Prioritize vulnerabilities listed in CISA’s Known Exploited Vulnerabilities (KEV) Catalog ahead of equally exposed vulnerabilities that only have a high CVSS score—but do not patch by catalog status alone. KEV is evidence that exploitation has been observed in the wild. CVSS is a standardized assessment of technical severity. The defensible order is to use KEV as a high-urgency signal, then verify the affected product and version, exposure, asset importance, available mitigation, change risk and any deadline that applies to your organization.
A critical CVSS score does not prove that attackers are exploiting a vulnerability. Being absent from KEV does not prove that exploitation is impossible or that the issue is safe to defer. Your final queue should combine threat evidence, technical severity and local business context.

What KEV and CVSS actually measure
KEV measures observed exploitation
CISA describes the KEV Catalog as its authoritative source for vulnerabilities known to have been exploited in the wild. A CVE’s presence in the catalog is therefore a threat-relevance signal: someone has obtained evidence of real-world exploitation, rather than merely predicting that exploitation could occur.
CISA’s stated guidance is direct: “Organizations should use the KEV catalog as an input to their vulnerability management prioritization framework.” KEV records also include a remediation action and a due date, which turn threat intelligence into an operational task.

KEV is not an inventory of every exploitable vulnerability. It is not a statement that every affected system is currently under attack, and it does not tell you whether the vulnerable version is installed in your environment. A vulnerability can be dangerous without appearing in the catalog, especially when exploitation is undiscovered, targeted, newly emerging or not yet reflected in the catalog.
CVSS measures assessed technical severity
The Common Vulnerability Scoring System expresses the technical characteristics of a vulnerability through a standardized score and vector. CVSS helps teams communicate how difficult exploitation may be and what damage could result if exploitation succeeds. It does not establish that exploitation is occurring.
In CVSS v4.0, Base metrics characterize the vulnerability itself. Environmental metrics can modify the assessment to reflect a particular organization’s deployment, such as the importance of the affected asset or the impact of local security controls. A published Base score is therefore a useful common language, not a complete patch-ordering decision.
FIRST’s CVSS guidance treats scores as inputs to risk decisions. Its v4.0 FAQ notes that “the vulnerability identified by the supplier (eg. vendor) with a higher Provider Urgency may be worth considering being remediated first.” Supplier urgency, threat intelligence and local conditions can change the practical order even when two vulnerabilities have similar Base scores.
Should KEV vulnerabilities come before critical CVSS scores?
Usually, yes when the systems are comparably exposed and important. A KEV entry supplies evidence of exploitation, so it deserves an urgent workflow. A “critical” CVSS rating supplies a severity assessment, but not evidence that attackers are using the flaw. That distinction is why a lower-scored KEV vulnerability on an internet-facing, business-critical system can reasonably outrank a higher-scored vulnerability on an isolated, low-value host.
Do not treat KEV and CVSS as competing numbers on one scale. They answer different questions:
- KEV: Has exploitation of this vulnerability been observed in the wild, and what action and date does CISA list?
- CVSS: How severe is the vulnerability according to defined technical metrics?
- Your environment: Is the affected version installed, reachable and important here, and can you mitigate it safely?
The resulting priority is a reasoned synthesis, not a formal CISA formula. Document why a vulnerability moved up or down so another analyst can reproduce the decision.

KEV and CVSS compared
| Question | KEV Catalog | CVSS |
|---|---|---|
| What does it tell you? | CISA has identified evidence that the vulnerability has been exploited in the wild. | A standardized assessment of technical severity using defined metrics. |
| What action does it support? | Place the vulnerability in an urgent remediation workflow; an applicable catalog due date is mandatory for FCEB agencies. | Understand and communicate severity, then combine it with environmental and organizational context to set local priority. |
| What must it not be mistaken for? | A complete list of exploitable vulnerabilities or proof that your own assets are exposed. | Proof of active exploitation or a complete, environment-specific patch order. |
| Useful decision inputs | Exploitation evidence, catalog remediation action, due date, affected versions and vendor guidance. | Technical severity, vector details, Environmental metrics, exposure, impact and business criticality. |
A practical vulnerability-prioritization workflow
1. Match every finding to an actual asset and version
Start with an authoritative inventory rather than the scanner title. For each CVE, verify the product, edition, installed version, operating system, service role and whether the vulnerable component is enabled. Resolve aliases and backported vendor patches before declaring an asset vulnerable.
Record the asset owner, network location and evidence used for the match. A catalog entry for a product you do not run is not a remediation task. A finding that names a product but cannot be tied to a reachable instance should be investigated, not automatically placed in the emergency queue.
2. Identify KEV entries and read the catalog action
Check the live KEV Catalog during triage. It changes over time, and new entries and due dates make an old export unsuitable as a permanent source of truth. Do not quote a current total of catalog entries unless you have checked the live catalog immediately before publication; the catalog is dynamic.
For each matching entry, capture the listed remediation action and due date. The action may point to a vendor update, configuration change or other mitigation. Treat the date as an explicit planning constraint, not merely as metadata.
3. Apply the correct policy scope
Binding federal deadlines must be described precisely. Binding Operational Directive 22-01 requires Federal Civilian Executive Branch (FCEB) agencies to remediate listed vulnerabilities by the applicable catalog due date. That directive does not automatically apply to private companies, state and local governments or other organizations.
CISA’s broader advice is still significant: “Although BOD 22-01 only applies to FCEB agencies, CISA strongly urges all organizations to reduce their exposure to cyberattacks by prioritizing timely remediation of KEV Catalog vulnerabilities as part of their vulnerability management practice.” Organizations outside the FCEB should set an internal target appropriate to their exposure and risk rather than describing the federal date as a universal legal requirement.
4. Use CVSS for non-KEV ordering
For vulnerabilities not listed in KEV, inspect the CVSS version, vector and individual metrics instead of sorting only by the headline number. A CVSS v4.0 score can help distinguish a vulnerability with severe confidentiality, integrity or availability consequences from one with a narrower effect.

Where your process supports it, calculate Environmental metrics to represent your deployment. The same software flaw can have a different practical consequence on a public identity server, a laboratory workstation and a segmented development host. Keep the original Base score and the environmental assessment visible so reviewers can see what changed.
5. Add asset and exposure context
For every candidate, answer these operational questions:
- Is the service reachable from the public internet, a partner network, a user VLAN or only from an isolated segment?
- Can an attacker reach the vulnerable function without authentication, or is a privileged account and a prior foothold required?
- Does the asset process regulated data, credentials, payment information, intellectual property or safety-critical functions?
- Would compromise provide administrative privilege, a route into other networks or persistence?
- Are compensating controls such as segmentation, allow-listing, endpoint protection or an application firewall actually enabled and monitored?
- Is a vendor patch, configuration change or CISA-listed mitigation available, and what downtime or regression risk would deployment create?
These checks are local risk analysis. They complement, rather than replace, the KEV record and CVSS assessment.
6. Schedule, mitigate and track exceptions
Schedule work using exploitation evidence, applicable deadlines, exposure, asset impact and change risk. When immediate patching is unsafe or impossible, apply the vendor or CISA-recommended mitigation, restrict access where feasible and create a dated exception with an owner, compensating control and next review date.
An exception is not a permanent lower priority. Reassess it when the asset becomes internet-facing, a new exploit is reported, a mitigation expires, a replacement patch is released or the KEV record changes.

The following tiers are a practical operating model. They are not an official CISA scoring formula.
| Tier | Typical conditions | Operational response |
|---|---|---|
| Emergency | KEV vulnerability on an internet-facing or highly privileged asset; an FCEB due date is imminent or overdue; or exploitation is producing active indicators in your environment. | Assign an owner immediately, verify scope, deploy the catalog or vendor action as soon as change controls permit, and monitor for compromise. Use mitigation while a patch is being tested. |
| Urgent | KEV vulnerability on an internal but reachable business-critical system, or a very high-severity non-KEV flaw with broad exposure and no effective compensating control. | Set a short internal target, prioritize testing and maintenance capacity, and document why the chosen date is safe. |
| High | High or critical CVSS severity on an important system, limited exposure, strong authentication requirements or effective segmentation; no known exploitation. | Plan remediation in the next available change window, validate Environmental metrics and monitor threat reporting. |
| Planned | Lower exposure, lower-impact assets, reliable compensating controls or vulnerabilities with no current exploitation evidence and modest technical impact. | Remediate through the normal patch cycle, but keep ownership and a review date. A change in exposure or threat status can move it upward. |
The tier should be attached to an asset-CVE pair, not just to the CVE. One CVE may be Emergency on a public gateway and Planned on a disconnected test machine.

How to resolve common KEV-versus-CVSS conflicts
Lower CVSS, listed in KEV
Suppose a vulnerability has a lower technical score but appears in KEV and affects an internet-facing application server. Exploitation has been observed, the service is reachable and the asset stores sensitive records. That combination can justify faster remediation than a very high-scored vulnerability on an isolated host with no route from untrusted networks.
Critical CVSS, not in KEV
A critical score still deserves attention. Review exploit prerequisites, affected components, exposure and asset value. If the affected software is widely reachable and the flaw needs no authentication, place it high even without KEV membership. If it is disabled, isolated and protected by effective controls, a documented planned window may be reasonable while monitoring for new exploitation evidence.
KEV entry, but the product is not actually affected
Do not patch a non-vulnerable system merely because its product name resembles a catalog entry. Confirm the exact version, vendor backport status and configuration. Close the finding with evidence, or correct the inventory, so the same false positive does not consume emergency capacity later.
Patch available, but deployment could cause an outage
Use the catalog action and vendor guidance to select a mitigation, limit reachability and test the update on a representative system. Set a specific deployment date and escalation owner. Record the business reason for delay, the temporary protection and the condition that will trigger an earlier review.
Using CVSS v4.0 without misreading the score
Read the vector, not just the label
Two vulnerabilities can share a “critical” label while differing in attack requirements, user interaction, privileges and impact. The vector exposes those distinctions. Analysts should preserve the vector in tickets and explain which assumptions apply to the local deployment.
Separate Base and Environmental assessments
Base metrics are intended to describe the vulnerability independently of a particular organization. Environmental metrics allow an organization to reflect modified impact, asset importance and local controls. Environmental scoring is useful only when the underlying inventory and control information is current; an optimistic value based on an unverified firewall rule creates false confidence.
Recommended Free Tools
Do not treat a score as an exploit forecast
CVSS is not a probability that exploitation will occur, a count of active attacks or a deadline. It should inform risk communication and ordering alongside KEV status, supplier urgency, asset exposure and business consequences.
Best Value
Operating the KEV process over time
Check the live catalog at defined points
Include a live-catalog check in daily or weekly triage, emergency response and major inventory changes. New entries can alter the urgency of an existing finding, and a due date can become the governing constraint. Keep the retrieval date and record version in your ticketing system.
Watch for changes outside the catalog
Absence from KEV is not a clean bill of health. Monitor vendor advisories, incident telemetry and credible threat intelligence for exploitation reports that may precede catalog inclusion. A newly disclosed vulnerability with no KEV entry can still require rapid action when exposure and impact are extreme.
Measure process performance without inventing a false benchmark
Track practical measures such as time from KEV listing to owner assignment, time to mitigation, overdue exceptions, asset coverage and the percentage of findings with verified product versions. Official materials do not establish a comparative statistic showing that one prioritization method produces a particular remediation improvement, so avoid presenting an unsupported percentage as proof of effectiveness.

Governance and evidence for every priority decision
A defensible record should show:
- the CVE, affected product and verified version;
- KEV status, catalog action and catalog due date when applicable;
- CVSS version, vector, Base score and any Environmental assessment;
- internet or internal reachability and required attack privileges;
- asset owner, business function and data or privilege impact;
- available patch, configuration change or compensating control;
- chosen priority, target date and the analyst’s rationale;
- exception owner and next review date if remediation is deferred.
This evidence prevents a high CVSS number from automatically displacing a known exploited issue, while also preventing KEV status from triggering blind patching of assets that are not affected.
The practical rule to adopt
Use this sequence whenever a backlog is larger than your available maintenance capacity: verify the asset, elevate KEV, honor any binding deadline, assess exposure and business impact, use CVSS and Environmental metrics to order the rest, and document mitigation when patching must wait.
For FCEB agencies, BOD 22-01 deadlines for catalog vulnerabilities are requirements. For other organizations, the same entries are urgent risk signals and should drive internal targets proportionate to exposure. In every case, the right decision is made at the intersection of observed exploitation, technical severity and the conditions of the specific asset.
