagulants, antiplatelet drugs, corticosteroids, certain blood-pressure medicines and other treatments can alter bleeding, kidney or blood-pressure risk.
- Choose the lowest effective exposure. Dose and duration should match the clinical goal rather than continue automatically.
- Set monitoring and stop rules. Agree on what symptoms, laboratory changes or lack of benefit should trigger reassessment.
- Reevaluate the comparator. If a selective COX-2 inhibitor is considered, compare it with the specific nonselective alternative at the intended dose and for the intended duration, not with an abstract class average.
These steps are general decision principles, not a recommendation to start, stop or switch a medicine without professional guidance. Ibuprofen tablets, for example, are a nonselective NSAID and should not be treated as interchangeable with celecoxib simply because both appeared in PRECISION.
What would count as convincing evidence of a real comeback?
A credible comeback claim would need more than a promising laboratory compound or a press release. Readers should look for:
- A named drug and clearly defined molecular or COX-2 selectivity profile.
- Peer-reviewed human trials showing benefits and harms at the proposed dose and duration.
- A comparator chosen to answer the clinical question fairly, with cardiovascular, gastrointestinal and renal outcomes reported separately where appropriate.
- A regulator’s decision identifying the indication, approved dose, warnings and geographic scope.
- Evidence that the benefit applies to the patients who would actually receive the medicine, rather than only to a narrow experimental group.
Until those elements are documented for a specific product, “comeback” should be treated as a description of renewed interest in the field, not proof that the old safety debate has been settled.
Bottom line for readers
COX inhibitors remain clinically relevant, and selective COX-2 research is continuing. The reason for pursuing selectivity—potentially less gastrointestinal injury than some nonselective NSAIDs—still has medical logic, but cardiovascular and kidney risks remain. PRECISION found similar primary cardiovascular composite rates for celecoxib, naproxen and ibuprofen in its intention-to-treat arthritis population, with celecoxib meeting noninferiority comparisons. That result is informative but limited to its design and participants. No specific new approval or clinical milestone in the available evidence proves that COX inhibitors have staged a broad comeback.
