Common Causes of Nail Detachment (Onycholysis) and How They Differ

Nail detachment from the nail bed is called onycholysis. The nail plate lifts away from the skin beneath it, often creating an opaque white area. Onycholysis is a sign rather than a diagnosis: repeated injury, wet work and chemicals, fungal infection, psoriasis or another inflammatory disorder, medication effects, and occasionally a systemic illness can all produce it.

Appearance alone cannot reliably identify the cause. A clinician uses the number and distribution of affected nails, recent trauma, exposures, other nail or skin changes, medicines, and associated symptoms. If fungus or another disease remains possible, nail scrapings, clippings, cultures or blood tests may be needed. The detached part will not reattach; care is aimed at protecting the nail and allowing new growth to remain attached while the underlying cause is addressed.

What onycholysis looks and feels like

In onycholysis, the hard nail plate separates from the nail bed. The lifted portion commonly looks white because air occupies the space that was previously attached to the skin. The border may begin at the tip or side and gradually move toward the base if the trigger continues. A separated nail can catch on clothing and collect debris, but the appearance varies with the cause.

A white-looking nail is not automatically detached. Merck Manual Professional Edition explains that leukonychia can look white while the nail remains attached. A gentle clinical examination is more reliable than trying to diagnose the change from a photograph or color alone.

DermNet describes onycholysis as “a clinical diagnosis with the cause often obvious on history and examination.” That observation, from its page updated in December 2020, means the history and examination frequently reveal the likely trigger, not that every case can be identified without testing.

The main causes and the clues that separate them

The following patterns organize the differential diagnosis. They are clues, not a home diagnostic rule; several causes can coexist, and fungal infection and psoriasis can look alike.

Possible cause Clues that support it What limits the clue and what to do next
Trauma or repeated pressure A recent impact, repetitive tapping, tight footwear, sports, aggressive cleaning beneath the nail, or a manicure; often limited to a nail that receives the stress. Continuing pressure can extend the separation. Stop scraping under the nail, reduce the mechanical stress and monitor new growth. See a clinician if the explanation is uncertain or the lifting progresses.
Water, detergents and cosmetic irritants Frequent immersion, household cleaners, solvents, nail polish or remover, hardeners, artificial nails or other repeated chemical exposure. Wet work can overlap with trauma and irritant dermatitis, so exposure history is not conclusive. Reduce the irritant and protect hands with gloves.
Fungal nail infection Thickening, yellow discoloration, white streaks, crumbly material or debris under the nail; toenails are commonly involved in general clinical descriptions. Those features also occur in psoriasis and nonfungal nail disorders. A clipping or scraping can be examined or tested when the diagnosis is uncertain instead of treating appearance alone as proof.
Psoriasis or another inflammatory nail disorder Pitting, discoloration, crumbling, blood beneath the nail or buildup may accompany separation. A history of psoriasis helps, but nail psoriasis can occur without obvious skin plaques. Other conditions can mimic these changes. Examination of the nails and skin, and the overall pattern, determines whether further testing or treatment is appropriate.
Medication-related photosensitivity or treatment effect A clear timing relationship to a medicine, including some photosensitizing drugs or chemotherapy, can be important. DermNet lists tetracyclines, nonsteroidal anti-inflammatory drugs, psoralens, oral retinoids and taxanes as examples. Never stop a prescribed medicine on your own. Ask the prescriber or dermatologist whether the timing and known adverse effects fit and what alternatives, if any, are safe.
Systemic illness Several nails affected at once, or nail changes accompanied by other symptoms, may prompt consideration of thyroid disease, diabetes, pregnancy or less common internal causes. Multiple affected nails do not prove an internal disease. A clinician decides whether blood tests or another assessment is warranted.

These distinctions are consistent with the cause lists from DermNet and Cleveland Clinic. The distribution of affected nails, the speed of change, associated skin findings, work and hobby exposures, recent procedures, medicines and systemic symptoms all help narrow the possibilities.

Trauma, pressure and manicure-related lifting

A single blow can lift part of a nail, but repeated small injuries are just as relevant. Tight shoes and sports can stress toenails; keyboard tapping, instruments, occupational gripping or picking can stress fingernails. Digging under the edge with a file or tool may physically separate an otherwise healthy nail and then keep the gap open.

Nail-Aid - 5-in-1 Damaged Nails Multi-Repair for Fingernails or Toenails
  • Reduces discoloration for brighter nails.
  • Smooths out thickness, uneven surface, ridges.
  • Hydrates to end peeling and brittleness.
  • Strengthens, grows fragile, torn, cracked nails.

An aggressive manicure, removal of artificial product, or repeated buffing can injure the nail plate or the tissue beneath it. If one exposed nail began lifting soon after a specific event and no other nails changed, trauma is plausible. It is still worth checking a persistent or worsening separation, because an injury can coexist with infection or inflammation.

What to change immediately

  • Stop cleaning beneath the nail with sharp tools.
  • Reduce repetitive pressure and protect the nail from catching.
  • Keep the nail short, but cut only the portion that has already separated.
  • Pause procedures or products that caused burning, tenderness or visible damage.

Water, detergents, solvents and nail products

Frequent wet work softens the nail and repeatedly exposes the nail fold and surrounding skin to water. Detergents, disinfectants, solvents, polish remover, hardeners and artificial-nail products can add chemical irritation. The combination of wetness, friction and chemicals is more informative than any single exposure.

Use protective gloves for cleaning, dishwashing and other prolonged wet tasks, and follow the product’s safety instructions. Reducing exposure will not glue the already separated section back down; it lowers the chance that new growth will lift as it advances.

Could a fungus make a nail detach?

Yes. Fungal nail infection can cause thickening, yellow or white discoloration, streaks and debris, with the nail eventually separating from the bed. Toenails are frequently involved in clinical descriptions, although fingernails can also be affected.

The problem is that these visual signs are not specific. Psoriasis, trauma and other nail disorders can produce thickening, color change or subungual material. Cleveland Clinic and DermNet both emphasize that testing may be useful when the cause is not clear. A clinician may trim a sample or scrape material for fungal or bacterial testing. This avoids committing to an antifungal treatment when the nail is not infected.

Is nail separation a sign of psoriasis?

Psoriasis can affect the nail matrix and nail bed. Pitting, discoloration, crumbling, blood under the nail and buildup can accompany onycholysis. Nail psoriasis may occur in someone with known psoriasis, but it can also appear when there are no obvious skin lesions.

Because inflammatory nail disease overlaps visually with fungus and trauma, the diagnosis depends on the complete pattern: other nails, skin and scalp findings, joint symptoms, personal history and examination. The American Academy of Dermatology explains the features and treatment considerations for nail psoriasis at its nail-psoriasis guide. Do not assume that every crumbly or discolored detached nail is fungal.

Medicines and photosensitivity

Some medicines can contribute to nail separation through photosensitivity or a direct treatment effect. DermNet lists tetracyclines, nonsteroidal anti-inflammatory drugs, psoralens, oral retinoids and taxanes among examples; chemotherapy is also a relevant history. The key clue is timing: did the change begin after a medicine started, a dose changed or a treatment cycle occurred?

Medication-related onycholysis requires a prescriber-led review. Bring a complete list of prescription drugs, over-the-counter pain relievers, supplements and recent cancer treatments. Do not stop, skip or substitute a prescribed medicine without medical advice.

When several nails are involved: systemic possibilities

Multiple nails changing together, especially with fatigue, weight change, heat or cold intolerance, thirst, pregnancy or other new symptoms, may lead a clinician to consider thyroid disease, diabetes, pregnancy-related changes or another systemic condition. This is a prompt for assessment, not a reason to assume an internal illness.

Depending on the examination and history, evaluation may include blood tests such as thyroid testing. The clinician chooses tests based on the pattern rather than ordering every possible test for every detached nail.

How a clinician evaluates a lifting nail

Assessment starts with questions about onset, injury, manicure, wet work, chemicals, footwear, sports, medicines and whether one or many nails are affected. The clinician examines the color, thickness, debris, border of separation, nail folds and surrounding skin, and looks for psoriasis or other dermatologic signs.

  1. Document the pattern. Note which nails are involved, when the change began, whether it is spreading and any pain, redness, swelling or discharge.
  2. Review exposures and treatments. Include recent impacts, repetitive pressure, cleaning under the nail, manicures, artificial nails, polish removers, detergents, solvents and wet work.
  3. Review medicines. Record start dates and treatment cycles, including photosensitizing medicines and chemotherapy.
  4. Test when the appearance is ambiguous. Nail clippings or scrapings may be sent for fungal or bacterial testing rather than relying on visual diagnosis.
  5. Investigate systemic causes selectively. If the history or distribution suggests an internal condition, the clinician may order blood tests, including tests for thyroid disease.

This approach reflects the clinical guidance in DermNet’s onycholysis reference. A sample is especially useful before a prolonged course of treatment when fungus is only one of several plausible explanations.

Safe care while the nail grows out

Basic care protects the attached portion and prevents secondary injury. It does not reattach the portion that has already lifted.

  • Keep it short: clip the nail as it grows, trimming only the detached edge that can be cut safely.
  • Do not dig underneath: scraping, probing and aggressive filing can enlarge the gap and injure the nail bed.
  • Limit water and irritants: wear gloves for wet work and reduce contact with detergents, solvents and products that trigger irritation.
  • Reduce mechanical stress: address tight shoes, repetitive tapping, picking and activities that repeatedly strike the nail.
  • Keep tools clean: use clean clippers and do not share them, particularly when debris or possible infection is present.

As new nail grows, the goal is for it to remain attached to the bed. The separated segment itself cannot fuse back down. If the cause is fungal, inflammatory, medication-related or systemic, treating that cause is what determines whether future growth improves.

When should a lifting nail be checked?

Arrange a clinical assessment when there is no obvious explanation, when lifting persists or worsens, or when fungal infection is suspected. Redness, swelling, increasing pain or discharge around the nail can indicate infection and should not be managed only with cosmetic products.

A new or changing dark streak requires a dermatologist’s examination. The American Academy of Dermatology lists concerning nail changes, including pigment changes, in its guidance at “12 nail changes a dermatologist should examine.” Persistent, progressive or nonhealing separation also deserves evaluation because, rarely, a nail-unit tumor can be part of the differential.

Seek prompt care for

  • Rapidly increasing separation or involvement of several nails without a clear trigger.
  • Redness, warmth, swelling, pus, severe tenderness or spreading pain.
  • A new, widening or changing brown or black streak.
  • A nail that remains abnormal despite removing an obvious irritant.
  • Associated symptoms suggesting a broader illness.

Common mistakes to avoid

  • Diagnosing fungus by color alone: yellow, white or crumbly changes also occur with psoriasis and trauma.
  • Pulling off attached nail: remove only the already detached edge; tearing can injure the nail bed.
  • Continuing the trigger: repeated wet work, solvents, tight footwear or picking can prevent healthy growth from staying attached.
  • Stopping a medicine without advice: discuss a possible adverse effect with the prescriber first.
  • Covering a warning sign with cosmetics: dark streaks, inflammation, discharge and unexplained progression need examination.

Practical decision guide

If one nail lifted immediately after a clear injury and there is no inflammation, protect it, keep it short and watch the new growth while avoiding further trauma. If several nails are involved, the change is progressive, or there is thickening and debris, schedule an examination rather than self-treating for fungus. If psoriasis, a medicine change, pregnancy or other systemic symptoms are part of the history, mention them explicitly because they change the evaluation. Any dark streak, marked inflammation or discharge warrants prompt professional review.
