China’s Healthcare Resource Productivity Stagnated in 2018–2022, Study Finds

A study of all 31 Chinese provinces from 2003 to 2022 reports that average healthcare-resource allocation productivity was below 1 in every year from 2018 through 2022. The authors identify technological progress as a weak component and find persistent regional differences. This is a finding about comparative resource-allocation indices—not proof that healthcare quality or patient outcomes fell by the same amount. The study is descriptive, so it cannot establish what caused the pattern.

What the study measured—and what it did not

Published online on 5 October 2026, Huang and colleagues’ early-access article analyzes provincial panel data for all 31 Chinese provinces over 2003–2022. The publisher describes the article as an early-access version that may be edited and replaced by a final Version of Record. Read the study in BMC Health Services Research.

The authors combine three methods to answer different questions. DEA-Malmquist indices assess changes in efficiency and productivity; the Dagum Gini coefficient decomposes regional inequality; and the GM(1,1) grey model simulates future resource supply and demand. Together, these methods describe patterns in how measured inputs are converted into measured outputs and how those patterns vary by region. They do not provide a single all-purpose grade for China’s healthcare system.

  • TFP below 1: The study reports mean total factor productivity below 1 from 2018 to 2022, within a full-period trajectory the authors characterize as fluctuating downward. This is a model-index finding, not a measurement that health, quality, access, or life expectancy declined by an equivalent amount.
  • Techch of 0.929: The authors identify the technology-progress index as a weak component associated with the TFP pattern. The value should be read as the reported index, not translated into a 7.1% annual decline or a direct measure of clinical technology adoption.

The authors’ abstract summarizes the result this way: “This descriptive assessment shows that healthcare resource allocation in China over 2003–2022 was characterised by stagnating technological progress, a persistent East-versus-rest gradient in productivity change, and rising nominal costs.”

Why productivity can lag even as resources become more evenly distributed

The study’s equity findings concern two distinct things: the distribution of physical resources and the distribution of productivity change. According to the authors, inequality in physical resources per person declined over the period, while inequality in productivity change did not. A more even distribution of beds or staff therefore does not, by itself, show that provinces are converting those resources into services equally effectively.

The authors find a high-in-the-East, low-in-the-West pattern in productivity change and statistically distinguishable technological-change differences between the East and the other two regions in their province-level bootstrap analysis. They identify the East-West gradient as the root of overall inequality. The 2022 Dagum decomposition attributes 52.96% to trans-variation density; the paper offers overlap between regional distributions as one possible interpretation of that component.

The study also reports that decreasing returns to scale are concentrated in richer regions. This suggests that simply expanding scale may have limits in those settings, but the analysis does not establish the cause of the pattern or show that any particular management or investment change would reverse it.

The 2029 hospital-cost figure is a model projection, not a bill today

The grey-model simulation projects an average hospitalization cost of approximately CNY 15,587 per visit by 2029 in nominal terms. It extrapolates from historical data for 2003–2022; it is not an observed current cost, an inflation-adjusted estimate, or a certain forecast. It should be treated as an illustrative trajectory from one model rather than a prediction of what a particular patient or hospital will pay.

How this compares with earlier evidence

Earlier studies provide context, but their periods, inputs and outputs differ from the 2026 analysis. Their results are not direct replications of its provincial productivity index.

Study and period What it examined Reported result
Huang et al., 2003–2022 All 31 provinces; DEA-Malmquist productivity analysis, regional inequality decomposition and grey-model projection. Mean TFP below 1 in 2018–2022; Techch 0.929; persistent East-West differences. Study
Chai et al., 2004–2015 Provincial health-system productivity before and after the 2009 reform, using health expenditure, medical personnel and hospital beds per 1,000 residents as inputs and health outcomes as outputs. The authors report negative productivity effects after the reform period, improved scale efficiency and declining technological change. They associate stronger productivity growth with higher GDP per capita and a higher medical-staff-to-bed ratio; aging, lower educational attainment and higher out-of-pocket payments with adverse context. This is a separate study with a different period and specification. Study
BMC Health Services Research study, 1993–2012 Decomposition of health-expenditure growth. Annual health-expenditure growth was 11.6%, compared with annual economic growth of 9.9% over that period. Increased real expenditure per prevalent disease case contributed 8.4 percentage points; excess health-price inflation and population growth contributed 1.3 points each; aging contributed 0.8 points; and declining prevalence reduced growth by 0.3 points. These are historical estimates, not current growth rates. Study

Read together, the older studies make expenditure per case and technological change relevant areas to examine, but they do not explain why the later index behaved as it did. Differences in years, inputs, outputs and methods matter when comparing productivity results.

Broad insurance enrollment does not mean equal financial protection

The World Bank’s December 2024 China Economic Update reports National Healthcare Security Administration data showing health-insurance coverage above 95% in 2024. It also describes differences between insurance schemes: members of the resident program face higher deductibles and copayments, lower outpatient reimbursement and capped fund reimbursement than members of the urban employee program. Serious illness can still leave families paying a substantial share themselves. Enrollment coverage is therefore not the same as equal benefits or full protection from medical costs. What can—and cannot—be concluded

The evidence supports a bounded conclusion: the study’s measure of resource-allocation productivity stagnated or declined in its later years even as the distribution of physical resources per person became more even. It does not establish that China’s healthcare system as a whole produced less health, nor does it show that a specific policy, incentive, technology choice or management practice caused the findings.

The 2018–2022 period includes both reform developments and the COVID-19 period. Because the analysis is descriptive and does not identify causal effects, it cannot assign the reported trend to either. The productivity result is a reason to ask how resources are translated into services and how regional gaps persist—not a standalone verdict on patient care.
