Martinez traveled from Florida to Nashville with her husband to witness the attempt at Riverbend Maximum Security Institution. She told AP: “I watched the whole thing. She was awake. She was moving her head, moving her feet. She was trying to sit up. Didn’t work the first time they gave her a shot. It didn’t work the second time so she’s in the hospital and alive.” She added: “This isn’t justice for Colleen.” (AP, October 1, 2026)

What happened during the attempt
Pike’s execution date had been set by a Tennessee Supreme Court order dated September 30, 2025, subject to further court order. (Tennessee Supreme Court order) AP’s account says the witness-room curtain opened at 7:27 p.m. Pike was strapped to a gurney with a spiritual adviser beside her. She said her arm hurt. What is known about why the execution failed
The cause had not been established in the reporting available by October 3, 2026. The Tennessee Department of Correction said it followed every step of the state’s established protocol, approved by the Attorney General’s Office, and that the protocol did not permit additional procedures beyond those carried out. Pike’s attorneys offered a different account in a court filing. They alleged that IV lines were incorrectly placed or veins had blown, and that pentobarbital may have entered her body rather than her bloodstream. Pike’s condition and Tennessee’s response
In the October 2 report, AP said Pike’s lawyers reported in a court filing that she was unconscious and on a ventilator in a hospital. This is a dated update attributed to her attorneys; the cited reporting does not establish a later medical condition. (AP, October 2, 2026)
The Death Penalty Information Center reported that Gov. Bill Lee ordered a third-party review and paused executions for the remainder of 2026.
