ACIP evaluates the evidence and votes on recommendations; the CDC Director must adopt that recommendation before it becomes official CDC policy. CDC then publishes the policy, including vaccine-specific guidance in the MMWR and on CDC webpages. This distinction explains why an ACIP vote is an important decision point but is not, by itself, the final CDC vaccine schedule.

CDC and ACIP have different jobs

The Advisory Committee on Immunization Practices (ACIP) is an external committee of medical and public-health experts. Its chartered role is to provide expert advice and guidance to the Director of the Centers for Disease Control and Prevention (CDC) and to the Secretary of the Department of Health and Human Services (HHS) on using vaccines and related agents to control vaccine-preventable disease in the U.S. civilian population.

As an Amazon Associate I earn from qualifying purchases.

The CDC Director is the official adoption point for CDC policy. After ACIP votes, the Director reviews and adopts the recommendation. Once adopted, CDC publishes the resulting policy, commonly through a vaccine-specific recommendation in the MMWR and corresponding CDC guidance pages.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical shorthand is:

  • ACIP recommends. Its members assess evidence, weigh benefits and harms, and vote.
  • The CDC Director adopts. An ACIP recommendation becomes official CDC policy only after this step.
  • CDC publishes and implements. The agency communicates the adopted recommendation and incorporates it into clinical and public-health guidance.

HHS matters because ACIP’s charter includes advice to the HHS Secretary, but the operational step that turns an ACIP recommendation into CDC policy is adoption by the CDC Director.

The civilian-population wording is also important. ACIP’s charter addresses vaccine use for people in the U.S. civilian population. That is the policy setting in which its evidence reviews and votes are understood; it is not a statement that every setting or every individual receives identical advice without regard to the final population, product, schedule, or clinical circumstances.

CDC versus ACIP: the decision chain at a glance

Question ACIP CDC and its Director
Institutional role External advisory committee of medical and public-health experts Federal public-health agency; the CDC Director is the adoption authority for CDC policy
Decision point Members discuss evidence and vote on a recommendation The CDC Director adopts or does not adopt the recommendation
Output Advice on how vaccines should be used in the U.S. civilian population Official CDC policy after adoption
Evidence work Work groups conduct or use systematic reviews, assess certainty with GRADE, and complete an Evidence to Recommendations (EtR) framework Publishes the adopted recommendation and supporting policy documents
Public record Evidence profiles, GRADE assessments, EtR materials, meeting discussions and votes Final recommendations, including vaccine-specific MMWR publications and CDC guidance pages

Therefore, asking “Does CDC or ACIP make the recommendation?” has two answers depending on what “make” means: ACIP makes the expert recommendation, while the CDC Director makes it official CDC policy through adoption.

What ACIP’s scope means in practice

ACIP is not a single-study review panel and it is not simply a communications arm of CDC. Its work groups develop the technical record that members use when they deliberate. The committee’s advice concerns how a vaccine or related agent should be used for a defined civilian population, not an abstract declaration that a product is universally appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

That scope is why the population and outcomes are specified before the evidence is rated. A recommendation can be written for a particular age group or other defined population, with a particular product, schedule, comparator, and set of benefits and harms. A news report that omits those boundaries may be directionally correct but still too broad to guide an individual decision.

The CDC Director’s role is also distinct from independently designing the evidence review. The work groups supply the systematic review, evidence profiles, GRADE judgments, and EtR analysis. The Director’s adoption is the formal step that gives the voted advice the status of CDC policy.

What happens before an ACIP vote

ACIP does not ordinarily decide a vaccine question by looking at one trial or one efficacy percentage. Its work groups follow a staged evidence process intended to locate the relevant research, judge how reliable it is, and then document the policy considerations that connect evidence to a recommendation.

1. A work group defines the assignment

ACIP forms a guideline-development panel or work group for the vaccine question. The group sets the scope: which population is under consideration, what vaccine or related agent is being evaluated, what comparison is relevant, and which outcomes matter for the decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Defining the assignment first helps prevent a review from drifting toward whichever studies or outcomes are easiest to find. It also makes later documents easier to audit: a reader can compare the question the group set out to answer with the evidence it ultimately summarized.

2. The group writes a PICO question

The question is organized using PICO:

  • Population: the people to whom the recommendation might apply.
  • Intervention: the vaccine, dose, schedule or other immunization strategy being considered.
  • Comparator: an alternative vaccine, schedule, no vaccination or another relevant reference.
  • Outcomes: benefits and harms that matter to patients and policy makers.

Work groups identify outcomes as critical or important before rating the evidence. This prevents the review from focusing only on a convenient result while overlooking outcomes that could change the recommendation. It also separates the question of what happened in a study from the question of which outcomes deserve the greatest weight in policy deliberation.

3. Evidence is found systematically

The ACIP handbook says evidence must be identified and retrieved systematically before the GRADE assessment begins. A work group may use a high-quality published systematic review or conduct a new review when an adequate one is unavailable. Systematic methods are meant to capture the full body of evidence relevant to the PICO question, rather than relying on selected studies.

This step is a safeguard against selective attention. A single favorable trial, a single safety report, or an isolated headline cannot substitute for a search designed around the complete question. The aim is to identify the relevant studies and then explain what the collection of studies shows, including gaps and inconsistencies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Results are synthesized

Researchers quantitatively combine results when the studies can be meaningfully analyzed together. They also provide a narrative summary when differences among studies make statistical pooling inappropriate. The result is an evidence profile that shows what was studied, how large the effects were, and where uncertainty remains.

Quantitative synthesis can provide a combined estimate when the underlying studies are sufficiently comparable. Narrative synthesis is not a failure to analyze; it is the appropriate way to describe evidence when populations, interventions, outcomes, or designs do not support a responsible pooled estimate. Both forms of synthesis help the committee see the whole evidence base rather than one isolated result.

5. Certainty is rated with GRADE

GRADE—the Grading of Recommendations, Assessment, Development and Evaluation approach—assesses how confident the work group can be in the estimated effects. ACIP commonly presents certainty separately for benefits and harms. That matters because evidence may be highly certain for one outcome but less certain for another; a strong benefit estimate does not automatically mean the safety evidence is equally certain.

In practical terms, a GRADE assessment tells readers how much confidence to place in an effect estimate, not whether the committee must recommend or reject vaccination. The evidence profile preserves that distinction by showing the outcome, the estimated effect, and the certainty judgment associated with it.

6. The EtR framework records the policy judgment

Evidence alone does not determine a population-wide policy. The Evidence to Recommendations (EtR) framework makes the transition from study results to a recommendation explicit. It records the considerations behind the judgment, including:

  • the balance of expected benefits and harms;
  • the type and certainty of the evidence;
  • values and preferences of affected people;
  • resource use and health-economic implications;
  • acceptability to patients, clinicians and other groups;
  • feasibility of delivering the intervention; and
  • equity implications.

These categories explain why two discussions with similar efficacy estimates can still require different policy judgments. A vaccine may have meaningful expected benefits, but the committee must also consider harms, the resources required to deliver it, whether people and clinicians are likely to accept it, whether delivery is feasible, and how the policy affects equity. EtR places those judgments alongside the evidence instead of leaving them implicit.

7. The committee formulates and votes on a recommendation

After reviewing the evidence profiles and EtR considerations, the work group presents a proposed recommendation to ACIP. Members deliberate and vote. The vote is the committee’s formal advice; it is not yet official CDC policy until the CDC Director adopts it.

The vote therefore marks the end of ACIP’s advisory deliberation, not the end of the institutional chain. Readers who see a vote reported in real time should still look for the subsequent adoption and final CDC publication before treating the language as the operative policy.

Why GRADE and EtR are separate

GRADE answers an evidence question: How certain are we that the estimated benefits and harms reflect what would happen in the relevant population? It addresses limitations such as the design and consistency of studies, the precision of estimates and how directly the evidence applies to the decision.

EtR answers a policy question: Given the evidence and its certainty, does this intervention make sense for the population and the health system? Two policy options could have similarly certain evidence but different feasibility, equity, acceptability or resource-use implications. Conversely, evidence with some uncertainty might still support a recommendation when the potential benefit is important, harms are limited and implementation is practical.

This is why an ACIP recommendation is not a mechanical translation of one efficacy number. Members must make a documented judgment about outcomes, uncertainty and the circumstances in which vaccination will be delivered.

The separation also improves transparency. A reader can ask two different questions: first, whether the underlying effects are well established; second, how the committee weighed those effects against values, costs or resource use, acceptability, feasibility and equity. GRADE answers the first question, while EtR shows the reasoning used for the second.

What the evidence record lets readers inspect

The process creates several distinct records rather than one opaque conclusion:

  • EtR framework: the policy considerations that connect evidence to the proposed recommendation.
  • ACIP discussion and vote: the committee’s formal advisory action.
  • CDC adoption and publication: the step that turns the advice into official CDC policy and communicates the final wording.

Looking at these artifacts in order helps resolve disagreements that arise from quoting only a single sentence. A debate may concern the size of a benefit, the certainty of a safety estimate, or the feasibility of delivery; the relevant answer may appear in a different part of the record.

Worked example: the 2024–2025 COVID-19 vaccine recommendation

The process can be seen in the ACIP COVID-19 Vaccines Work Group’s review for the 2024–2025 COVID-19 recommendation for people aged 6 months and older.

The work group used GRADE to assess the certainty of evidence on benefits and harms associated with an updated COVID-19 vaccine—described in the methods as a bivalent or 2023–2024 vaccine—administered in the United States during September 2022 through May 2024. That date range identifies the evidence and vaccine-use period considered in the review; it is not a claim that every study or every person was observed for the same length of time.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The work group then used EtR to consider the importance of COVID-19 as a public-health problem, resource use, benefits and harms, patient values, acceptability, feasibility and equity. Those considerations were part of the recommendation process alongside the GRADE assessments, not a substitute for them.

This example shows how the two frameworks work together. The GRADE portion asks how certain the work group is about the estimated benefits and harms of the updated vaccine. The EtR portion asks whether those effects, in the context of COVID-19 as a public-health problem and the practical conditions of delivery, support the policy being considered for people aged 6 months and older.

Once ACIP completed its deliberation and vote, the recommendation still required adoption by the CDC Director to become official CDC policy. The final policy was then published through CDC’s formal recommendation channels.

How to verify where a recommendation stands

When a headline says “ACIP recommends,” check which stage it describes. The following sequence prevents the most common misunderstanding:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Locate the ACIP action. Confirm the vaccine, population, schedule and the date of the committee vote.
  2. Check for CDC Director adoption. An ACIP vote remains advisory until the Director adopts it.
  3. Read the final CDC publication. The adopted recommendation is published in the relevant MMWR document and linked CDC guidance.
  4. Use the final wording for clinical or public-health decisions. Adoption documents can contain conditions, age ranges, timing, product details or exceptions that a meeting summary does not fully capture.

Readers can also inspect the evidence profiles, GRADE tables and EtR materials when they want to understand how the committee moved from individual studies to a policy judgment.

A useful verification checklist is to compare the population in the headline with the population in the adopted text, then compare the product and schedule details. Next, check whether the statement describes a recommendation, an adoption, or merely a discussion. Finally, read the MMWR or linked CDC page for the wording that clinicians and public-health programs are expected to use.

What the distinction means for patients and clinicians

An ACIP vote can signal a change without being the final rule

News coverage often reports the vote because it is the visible committee decision. Until CDC Director adoption, however, the vote is not the final CDC policy that appears in official recommendations. Timing matters when a schedule, eligibility rule or product recommendation is changing.

The final recommendation may be narrower than the headline

ACIP recommendations are written for defined populations and outcomes. Age, immune status, prior vaccination, product type, dose timing and other conditions can affect the actual recommendation. The adopted CDC language, rather than a general statement that “ACIP supports the vaccine,” supplies the operative details.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Uncertainty is not hidden by a single label

GRADE can show different certainty levels for benefits and harms. EtR then makes visible how the committee handled that uncertainty with respect to values, resources, feasibility, acceptability and equity. A recommendation therefore communicates both a policy direction and the reasoning boundaries around it.

Resource considerations are part of the decision

EtR includes resource use and health-economic implications. That does not reduce the recommendation to a price tag. It means the committee records how resources and delivery requirements fit with the expected benefits, harms and practical implementation of the policy.

Implementation is not an afterthought

Feasibility and acceptability are explicit EtR considerations. A policy must be considered in the circumstances in which people receive vaccines, clinicians deliver them, and public-health systems communicate and support them. Those considerations help explain why the final policy language may include defined conditions rather than a one-size-fits-all statement.

Common misconceptions

“ACIP is part of CDC, so its vote is automatically CDC policy.”

ACIP advises CDC, but the CDC Director’s adoption is a separate step. The recommendation becomes official CDC policy only after that adoption.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

“The CDC Director independently creates the vaccine schedule.”

The Director’s formal role in this chain is adoption. ACIP work groups perform the systematic evidence review, GRADE assessment and EtR analysis that support the recommendation the Director considers.

“GRADE decides the recommendation.”

GRADE rates certainty in the evidence. It does not by itself choose the policy. EtR documents the broader judgment about benefits, harms, values, resources, acceptability, feasibility and equity.

“A recommendation is based on one decisive study.”

The ACIP process begins with a systematic search and may use a published systematic review or a de novo review. Results are synthesized quantitatively when appropriate and summarized narratively when they cannot be pooled responsibly.

“An ACIP vote tells me exactly what to do.”

A vote identifies the committee’s advice, but the adopted CDC language supplies the official population, product, timing, conditions and exceptions. A meeting headline should not replace the final recommendation when a clinical or program decision depends on those details.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“Lower certainty means no recommendation is possible.”

GRADE describes confidence in the evidence; it does not automatically dictate the outcome. EtR allows the committee to consider the importance of the public-health problem, expected benefits and harms, values, resources, acceptability, feasibility and equity when evidence is not equally certain for every outcome.

Questions readers commonly ask

Does ACIP make the U.S. vaccine schedule?

ACIP recommends how vaccines should be used for the U.S. civilian population. The CDC Director must adopt that recommendation before it becomes official CDC policy, and CDC then publishes the adopted guidance.

What is the difference between an ACIP recommendation and CDC policy?

An ACIP recommendation is the committee’s expert advice after its evidence review and vote. CDC policy is the recommendation after adoption by the CDC Director and publication through CDC’s formal channels.

Why does the CDC Director’s adoption matter?

Adoption is the institutional step that gives the ACIP recommendation official CDC-policy status. Without it, the vote remains advisory.

What does PICO have to do with vaccines?

PICO structures the review around the population, intervention, comparator and outcomes. It makes clear who is being considered, what strategy is under review, what it is compared with, and which benefits or harms are important.

Can ACIP use an existing systematic review?

Yes. The handbook process allows a work group to use a high-quality published systematic review or conduct a new review when an adequate review is not available.

Are benefits and harms given the same certainty rating?

Not necessarily. ACIP commonly presents certainty separately for benefits and harms, so confidence can differ by outcome.

Where can I see how the recommendation was developed?

Look for the evidence profiles and GRADE tables, the EtR framework, ACIP meeting discussions and vote, and the final MMWR recommendation with its linked CDC guidance page.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does the EtR framework replace clinical judgment?

No. EtR documents the population-level policy judgment. The final CDC wording still defines the relevant population and conditions, while individual care may require applying that policy to a person’s circumstances.

The simplest accurate answer

If someone asks who makes U.S. vaccine recommendations, say: ACIP is the expert advisory body that reviews the evidence and recommends how vaccines should be used; the CDC Director adopts the recommendation, and that adoption makes it official CDC policy. The evidence trail runs from a defined PICO question and systematic review through GRADE evidence profiles and an EtR framework, then to the ACIP vote and CDC publication.

Frequently Asked Questions

Does ACIP or CDC make vaccine recommendations?

ACIP reviews the evidence and votes on the recommendation. The CDC Director must adopt it before it becomes official CDC policy, which CDC then publishes.

Is an ACIP vote immediately binding CDC guidance?

No. The vote is advisory until the CDC Director adopts the recommendation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How does ACIP judge vaccine evidence?

Work groups define a PICO question, use a published systematic review or conduct one, synthesize results, assess certainty with GRADE, and use the EtR framework to document policy considerations.

What does GRADE measure?

GRADE measures certainty in the evidence for estimated benefits and harms. Certainty can differ between outcomes.

What does the Evidence to Recommendations framework add?

EtR records the balance of benefits and harms, evidence certainty, values and preferences, resource use, acceptability, feasibility and equity.

Where is the final recommendation published?

After CDC Director adoption, CDC publishes the recommendation through the relevant MMWR document and linked CDC guidance pages.

The Bottom Line

Bottom line: ACIP is the expert advisory committee; the CDC Director is the adoption authority. Systematic reviews, GRADE assessments and EtR frameworks show how evidence and policy considerations lead to an ACIP vote, while CDC adoption and publication make the recommendation official policy.
