Common Side Effects of Targeted Cancer Therapies and How Patients Manage Them

Targeted cancer therapies can cause side effects because the proteins they block in cancer cells may also exist in healthy tissue. There is no single side-effect list for “targeted therapy”: risks depend on the drug, its target, dose, combinations and your health. Diarrhea and liver problems are among the most common effects overall, while rash, other skin changes, fatigue, high blood pressure, mouth sores, nail changes and blood-count changes are also possible.

Ask your oncology team for the side effects expected with your exact medicine, how to manage them, which tests you need and who to call after hours. Report new or unusual symptoms promptly; do not stop, reduce or add treatment on your own.

What targeted therapy is and why side effects happen

The National Cancer Institute (NCI) describes targeted therapy as treatment aimed at proteins that control how cancer cells grow, divide and spread. The goal is greater selectivity than traditional chemotherapy, but selectivity is not perfect. Healthy cells can carry some of the same proteins or depend on related pathways, so treatment can affect normal tissue too. See the NCI overview at Targeted Therapy for Cancer.

Depending on the medicine, affected tissues may include skin and nail cells, hair follicles, the lining of the mouth and digestive tract, reproductive-system cells and blood-forming cells. Some drugs can also affect organs such as the heart, lungs, kidneys or thyroid. A side effect is therefore a possibility, not a prediction that every patient will experience it.

Targeted treatment may be taken by mouth, given by injection or delivered by infusion. The route, drug class, target, dose and combination with other treatments all change the risk profile. A class-level article cannot tell you how likely a particular symptom is for your prescription.

What are the common side effects of targeted therapy?

NCI identifies diarrhea and liver problems as common overall effects. The American Cancer Society (ACS) lists a broader range of possible reactions and emphasizes that its list is not complete. The table below groups effects readers may hear about; it does not mean that all targeted drugs cause them.

Possible effect What it can involve Why your team may monitor it
Diarrhea, nausea, vomiting or constipation Changes in bowel frequency, loose stools, queasiness, vomiting or difficulty passing stool Fluid loss, dehydration, electrolyte changes and interactions with other medicines may require treatment
Liver problems Abnormal liver-test results, sometimes with symptoms such as unusual fatigue, dark urine or yellowing skin Blood tests can detect injury before it becomes obvious; the exact response depends on the drug
Skin, hair and nail changes Rash, dry or sensitive skin, photosensitivity, nail changes or loss of hair color Some reactions are mild; extensive, painful or blistering changes can need urgent evaluation
Fatigue Persistent tiredness or reduced ability to perform usual activities Anemia, thyroid changes, infection or other causes may need assessment
High blood pressure Elevated readings, sometimes without noticeable symptoms Uncontrolled pressure can damage organs; clinicians may check it regularly and prescribe medicine
Mouth sores Tender areas or ulcers that make eating, drinking or brushing difficult Pain control, hydration and infection prevention may be needed
Bleeding, clotting or wound-healing problems Easy bruising, nosebleeds, prolonged bleeding or delayed healing Some drugs alter blood-vessel growth or clotting; severe bleeding is an emergency
Low blood counts and infection risk Reduced red cells, white cells or platelets, with possible weakness, fever or infections Blood counts help identify complications and guide care
Swelling or organ changes Fluid retention, breathing symptoms or changes in heart, lung, kidney or thyroid function Symptoms and test results can signal a serious drug reaction
Allergic or infusion reactions Chills, rash, dizziness, throat or chest tightness, swelling or breathing difficulty during or after treatment Severe reactions require immediate emergency treatment

Certain targeted medicines can trigger autoimmune reactions, in which the immune system attacks healthy organs. The specific organs and warning symptoms vary by medicine. Your team should explain which symptoms are especially important for your treatment.

What should I do when a side effect starts?

Use the plan provided by your oncology team rather than waiting for the next appointment. ACS patient guidance says, “Report all changes and side effects as soon as possible to your cancer care team.”

  1. Record the change. Note when it began, how often it occurs, its severity, what makes it better or worse, your temperature when relevant, and any medicine or supplement taken for it.
  2. Check your treatment instructions. Use the clinic’s written fever threshold, after-hours number and emergency instructions. Do not substitute a generic threshold for the one your team gave you.
  3. Call the appropriate contact. Tell the nurse or oncologist the drug name, dose and last treatment time, your symptoms and any relevant medical conditions. Ask whether you need an examination, laboratory test or urgent care.
  4. Follow only approved symptom measures. Your clinicians may recommend prevention, fluids, prescription or over-the-counter treatment, monitoring or a treatment adjustment. Do not start, stop or change a prescribed cancer medicine without their direction.
  5. Keep follow-up appointments and tests. Bring your symptom record and complete requested blood tests, scans or other checks even when you feel well.

What should I do if I have skin, nail or hair changes?

Skin and nail effects can include rash, dryness, redness, tenderness, cracking, color changes and nail inflammation. Hair may thin, change texture or lose color. A drug-related rash can look acne-like, but ordinary acne products may irritate it or be inappropriate. Contact your care team before applying a new cream, steroid, antibiotic, acne treatment or other product.

For a new rash or dry skin

  • Tell the oncology team when the change appears, where it is located and whether it is spreading, painful, itchy, blistering or interfering with daily activities.
  • Ask which moisturizer, cleanser or prescription cream is suitable for your medicine and skin condition. The right product and schedule are drug-specific.
  • Seek prompt evaluation for extensive peeling, open areas, severe pain, blisters, sores around the eyes or mouth, or signs of infection such as increasing warmth, pus or fever.

For sun sensitivity

Some therapies increase sensitivity to sunlight. If your team confirms that this applies to your medicine, ask about protective clothing, shade and a broad-spectrum sunscreen with SPF 30 or higher. Sunscreen is for sun protection, not a treatment for a drug rash; follow the product and ingredient advice supplied by your clinicians. ACS discusses sun protection in its guidance on targeted drug therapy side effects.

For nail or hair changes

Report painful nail folds, separation, drainage, scalp sores or sudden severe hair changes. Your team can advise on protection, infection evaluation and cosmetic options that will not worsen the condition. Do not assume a nail or hair symptom is harmless simply because it is not painful.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

NCI advises discussing a rash with the care team before stopping targeted therapy. Its skin and nail guidance is available at Skin and Nail Changes and Cancer Treatment.

Interactions with medicines, foods and supplements

Interactions can increase side effects or reduce how well a targeted medicine works. ACS specifically notes that aspirin-like medicines may add to bleeding risk with some therapies and that grapefruit juice can interfere with absorption or effectiveness of some targeted drugs. These examples do not apply to every medicine, so do not make a blanket change without advice.

When to seek emergency help

For trouble breathing, call emergency services first. Severe allergic or infusion-reaction signs can include breathing difficulty, dizziness, throat or chest tightness, or swelling of the lips or tongue. Serious bleeding, collapse, confusion or an inability to keep fluids down also warrants emergency assessment. After calling emergency services, notify your cancer team as soon as it is safe.

How side effects are monitored

NCI notes that people receiving targeted therapy may have blood tests, X-rays, scans and other medical tests. There is no single schedule that fits every targeted drug. Ask your team:

  • Which blood tests or scans are needed before and during treatment?
  • How often will blood pressure, weight, heart, lung, kidney, liver or thyroid function be checked?
  • Which result or symptom should trigger a same-day call?
  • Who reviews results, and how will you receive instructions?

Monitoring can identify a problem before you feel it, but it does not replace reporting symptoms. Keep appointments and tell the team about changes between tests.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How long do targeted-therapy side effects last?

Many side effects improve after treatment ends, but the timing differs by medicine and person. Some effects resolve gradually; others can persist or require follow-up. Because newer therapies have limited long-term experience, some delayed effects remain uncertain. Do not assume that a symptom will disappear on a fixed timetable or that a persistent symptom is unrelated to treatment. Ask when your team expects improvement and what follow-up is needed.

Questions to ask before and during treatment

  • What is the exact name, route and schedule of my targeted therapy?
  • Which side effects are most likely with this medicine, and which are emergencies?
  • How should I manage diarrhea, nausea, rash, fatigue or blood-pressure changes if they occur?
  • What temperature, blood-pressure reading or symptom requires an immediate call?
  • Which tests will I have, and how often?
  • Are there medicines, foods, vitamins or supplements I should avoid?
  • What is the after-hours and emergency contact plan?
  • If a side effect is severe, what clinician—not the patient—decides whether treatment is paused or changed?

A drug-specific medication guide and your oncology team’s instructions are more useful for personal decisions than a general list. Bring questions and a complete medication list to every visit.
