Glucocorticoid signaling is required for normal energy regulation and reproductive biology, including placental and fetal endocrine function. The concern is sustained or excessive hypothalamic-pituitary-adrenal (HPA) activity, which can suppress reproductive signaling in some circumstances. A stressful day or one abnormal cortisol result does not establish infertility.
What cortisol does in reproductive biology
It supports normal physiology
Cortisol is a glucocorticoid made by the adrenal glands. At physiologic levels, glucocorticoids help keep energy available, coordinate immune and metabolic responses, and participate in placental and fetal endocrine development. Reproductive tissues therefore need regulated cortisol signaling rather than its complete suppression.

The relevant system is the HPA axis: corticotropin-releasing hormone (CRH) stimulates adrenocorticotropic hormone (ACTH), which prompts the adrenal glands to release glucocorticoids, including cortisol. This system communicates with the hypothalamic-pituitary-gonadal (HPG) axis that controls reproductive hormones.

Reviews describe several routes by which sustained stress signaling may affect reproduction. Elevated glucocorticoid exposure can reduce hypothalamic gonadotropin-releasing hormone (GnRH) signaling, alter pituitary luteinizing hormone (LH) and follicle-stimulating hormone (FSH) release, and act directly on ovarian or testicular steroid production. These effects depend on the hormone level, duration, timing, tissue and feedback state; they are not a universal response to every cortisol rise.

“The stress system has suppressive effects on female and male reproductive function.”
— S. N. Kalantaridou and colleagues, 2010 review
CRH also has local reproductive jobs
CRH is produced in reproductive tissues, not only in the brain. Ovarian and endometrial CRH participate in inflammatory and steroidogenic processes. In the placenta, CRH contributes to signaling related to fetal-maternal interaction, immune tolerance and the timing of labor. That local activity is one reason a blanket instruction to “lower cortisol” is biologically inappropriate.
When cortisol is adaptive versus potentially disruptive
There is no single cortisol cutoff that separates healthy from fertility-threatening levels for everyone or for every stage of pregnancy. Interpretation requires several dimensions:

| Question | Adaptive or physiologic pattern | Potentially disruptive pattern |
|---|---|---|
| Magnitude and duration | Regulated increases that resolve as the demand ends | Persistently high or excessive exposure |
| Timing | Aligned with normal daily rhythms and reproductive stage | Exposure that coincides with vulnerable windows such as ovulation or implantation, or remains abnormal over time |
| Daily pattern and assay | A result interpreted with its collection time and measurement method | An apparently abnormal value considered without diurnal timing or assay context |
| Feedback control | HPA and HPG systems continue to regulate one another | Evidence of disrupted feedback, altered GnRH/LH/FSH signaling or a diagnosed endocrine disorder |
| Clinical context | No persistent cycle or reproductive symptoms | Cycle loss, an endocrine illness, medication-related glucocorticoid exposure or a pregnancy complication |
This framework describes patterns rather than diagnosing an individual. Stress-related mechanisms are biologically plausible, but plausibility is not proof that ordinary stress caused a particular person’s infertility.
Why cortisol normally rises in pregnancy
Pregnancy changes the maternal HPA axis. Placental CRH rises later in gestation and contributes to physiologic maternal hypercortisolism. The increase supports placental signaling and helps coordinate the endocrine environment involved in fetal development and labor. A higher cortisol measurement during pregnancy therefore cannot automatically be labeled pathological; the gestational stage and the laboratory method are essential to interpretation.
Placental CRH is part of a tightly regulated maternal-fetal system. Its presence does not mean that every increase in maternal stress or cortisol is harmless, but it does show why “high cortisol” has a different meaning in late pregnancy than it might outside pregnancy.

What human studies show about stress, cortisol and fertility
Human evidence is mixed. A 2023 scoping review by Rinne, Hartstein, Guardino and Dunkel Schetter synthesized 48 papers examining stress before or during pregnancy and maternal cortisol. Childhood stress showed more consistent associations with cortisol-awakening responses and altered diurnal patterns across pregnancy. Findings for stress close to conception and for prenatal stress were often null or inconsistent.
The studies also differed according to moderators such as social support and environmental pollution. Those differences make it difficult to convert an association in one group into a prediction for every person trying to conceive.
A separate 2023 systematic review focused on infertility and cortisol likewise reflects heterogeneous human designs and cortisol measurements. Taken together, these reviews support possible pathways between chronic stress biology and reproductive function, but they do not show that everyday stress alone causes infertility or that reducing a measured cortisol value will restore fertility.

Can a cortisol test explain difficulty conceiving?
Why one result is not diagnostic
A single cortisol result cannot diagnose infertility or prove that stress caused it. Cortisol changes across the day, and interpretation depends on the assay matrix, collection time, pregnancy stage and clinical circumstances. A value obtained at one time point may not represent the person’s overall exposure or HPA-axis regulation.
Even repeated measurements should be interpreted alongside menstrual or ovulatory history, reproductive hormones, medications and symptoms. A cortisol test is not a stand-alone explanation for absent ovulation, reduced sperm production, implantation problems or pregnancy loss.

When to seek clinical evaluation
Arrange medical assessment for persistent loss of periods, suspected endocrine disease, infertility, pregnancy complications or possible medication-related glucocorticoid exposure. Clinicians can decide whether cortisol testing is appropriate and select the method and timing that fit the question. The underlying reproductive evaluation should not be replaced by a home cortisol result.
What to do with the stress question
- Do not try to eliminate cortisol indiscriminately; normal glucocorticoid signaling is necessary.
- Do not self-treat with cortisol supplements, “adaptogens” or other products marketed to manipulate the HPA axis.
- Treat persistent cycle changes, endocrine symptoms and fertility problems as clinical issues requiring appropriate evaluation.
- Use stress-support measures for wellbeing, sleep and coping, not as proof that stress is the sole cause of infertility.
The practical answer
Cortisol is both a reproductive participant and, under sustained or excessive HPA activation, a potential reproductive brake. Its effects depend on dose, duration, timing, tissue and feedback context. Pregnancy itself normally raises maternal HPA activity through placental CRH, so a higher value is not automatically a problem. Because human studies are inconsistent and cortisol testing is highly context-dependent, neither stress nor one laboratory result can explain fertility on its own.
