Neither chemical nor physical exfoliation is proven to work better for everyone with keratosis pilaris (KP). If you want to treat the rough bumps, choose a gentle approach you can tolerate: a keratolytic moisturizer, light surface exfoliation, or both. Avoid vigorous scrubbing, moisturize regularly, and reduce or pause products if your skin becomes irritated.
What keratosis pilaris is—and whether it needs treatment
KP causes small, rough bumps, commonly on the upper arms and thighs. They form when dead skin builds up around hair follicles. KP is common, not contagious, and does not need treatment if it does not bother you. The American Academy of Dermatology Association (AAD) overview and DermNet’s March 2022 guidance describe the condition and its typical appearance.

Chemical and physical exfoliation: what’s the difference?
| Approach | How it works | Examples | Main caution |
|---|---|---|---|
| Chemical (keratolytic) | Ingredients help loosen or remove dead surface skin and follicular buildup. | Lactic acid, glycolic acid, salicylic acid, other alpha hydroxy acids (AHAs), urea, and retinoids. | Follow the product or clinician’s directions. Overuse can leave skin raw, dry, or irritated. |
| Physical | Gentle friction lifts dead skin from the surface. | A soft washcloth, loofah, exfoliating sponge, or mitt. | Hard rubbing and harsh abrasives can irritate skin and worsen KP. |
| Moisturizing support | Helps relieve dryness and support skin comfort. | A cream or ointment; some moisturizers contain urea or lactic acid. | Apply after bathing while skin is damp; reapply when skin feels dry. |
A product can be both a moisturizer and a chemical exfoliant: for example, a moisturizer containing urea or lactic acid. The AAD’s treatment guidance lists these ingredients among options for KP.
Which should you choose?
There is no direct head-to-head evidence in the cited guidance showing that chemical exfoliation is superior to gentle physical exfoliation, or vice versa. Choose based on comfort and ease of use rather than assuming stronger exfoliation will produce better results.

- Consider a keratolytic moisturizer if you prefer an ingredient-based product that can address roughness while moisturizing. Follow its label or your clinician’s instructions.
- Consider gentle physical exfoliation if you prefer a simple tool such as a soft washcloth. Use light pressure; the aim is not to scrub the bumps off.
- Use both only if your skin tolerates the routine. AAD self-care guidance describes gentle exfoliation, a keratolytic product, and then moisturizer, but this is general guidance—not a prescription for every person.
How to build a gentle routine
- Keep bathing mild. The NHS recommends lukewarm bathing and mild, unperfumed products. Avoid scratching, picking, rubbing hard, or using harsh scrubs.
- If exfoliating physically, use a soft tool with a light touch. A washcloth or exfoliating mitt is optional; vigorous friction can irritate skin.
- Apply a keratolytic only as directed. Ingredient strength and directions differ by product, so do not assume that more frequent or heavier application will help.
- Moisturize after bathing. AAD treatment guidance recommends applying moisturizer within five minutes, while skin is damp, and at least two or three times a day. Its self-care guidance also advises moisturizing after bathing and whenever skin feels dry.
- Adjust if irritation develops. AAD self-care guidance advises pausing a keratolytic for a few days if skin becomes too dry or irritated. Seek clinician advice for persistent irritation or symptoms that concern you.
What results to expect
Treatment may improve KP’s appearance or symptoms, but it does not cure the condition; maintenance may be needed. AAD recommends contacting a dermatologist if a treatment plan has not helped after four to six weeks. That is a follow-up benchmark, not a promise that everyone will see improvement by then.
If you are unsure that a rash is KP, or it persists despite a gentle routine, a clinician can assess it. The AAD notes that diagnosis includes visual examination.
