Cancer cachexia is more than losing weight: it is a disease-related syndrome involving loss of skeletal muscle, often alongside loss of body fat, reduced appetite, fatigue, and weakness. Cancer and its treatments can also cause unintentional weight loss for other reasons, so weight loss—or appetite loss—alone does not establish cachexia. Persistent changes deserve discussion with the oncology team because cachexia can involve inflammation and altered metabolism, not just inadequate food intake.
What distinguishes cachexia from other cancer-related weight loss?
“Ordinary” weight loss is not a precise medical diagnosis. In someone with cancer, weight may fall because symptoms or treatment make eating difficult, or because the disease affects the body in other ways. Cachexia is a specific, complex wasting syndrome in which weight loss includes skeletal-muscle loss and is associated with changes in appetite and metabolism. The National Cancer Institute (NCI) describes cachexia as involving loss of muscle and fat, weakness, fatigue, and appetite loss; the American Society of Clinical Oncology (ASCO) likewise emphasizes loss of appetite, weight, and skeletal muscle. These patterns can overlap. Nausea, pain, mood symptoms, taste changes, swallowing difficulties, or feeling full quickly may reduce food intake and contribute to weight loss, whether or not cachexia is present. Fatigue and weakness can also have multiple causes. Symptoms are important clues, but no single symptom distinguishes cachexia from other cancer or treatment effects. Symptoms to notice
Possible signs of cachexia include:
- Unintentional weight loss, often with visible loss of muscle and sometimes body fat.
- Reduced appetite or eating less than usual.
- Fatigue, weakness, or declining strength and ability to carry out daily activities.
- Early fullness, nausea, bloating, altered taste, or difficulty swallowing that interferes with eating.
Muscle loss may not be obvious on the scale, and it can coexist with a high body mass index (BMI). A stable or higher BMI therefore does not, by itself, rule out loss of muscle or nutritional problems. The pattern over time and a clinical assessment matter more than any one measurement. Why cachexia happens
Cachexia is not explained by one universal cause. Reduced intake can contribute—for example, when appetite changes, nausea, pain, taste alterations, early fullness, or swallowing problems make eating harder. At the same time, cancer-related inflammation and metabolic changes can accelerate protein breakdown and contribute to loss of muscle. Reduced physical activity and treatment effects may also be part of the picture. These influences can operate together, which is why simply eating more may not fully address the syndrome. How clinicians assess possible cachexia
A commonly cited international consensus definition, reported in ASCO’s 2020 guideline, is more than 5% weight loss over six months, or 2%–5% weight loss when BMI is below 20 kg/m² or muscle mass is reduced. This threshold is a clinical reference, not a self-diagnosis test: circumstances and definitions can vary, and assessment involves more than a scale reading. ASCO guideline.

The care team may review weight history, appetite and nutrition symptoms, muscle mass, strength and daily function, as well as the cancer and its treatment. ESMO recommends regular nutritional screening and support for people receiving anticancer treatment. Dietitian assessment and practical food strategies
ASCO says clinicians may refer people with advanced cancer and appetite or weight loss to a registered dietitian for assessment and practical advice. Depending on the person’s needs, this may include nutrient-dense foods that are higher in protein and calories. Dietary counseling, with or without oral nutrition supplements, increased weight in some studies, but ASCO characterizes the evidence as limited. Medication for appetite or weight loss
ASCO says a clinician may offer a short-term trial of a progesterone analog or a corticosteroid to address appetite or weight loss. Whether either is appropriate depends on weighing potential benefits and risks against the person’s goals; the clinician should consider the intended duration. Tube feeding and intravenous nutrition
ASCO advises against routinely using feeding tubes (enteral nutrition) or intravenous nutrition (parenteral nutrition) to manage cachexia in advanced cancer outside a clinical trial. Match the approach to prognosis and goals
ESMO describes adapting nutrition support to the expected course of illness and the person’s goals: screening and nutritional support during anticancer treatment or when survival is expected to exceed a few months; less invasive measures, such as counseling and oral supplements, when expected survival is shorter; and comfort-focused care when expected survival is a few weeks.
