Tennessee officials attempted to execute Christa Pike on September 30, 2026, but did not complete the execution. They administered two doses of pentobarbital before taking her by ambulance to a hospital. The Tennessee Department of Correction said staff followed the state’s protocol; Pike’s attorneys later alleged problems with the IV lines and reported that she was unconscious and on a ventilator. The cause of the failed attempt—and Pike’s confirmed medical condition—had not been established in the available public accounts as of October 3, 2026.
What happened during the September 30 attempt?
Pike’s execution was scheduled for September 30, 2026. On that date, officials administered two doses of pentobarbital but did not complete the execution. Pike was transported by ambulance to an off-site medical facility, according to the Tennessee Department of Correction (TDOC) and the Associated Press.
The TDOC’s September 30 media advisory said the department followed every step of the established protocol approved by the Attorney General’s Office. It also said the protocol did not allow additional procedures beyond those carried out that evening. The statement confirmed Pike’s transport, but did not describe her medical condition or explain why the execution was not completed.
What do Tennessee officials and Pike’s lawyers say?
| Source | What it said | What the account establishes |
|---|---|---|
| Tennessee Department of Correction, September 30, 2026 | TDOC said staff followed the established protocol and that it did not permit further procedures. The department stated: “The lethal injection chemical in the protocol has consistently been effective, and the protocol does not allow for additional procedures beyond what was carried out this evening.” | The department’s position on protocol and confirmation that Pike was taken to a medical facility. The statement did not disclose a medical finding about the cause or Pike’s condition. |
| Pike’s attorneys, as reported by the Associated Press on October 2, 2026 | Her lawyers alleged that the IV lines were incorrectly placed or that veins had blown, causing pentobarbital to enter her body rather than her bloodstream. They reported that Pike was unconscious, on a ventilator, and receiving hospital treatment. | Counsel’s allegations about IV access and their account of Pike’s condition—not an official medical finding in the available record. |
These accounts address different aspects of the event: TDOC described its compliance with the protocol, while Pike’s attorneys alleged a failure in IV access and reported her condition. The information available as of October 3 did not resolve the cause of the attempt’s failure.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What remains unresolved?
- Why the execution was not completed: TDOC said the protocol was followed; Pike’s attorneys alleged that IV access failed. No independent finding resolving the competing accounts was available.
- Pike’s medical condition: Her attorneys reported on October 2 that she was unconscious and on a ventilator. TDOC confirmed transport to a medical facility but did not provide a condition, and the available sources did not establish a later confirmed status.
- What happens to Pike’s sentence: The records and reporting available by October 3 did not establish a final legal outcome after the attempt or a new execution date.
- What the independent review will find: The Governor ordered a review, but its findings were not yet available.
The litigation before the attempt also addressed proposed alternatives to the state’s lethal-injection procedure, including a 23-gauge butterfly needle and hanging. Those were proposals Pike made in litigation, as described by the Death Penalty Information Center in August 2026; they were not adopted methods or judicial conclusions.
