The most dangerous defects are usually semantic: a valid API used with the wrong lifecycle, a boundary condition that returns the wrong data, an authorization check applied too late, or a dependency that does not exist. Catch them with layered review: define the intended behavior, verify APIs and packages against their exact documentation, build and execute the real project, test negative and edge cases, trace untrusted data, run static and dependency analysis, and require qualified human approval for security-sensitive paths.
Why visual review is not enough
Generated code optimizes for a plausible-looking answer to the prompt. It does not know whether the surrounding service expects a transaction to be closed, whether a token is authorized for this tenant, whether a date is in UTC, or whether a package name is real. A reviewer who reads only the happy path can therefore approve code that is syntactically tidy but operationally wrong.

The available studies show why confidence is a poor quality signal. In the RobustAPI study of 1,208 Stack Overflow questions covering 18 Java APIs, 62% of GPT-4-generated code contained an API misuse. The authors warn that “the executable code is not equivalent to reliable and robust code”; misuse can lead to resource leaks, crashes, and other unexpected consequences (Li Zhong and Zilong Wang, AAAI-24, 2024).

The bug patterns reviewers most often miss
1. A real API used incorrectly
Hallucination does not require an invented method. More often, the model selects a real class or function but supplies the wrong parameter, assumes an old default, skips a required initialization step, or fails to close a resource. It may catch a broad exception that hides a failed operation, use a method that is deprecated in the project’s version, or confuse a synchronous and asynchronous lifecycle.
For every external call, check the official documentation for the exact version declared by the project. Confirm the signature, units, default values, nullability, thread-safety guarantees, error behavior, retry rules, and cleanup obligations. Then inspect what the code does when the call fails. A successful compile proves only that names and types line up; it does not prove that the API contract is respected.
- Identify every library, framework, cloud SDK, database driver, and operating-system call in the diff.
- Compare arguments and return-value handling with the versioned reference documentation.
- Look for required close, dispose, commit, rollback, unsubscribe, or cancellation operations.
- Exercise documented failure responses, not just the success response.
2. Code that compiles but returns the wrong result
Semantic errors survive a green build. Common examples include an inverted comparison, an off-by-one slice, a pagination loop that skips the final page, a timezone conversion applied twice, an encoding assumption that corrupts non-ASCII text, or a state transition allowed in the wrong order. Generated code often mirrors the wording of a prompt rather than the invariant the system must maintain.
Write down the intended input-output behavior before judging the implementation. Add tests for empty, malformed, maximum-size, duplicate, unauthorized, expired, and downstream-failure inputs. For calculations, include hand-checked examples and boundary values. For stateful code, test every permitted transition and at least one forbidden transition. The Monash result—1,082 wrong-output programs in its 4,066-program set—shows why output assertions matter more than visual completeness.

3. Compilation and runtime failures hidden by an incomplete review
A snippet copied into a project may refer to a symbol available only in a different package version, rely on an import omitted from the answer, or assume configuration that production does not provide. Runtime failures can appear only with a missing environment variable, a null response, a large payload, a network timeout, or a platform-specific filesystem behavior. The same Monash study recorded 177 programs with compilation or runtime errors.
Build the exact repository and run the same checks used by the merge pipeline. Do not compile an isolated excerpt and infer that the application is healthy. Run unit, integration, and regression tests; start the service with production-like configuration; and exercise timeout, retry, cancellation, and shutdown paths. Record which checks ran and which were unavailable rather than treating an unrun test as a pass.
4. Security flaws in ordinary-looking glue code
Security bugs frequently sit in code that appears to be plumbing: a request handler, serializer, file upload, redirect, logger, or database helper. Review authentication and authorization separately. Authentication establishes who a caller is; authorization must still verify what that identity may access, for this object and this tenant, at this point in the workflow.

Run a SAST or CodeQL analysis, dependency and secret scanners, and the project’s type and format checks. A 2024 CodeQL comparison found 248 vulnerabilities in ChatGPT snippets versus 302 in Stack Overflow snippets, spanning 19 versus 22 CWE types. That comparison does not show that generated code is safe; it shows that vulnerability counts vary with the benchmark and source. A 2024 literature review covering 15 primary studies identified seven vulnerability kinds and six mitigation strategies, reinforcing that no single scan covers every weakness.
5. Hallucinated or unsafe dependencies
A plausible package name is not evidence that a package exists, is maintained, or is trustworthy. Generated code can introduce a nonexistent import, select a similarly named package from an unexpected registry, or add a version with a vulnerable transitive dependency. It can also silently expand permissions through a new plugin or cloud SDK.
Verify each import against the registry used by the project, then inspect the lockfile and the complete transitive dependency tree. Pin versions according to your update policy, review license and maintenance signals required by your organization, and run dependency and secret audits. Reject additions that are unexplained, unused, or broader than the feature requires. A clean install in one environment is not proof that a dependency is appropriate for production.
6. Maintainability defects that become future bugs
Generated code may duplicate business rules, create deeply nested control flow, swallow errors, omit tests, or use a different logging and naming convention from the surrounding system. These are not merely style disagreements. Duplication lets two copies of a rule diverge; broad exception handling removes evidence needed to diagnose failures; and needless complexity makes security review harder.

Static analysis in the Monash study identified maintainability issues in 1,930 snippets. Review ownership and operability as part of correctness: Can another engineer explain the invariant? Are metrics and structured logs present at the failure boundary? Is the code easy to test without real credentials or production data? Simplify generated code when that improves those answers, and require tests for every newly introduced branch.
A review sequence that works before merge
- Define behavior and threat boundaries. Write the accepted inputs, outputs, permissions, data classifications, failure behavior, latency or resource constraints, and invariants. Identify which callers and assets are trusted.
- Verify external assumptions. Open the official API and dependency documentation for the exact versions in the repository. Confirm method signatures, defaults, lifecycle obligations, deprecations, supported platforms, and documented error responses.
- Build the exact project. Use the repository’s normal toolchain and lockfile. Resolve compilation, type, migration, configuration, and generated-code steps before interpreting test results.
- Run deterministic tests. Execute unit, integration, and regression suites. Add focused tests for empty, malformed, maximum, duplicate, unauthorized, timeout, retry, cancellation, and downstream-error cases exposed by the change.
- Trace untrusted data. Mark every external input and follow it to interpreters, queries, commands, templates, redirects, files, deserializers, and logs. Verify validation, canonicalization, encoding, and authorization at each boundary.
- Run static and dependency analysis. Use CodeQL or an equivalent SAST tool, dependency and secret scanners, plus format and type checks. Triage findings manually: tools have coverage gaps and false positives, while a clean report is limited evidence.
- Review the diff for intent. Look for silent behavior changes, broad exception handlers, authorization checks placed after data access, resource leaks, race conditions, unsafe defaults, copied code the author cannot explain, and unrelated generated churn.
- Use the model only as a review aid. Ask it to state assumptions, invariants, failure modes, and proposed tests. Verify every statement against documentation and execution; a confident explanation is not proof.
- Require qualified sign-off. A reviewer who understands the system and its threat model must approve authentication, authorization, cryptography, payments, personal data, deserialization, infrastructure permissions, and deployment configuration.
How to test the cases generated code commonly misses
Boundary and malformed data
- Use empty strings, empty collections, null or absent fields, zero, negative values, maximum representable values, and over-limit payloads.
- Include duplicate identifiers, reordered events, stale versions, invalid encodings, unexpected Unicode, and malformed dates.
- Assert both the returned result and the side effects: records created, permissions changed, messages sent, and resources released.
Failures and partial completion
- Force timeouts, connection resets, rate limits, unavailable dependencies, rejected writes, and interrupted cancellation.
- Check whether retries are bounded and idempotent rather than duplicating payments, messages, or state changes.
- Verify rollback and cleanup when a multi-step operation fails halfway through.
Authorization and tenant isolation
- Test an authenticated user with no permission, a user with permission for a different object, and a request with a changed tenant or account identifier.
- Check direct-object lookups and background jobs separately; authorization that exists in a controller may be absent in a worker.
- Confirm that error responses do not reveal whether another tenant’s object exists.
What tools can and cannot establish
| Approach | Strongest evidence | Blind spots | Human decision still required |
|---|---|---|---|
| Compiler, type checker, formatter | Names, types, syntax, and basic structural consistency | Wrong business logic, unsafe defaults, missing authorization, incorrect API semantics | Whether the implementation meets the intended behavior |
| Unit and integration tests | Observed behavior for the cases exercised | Untested boundaries, rare races, environment differences, inadequate assertions | Which cases and invariants must be added |
| SAST or CodeQL | Known patterns, data-flow paths, and some CWE classes | Novel logic flaws, incomplete modeling, false positives and negatives | Exploitability, severity, and remediation |
| Dependency and secret scanners | Known vulnerable packages, leaked credentials, and suspicious dependency changes | Unknown vulnerabilities, malicious but unflagged packages, secrets outside scanned locations | Trust, necessity, version policy, and rotation |
| Model explanation or test generation | Candidate assumptions, cases, and hypotheses to investigate | Confidently invented facts, incomplete threat models, tests that encode the same wrong assumption | Independent verification and acceptance |
| Human code review | System intent, domain rules, architecture, and contextual risk | Attention limits, unfamiliar APIs, and confirmation bias | At least one reviewer with relevant system and security knowledge |
When choosing or combining review tools, compare language and framework coverage, API and dependency awareness, data-flow detection, test generation and execution, false-positive and false-negative handling, pull-request and CI integration, explainability, and the required level of human validation. No tool should be described as proving correctness.
How much can AI-assisted repair help?
Tool assistance is useful after a finding is already understood. A study of GitHub projects reported that Copilot Chat fixed up to 55.5% of security issues when supplied static-analysis warnings (Fu et al., 2023, revised 2025). “Up to” is a best observed result in that study, not a guarantee for a new repository. The workflow is valuable because a scanner supplies a concrete location and rule, while the model can propose a patch or a test. The reviewer must still confirm that the fix closes the actual data-flow path, preserves behavior, and does not move the vulnerability elsewhere.
Ask the model for a minimal patch, an explanation of the violated invariant, and regression tests. Then inspect the diff, rerun the scanner, execute the tests, and review adjacent code manually. Never let a model-generated remediation suppress a warning without documenting why the finding is not applicable.

What the evidence says about trusting AI-generated code
The studies do not support a single “AI defect rate.” The RobustAPI result concerns GPT-4 responses to Java API questions; the Monash counts concern 4,066 ChatGPT programs; the CodeQL comparison uses different snippets and vulnerability categories; and the Copilot result measures repairs after warnings are supplied. Each uses different models, languages, prompts, datasets, and evaluation methods.
A low-level C user study found that AI-assisted users produced critical security bugs at a rate no greater than 10% above the control group (Sandoval et al., USENIX Security, 2023). That finding is limited to its language, task, participants, and experimental conditions; it is not evidence that generated code is safe in a web service, mobile app, or infrastructure repository.
The defensible conclusion is operational rather than numerical: AI can accelerate drafting and remediation, but reliability comes from specification, execution, adversarial tests, documentation checks, analysis tools, and accountable human review. As the IEEE comparison study’s conclusion puts it, “Any code copied and pasted, created by AI or humans, cannot be trusted blindly, requiring good software engineering practices to reduce risk.”
