The most useful CSP response to a compromised website is a restrictive, browser-enforced allow-list: deny capabilities by default, authorize only the scripts and destinations the application actually needs, and control outbound connections, forms, frames and legacy objects. A practical starting point is default-src 'self'; object-src 'none'; base-uri 'none'; frame-ancestors 'none'; form-action 'self'; script-src 'self' 'nonce-{per-request-random-value}'; style-src 'self'; img-src 'self' data:; font-src 'self'; connect-src 'self'; frame-src 'none', with violation reporting enabled.
CSP limits what injected or altered page content can do in visitors’ browsers. It does not remove the attacker from your server, restore modified files or replace incident response. Treat the policy below as a starting pattern: inventory the application, add only demonstrably required origins, test in report-only mode, then enforce the reviewed policy.

What CSP can and cannot do after a compromise
Content Security Policy is delivered primarily in the HTTP Content-Security-Policy response header. The browser evaluates that policy before loading or executing many classes of content. You can restrict script execution, styles, images, fonts, frames, network connections, form submissions and embedded objects.

If an attacker injects markup into a page, a tight policy can prevent that markup from loading a remote script, opening a WebSocket to an unapproved host, posting a login form to an attacker’s server or embedding a legacy plugin. This reduces the capabilities available to the compromised page in each user’s browser.

CSP is not a server-side repair. A stolen administrator account, malicious server process, altered database record or vulnerable application remains compromised until you investigate and remediate it. Use CSP as a containment layer while you rotate credentials, remove unauthorized code, patch the underlying flaw and verify the deployment.
No authoritative source establishes a universal percentage reduction in harm from CSP after a compromise. Its value depends on the policy’s coverage, the browser, the application’s legitimate dependencies and whether the attacker can use an already-authorized origin.
A restrictive baseline policy
The following header expresses a deny-by-default posture while allowing same-origin resources and intentional inline scripts identified with a per-response nonce:

Content-Security-Policy: default-src 'self'; object-src 'none'; base-uri 'none'; frame-ancestors 'none'; form-action 'self'; script-src 'self' 'nonce-{per-request-random-value}'; style-src 'self'; img-src 'self' data:; font-src 'self'; connect-src 'self'; frame-src 'none'; report-to csp-endpoint; report-uri /csp-report
Do not copy this unchanged into every site. Replace or remove directives according to actual application behavior. Generate a fresh, unpredictable nonce for every response that contains nonce-authorized script, and place the same value on only the intended inline <script> elements. Never use a fixed nonce.
| Directive | What the baseline does | When to change it |
|---|---|---|
default-src |
Uses the same-origin set as the fallback for fetch directives. | Keep restrictive; override only a specific resource type that needs another origin. |
script-src |
Allows same-origin scripts and nonce-authorized inline scripts. | Add narrowly scoped origins, hashes or nonces for known code. Avoid broad keywords. |
connect-src |
Limits fetch, XHR, EventSource and WebSocket destinations to the site itself. | Add each required API or socket origin explicitly. |
object-src 'none' |
Disables plugin-style object and embed loads. | Change only if the application genuinely needs such content. |
base-uri 'none' |
Blocks attacker-controlled <base> elements. |
Use a narrow origin if the application deliberately relies on a base URL. |
form-action 'self' |
Allows forms to submit only back to the site. | List a specific payment, identity or other destination when required. |
frame-ancestors 'none' |
Prevents other origins from embedding the page. | Use a narrow allow-list for an approved portal, dashboard or integration. |
frame-src 'none' |
Prevents the page from loading child frames. | Add only the origins of required embedded content. |
style-src, img-src, font-src |
Allows same-origin styles, images from the site plus data:, and same-origin fonts. |
Add a specific CDN or asset host when inventory proves it is needed. |
report-to and report-uri |
Sends policy violations to a named reporting destination and a legacy-compatible path. | Configure a controlled collector and retain report-uri during browser-compatibility transition. |
How each control limits an attacker
default-src: establish the fallback
default-src 'self' supplies a restrictive fallback for fetch directives that you have not separately declared. It prevents an omitted directive from silently becoming permissive. Explicit directives still take precedence, so review every override rather than assuming the fallback covers it.
script-src: make code execution intentional
Scripts are usually the highest-impact capability in an injected page. script-src 'self' permits same-origin files, while a nonce authorizes only the inline blocks carrying that unpredictable value. A hash can authorize a specific inline block when its contents are stable. Both approaches are safer than allowing every inline script.

Avoid 'unsafe-inline' and 'unsafe-eval' unless a documented compatibility requirement leaves no alternative. 'unsafe-inline' broadly re-enables inline execution, and 'unsafe-eval' permits string-to-code evaluation patterns. If a legacy dependency needs one, isolate the exception, record why it exists and plan its removal.
connect-src: close common exfiltration paths
connect-src governs browser connections such as fetch and XHR requests, EventSource streams and WebSockets. Keeping it at 'self' blocks injected code from sending data directly to arbitrary domains. Add each legitimate API origin individually; do not replace the list with a broad wildcard merely to silence reports.
object-src and base-uri: remove legacy and URL-rewriting surprises
object-src 'none' disables plugin-style objects when the site has no business need for them. base-uri 'none' prevents an injected <base> element from changing how relative links and resource URLs resolve. These small directives eliminate attack surface that is easy to overlook because modern applications rarely use either feature intentionally.

form-action: protect credential and data submissions
form-action 'self' restricts where forms can submit. If checkout, identity or support workflows post to another origin, list only that exact destination. This control is separate from connect-src; allowing an API connection does not automatically authorize a form submission, and vice versa.
frame-ancestors versus frame-src
These directives protect opposite directions. frame-ancestors controls which origins may embed your page and is the CSP control for clickjacking exposure. frame-src controls which frames your page may load. A site can legitimately allow a child frame while refusing to be embedded itself, so configure them independently.
Styles, images and fonts
Keep style-src and font-src narrow because injected styles can alter user-visible controls and third-party assets expand trust. The baseline permits data: images to accommodate data-URL assets; retain that scheme only if the application uses it. Otherwise remove it and allow images from explicit origins.
Best Value
Build the allow-list from the application, not from guesses
- Inventory every resource class. Record scripts, inline blocks, API calls, WebSockets, frames, fonts, images, styles and form destinations for authenticated and unauthenticated pages. Include error pages and administrative screens.
- Separate first-party and third-party origins. A vendor host used for analytics, payments or video should be listed only for the directive that needs it. An origin needed for images does not automatically belong in
script-srcorconnect-src. - Replace broad script exceptions. Add nonces for intentional per-response inline code or hashes for stable inline code. Remove dead scripts and inline event handlers instead of authorizing them wholesale.
- Document compatibility exceptions. If a framework requires an unsafe keyword, identify the affected pages, the reason, the owner and the removal plan. This turns a permanent blind spot into a tracked trade-off.
- Keep the policy readable. Group directives consistently, use one origin spelling, and review changes as security-sensitive configuration. A shorter policy is not automatically safer; an explicit, necessary origin is safer than an undocumented wildcard.
Roll out CSP without taking the site down
The W3C CSP specification says the Content-Security-Policy-Report-Only response header lets servers experiment by monitoring a policy rather than enforcing it. MDN likewise advises testing with that header before deploying an enforcing policy.
- Send report-only first. Return the proposed policy in
Content-Security-Policy-Report-Onlywhile the existing site continues to serve. Do not assume a quiet report stream means complete coverage; exercise important user journeys. - Collect reports at a controlled endpoint. Configure the named
report-togroup and a collector for/csp-report. Restrict access, rate-limit requests and avoid storing sensitive request data unnecessarily. Reports are input for tuning, not proof that every violation is malicious. - Remove noise and classify findings. Mark each report as required application behavior, obsolete dependency, deployment error or suspicious activity. Fix the application or narrow the policy rather than adding every reported host.
- Add nonces or hashes. Update templates so intentional inline code receives the response nonce, or hash stable blocks. Verify that the nonce is unpredictable and changes on each response.
- Enforce the reviewed policy. Switch to
Content-Security-Policyafter critical flows work under report-only. Keep reporting enabled so later regressions and newly introduced dependencies remain visible.
Maintain a short overlap in which report-only observations continue after enforcement. A deployment can pass a static review yet fail when a rarely used export, password-reset page or regional integration is exercised.
Reporting details and compatibility
report-to is the current reporting mechanism and requires a corresponding Reporting-Endpoints configuration that maps the group name, such as csp-endpoint, to your collector. Keep report-uri during a compatibility period when older browsers matter; it provides a fallback path for clients that do not implement the newer mechanism.
Recommended Free Tools
Protect the collector from becoming an attack surface. Accept reports without trusting their claimed origin, cap body size, discard or redact sensitive fields, and monitor spikes. A compromised page can generate large volumes of reports, so operational controls matter as much as the directive syntax.
Common breakages and precise fixes
| Symptom | Likely cause | Safer fix |
|---|---|---|
| Inline script is refused | The block has no nonce or matching hash. | Add a per-response nonce or a hash for stable code; do not add 'unsafe-inline' by default. |
| Framework feature reports an eval violation | A dependency constructs code from strings. | Upgrade or reconfigure the dependency; use 'unsafe-eval' only as a documented temporary exception. |
| API calls fail | The destination is absent from connect-src. |
Add the exact required API or WebSocket origin and retest authentication flows. |
| Embedded payment, video or identity frame is blank | The child origin is blocked by frame-src. |
Add that specific origin to frame-src; do not weaken frame-ancestors unless others must embed your page. |
| Form submission is blocked | The target is outside form-action 'self'. |
List the approved destination in form-action and verify the complete redirect flow. |
| Images or fonts disappear | The asset host or scheme is absent from its directive. | Allow the exact host in img-src or font-src; remove unused schemes such as data:. |
| Relative URLs behave unexpectedly | A required base URL conflicts with base-uri 'none'. |
Remove unnecessary <base> use or allow only the required base origin. |
How to compare two CSP designs
Do not judge policies by directive count alone. Compare them on the dimensions that determine containment:
Quick Recap
| Axis | Stronger design | Weaker design |
|---|---|---|
| Enforcement strength | Deny-by-default with narrowly scoped overrides. | Broad origins or a permissive fallback that authorizes unknown resources. |
| Script authorization | Nonces or hashes for intentional inline code. | 'unsafe-inline', unnecessary 'unsafe-eval' or unrestricted script hosts. |
| Outbound control | connect-src covers every approved API and socket destination only. |
Wildcard or broad connection permissions that leave easy exfiltration paths. |
| Embedding and navigation | Narrow frame-ancestors and form-action policies. |
Unrestricted embedding or forms that can post to arbitrary origins. |
| Legacy exposure | object-src 'none' where plugins are unnecessary. |
Unreviewed object and embed capabilities. |
| Operational readiness | Useful report collection, triage and regression monitoring. | Enforcement without visibility, or report endpoints that accept unlimited traffic. |
| Compatibility cost | Known exceptions tied to specific features and owners. | Permanent broadening to silence unexplained violations. |
Limitations to plan for
- CSP cannot stop server-side compromise, repair tampered content or revoke stolen credentials.
- An attacker who can modify trusted same-origin JavaScript may still abuse capabilities that the policy intentionally grants to that origin.
- Allowing a third-party origin for one resource class creates trust in that origin for that class; keep permissions separated by directive.
- Browser support and reporting behavior differ, which is why report-only testing and a temporary
report-urifallback are useful. - A policy that blocks legitimate business flows is likely to be disabled under pressure. Incremental rollout and documented exceptions preserve both security and availability.
Deployment checklist
- Set the policy as an HTTP response header on every relevant page, including authenticated areas.
- Start with
Content-Security-Policy-Report-Onlyand exercise real workflows. - Use
default-src 'self'or an equally restrictive fallback, then justify each override. - Use unpredictable per-response nonces or stable hashes instead of broad inline allowances.
- Constrain
connect-src,form-action,frame-srcandframe-ancestorsindependently. - Disable unused plugin objects with
object-src 'none'and prevent unauthorized base URL changes withbase-uri 'none'. - Configure
report-towithReporting-Endpoints; retainreport-uriwhile older browsers require it. - Review every new origin and every unsafe keyword as a security change.
- Continue incident response and credential rotation; CSP is containment, not remediation.
