Cork University Maternity Hospital’s DOMINO service is reported to be changing amid staffing pressure, but the available information does not show that the whole scheme has closed. The HSE says midwifery-led community care will continue. The distinction matters: changes to how the service operates could still affect expectant parents, even if community care remains.
What the DOMINO scheme is
DOMINO stands for “Domiciliary In and Out.” In Ireland, it generally describes a hospital-based community-midwifery model that can provide care through pregnancy, birth and the early postnatal period. The exact arrangements vary between hospitals, and services usually operate within defined geographic boundaries.
The National Maternity Strategy says antenatal visits may take place in hospital or in the community. After birth, women generally transfer home within 12–24 hours, where community midwives continue postnatal care for the first few days. These are general features of the model, not confirmation of CUMH’s current protocol.
CUMH’s regional directorate annual report says its DOMINO model has been in place since 2014 and supports low-risk women toward natural birth in line with the national strategy. Nationally, the HSE recorded 2,297 DOMINO births in 2014, or 3.35% of births that year; that historical figure is not a current CUMH total. What is reported to be changing at CUMH
In an Irish Examiner opinion article published 2 October 2026, former service user Sarah Thatt-Foley describes her 2021 experience: support from a DOMINO midwife during a long labour, transfer into hospital, and follow-up at home. She says staffing was given as the reason for current changes. The article attributes to the Irish Nurses and Midwives Organisation (INMO) a figure of more than 80 nursing and midwifery vacancies at CUMH; the HSE service listing does not independently verify that number. A contemporaneous Irish Examiner news report says changes had been shared with expectant mothers. The HSE told the paper: “The Domino scheme will continue to provide midwifery-led community care, with patient safety remaining the overriding priority.” Social Democrats health spokesperson Pádraig Rice called for expansion rather than reduction. The HSE’s CUMH listing still identifies DOMINO and gives Tuesday, Thursday or Friday as opening days, but it does not explain the reported changes or state current eligibility, catchment, capacity or staffing information. A listing is not proof that every previous feature remains unchanged. (HSE CUMH DOMINO listing)
What the evidence says about continuity midwifery
A 2016 Cochrane review pooled trials of midwife-led continuity models, which are broader than CUMH’s particular DOMINO service. It found the following average results across the included studies:

These pooled findings support potential benefits for continuity models generally; they are not an evaluation of CUMH’s DOMINO scheme and do not establish its local outcomes. In particular, the review did not find a statistically significant difference in caesarean birth. It also notes that the best way to organize continuity services varies and needs further study. Thatt-Foley’s account gives one person’s perspective, not a clinical evaluation, but it illustrates what continuity can mean to a patient: support from a midwife across different stages of care, including transfer to hospital and follow-up at home. The broader trial evidence offers reason to take continuity models seriously, while not proving what CUMH’s scheme achieves on its own.

For families making plans, the unresolved practical details matter most. The public HSE listing does not say which elements changed, when changes began, how many people are affected, whether new enrolments are restricted, or what current catchment and eligibility rules apply. Anyone considering the service should ask CUMH or the HSE directly for its current criteria and arrangements rather than assume that the historical model description still applies.
- Ask whether new enrolments are being accepted and what eligibility criteria apply.
- Confirm the current catchment area and where antenatal, birth and postnatal care take place.
- Ask how continuity is organized and what happens if a hospital transfer or escalation is needed.
The strongest public case for keeping DOMINO is not that every detail must remain untouched regardless of staffing or safety. It is that a service with potential value to families should not be reduced without clear information about what is changing, why, and how equivalent continuity of care will be maintained.
