A National Institutes of Health analysis of U.S. data from 2010 through 2019 found that 14 of 33 cancer types rose in at least one younger age group, while 19 declined. The overall rate of cancer and overall cancer death did not increase among people under 50.
Scientists see a mixture of possible explanations—earlier exposure to established risks, obesity, alcohol, environmental and microbiome changes, birth-cohort effects, and better detection—but no single cause has been proved. The findings do not justify blaming one food, product or behavior.

What “younger adults” means in the latest U.S. analysis
The National Cancer Institute’s 2025 analysis defines early-onset cancer as a diagnosis before age 50. It separates people into three groups: ages 15–29, 30–39 and 40–49. Other adolescent and young-adult cancer programs may use different age ranges, so “under 50” is the clearest label for this particular evidence.

Which cancers are increasing?
Four cancers accounted for more than 80% of the additional U.S. early-onset cases estimated for 2019 compared with what would have been expected if 2010 rates had continued. The estimates are not a count of every case caused by a particular factor; they are a comparison with an earlier rate.
| Cancer type | Estimated additional U.S. cases in 2019 | Qualification |
|---|---|---|
| Female breast | About 4,800 | Estimated excess versus the expected 2019 total at 2010 rates |
| Colorectal | About 2,100 | Estimated excess versus the expected 2019 total at 2010 rates |
| Kidney | About 1,800 | Estimated excess versus the expected 2019 total at 2010 rates |
| Uterine | About 1,200 | Estimated excess versus the expected 2019 total at 2010 rates |
| Pancreatic | About 500 | Estimated excess versus the expected 2019 total at 2010 rates |
Nineteen of the 33 cancers studied declined in at least one under-50 age group. Nine of the 14 cancers that increased among younger people also increased in at least one older age group, suggesting that some influences affect more than one generation. Five rose only in a younger group in this analysis: melanoma, cervical cancer, stomach cancer, myeloma, and bone and joint cancers.
Overall cancer mortality remained level among people under 50, but deaths from colorectal and uterine cancers increased at younger ages. NCI researchers describe those mortality changes as concerning because they cannot be dismissed as diagnosis alone.

The international signal is clearest for colorectal cancer
Early-onset colorectal cancer is not solely a U.S. phenomenon. NCI reports that nearly 10% of new colorectal cancers worldwide occur in people under 50. An American Cancer Society-led analysis published in The Lancet Oncology in 2024 found that incidence rose among younger adults in 27 of 50 countries examined.

Country patterns differ, however. A rise in 27 countries does not establish one worldwide rate or prove that the same exposure is responsible everywhere. Differences in diet, obesity, alcohol use, medical access, screening, diagnosis and cancer registration can all affect comparisons.
What might be driving the increase?
The explanations below are active hypotheses with different levels of evidence. Biological plausibility is not the same as demonstrated population-level causation.
Risk factors appearing earlier in life
NCI scientists say causes are likely to be cancer-specific and may include established risk factors becoming more common at younger ages. Obesity is the leading hypothesis for several cancers. NCI summarized a 2024 analysis covering 21 cancers, adults ages 25–49, 42 countries and the years 2000–2012. Six of nine obesity-related cancers increased among young adults, and increases in body weight were associated with colon, rectal, pancreatic and kidney cancers.
Those are population associations. They do not show that obesity caused each individual cancer, and they do not explain cancers that do not share the same relationship with body weight.

Screening, imaging and diagnostic changes
More imaging, surveillance of people known to be at high risk, revised screening recommendations, improved pathology and changes in diagnosis or coding can raise the number of recorded cases. These effects may account for part of an increase, particularly where detection has changed rapidly.
They cannot automatically explain increases occurring below routine screening ages or similar trends in multiple countries. Researchers therefore treat detection as one contributor rather than a complete answer.
Birth-cohort effects
People born from the 1950s onward may have encountered a “whole package” of changing environmental, lifestyle and other exposures. NCI researchers propose that this combination could move the time of cancer development earlier. A cohort effect can fit a pattern that appears first in younger generations and then follows them as they age, but it remains a hypothesis rather than a confirmed mechanism.
Gut microbiome and infectious mechanisms
Changes in the composition of gut microbes are being studied as possible influences on colorectal and other cancers. NCI researchers are also examining a DNA-damaging toxin produced by some E. coli strains. These are mechanistic clues from laboratory and epidemiologic work, not proof that a particular bacterium explains the population trend or that changing the microbiome prevents cancer.

Inherited genetic susceptibility
Inherited mutations can substantially raise risk for some people. NCI cites a Memorial Sloan Kettering study in which nearly 20% of patients under 35 with cancer carried a predisposition identified through that study. Genetic counseling and testing can therefore matter for an individual patient and relatives.
Inherited genetics cannot by itself explain a population-wide increase over a few decades: human genomes do not change that quickly. Comparisons of tumor genetics across generations have produced conflicting findings. As NCI’s Rihab Yassin, Ph.D., put it, findings on specific genetic contributors “have been conflicting.”
Alcohol and environmental exposures
NCI lists heavy alcohol use, microplastics and other environmental exposures as possible contributors. For many individual factors, strong epidemiologic evidence linking them specifically to early-onset cancer is still lacking. Fred Hutch Cancer Center’s Ulrike Peters, Ph.D., cautioned: “For many of these [factors], there’s no strong epidemiological evidence that they’re [individually] linked to early-onset cancers.”
How to weigh the competing explanations
| Explanation | What supports it | What remains uncertain |
|---|---|---|
| Earlier or more common risk factors | Associations with obesity and other established risks; patterns differ by cancer | Association does not prove cause for an individual; many cancers do not share the same pattern |
| Detection and coding | Screening, imaging, surveillance and coding changes can increase recorded incidence | Cannot by themselves account for every rise, especially below routine screening ages or across countries |
| Birth-cohort effects | Could explain earlier disease in generations exposed to changing conditions | The specific exposure package and mechanism have not been identified |
| Microbiome or infectious mechanisms | Biological mechanisms such as DNA-damaging bacterial toxins are under study | Mechanistic clues have not established a population-level explanation |
| Inherited genetics | Predisposition syndromes explain some patients and families | Genomes cannot shift fast enough to explain a broad rise; tumor-genetic evidence is conflicting |
| Alcohol and environmental chemicals | Known or plausible carcinogenic pathways exist for some exposures | Evidence for many specific early-onset links, including microplastics, is not yet strong |
MD Anderson’s Y. Nancy You, M.D., summarized the uncertainty this way: “I don’t think there’s going to be a single ‘smoking gun’… It’s the million-dollar question everybody [is asking]. But it’s a long-horizon answer.”

What younger adults should do now
Do not dismiss warning symptoms because of age
Being young lowers the odds of many cancers, but it does not rule cancer out. NCI clinicians highlight rectal bleeding, persistent abdominal pain, diarrhea and iron-deficiency anemia as symptoms that warrant prompt medical evaluation, especially when they are new, recurring or worsening. These signs have many noncancer causes and are not a diagnosis; the point is to avoid delaying assessment solely because someone is under 50.
Screening is still individualized
The increase in early-onset cancers does not create a universal recommendation for every young adult to begin screening early. Decisions depend on symptoms, family history, known genetic syndromes, prior medical conditions, personal risk factors and the guidelines used in the person’s country. A clinician can determine whether earlier or more frequent testing is appropriate.
Ask about genetics after a diagnosis
After cancer is diagnosed, genetic counseling and testing can identify inherited syndromes such as Lynch syndrome. Results may influence treatment, determine whether relatives should be tested and guide future screening. Testing is most useful when interpreted with a counselor or qualified clinician rather than as a stand-alone consumer result.
What the evidence does—and does not—establish
The clearest conclusion is not that all young people are getting cancer or that one modern exposure is responsible. It is that several cancer-specific patterns have shifted, most visibly for breast, colorectal, kidney and uterine cancers in the United States, while other cancers have declined. Incidence and mortality must be tracked separately, and the causes may differ by cancer, age group and country.
NIH’s Meredith Shiels, Ph.D., described the likely explanation as cancer-specific risk factors becoming more common at younger ages, combined with changes in screening, detection, diagnosis and coding. That framework fits the data better than a single-cause claim, but researchers are still working to determine how much each component contributes.
