Can You Trust an AI Diagnosis From a Medical Scan? FAQs for Patients

Sometimes, but never on the word “AI” alone. Software used with an X-ray, CT, MRI, mammogram, ultrasound or another scan may enhance an image, detect or measure a suspicious area, support a radiologist’s interpretation, or simply move an urgent-looking study higher in the review queue. Those functions have different purposes and evidence. In the United States, the clinician who receives the software’s output remains responsible for the diagnostic and patient-management decision in the FDA category covering lesion detection and diagnosis. Ask your care team exactly what the software did, who reviewed the complete scan, and how the result fits your symptoms, history and prior imaging.

What “trusting an AI diagnosis” really means

There is no single technology called an “AI diagnosis.” The label can describe several kinds of medical-device software with different intended uses. Trust therefore has to be specific: trust the particular tool for the task it was designed and evaluated to perform, and trust the clinician’s interpretation of that output in your case.

A scan is only one part of a medical decision. Symptoms, physical findings, previous images, laboratory results, treatment history and the quality of the current images can all change what a finding means. An algorithm generally sees the image data supplied to it; your treating team has to place that information in the full clinical context.

What the software may have done to your scan

Image enhancement or reconstruction

Some systems process the image itself: for example, they may reduce noise, improve visualization or help produce a reconstruction. This can make a study easier to interpret, but image processing is not the same as diagnosing a disease. The radiologist still interprets the resulting images.

Detection, localization or characterization

Other software looks for a feature such as a nodule, mass or other lesion, marks where it is, or estimates characteristics that may help the clinical user. FDA’s classification for “Radiological Computer-Assisted Detection/Diagnosis Software for Lesions Suspicious for Cancer” specifies that primary diagnostic and patient-management decisions are made by the clinical user. A marked or described finding is therefore decision support, not a diagnosis delivered directly to you.

Diagnostic support

A tool may combine image measurements or patterns to support a differential diagnosis or another clinical judgment. Its output is one input to the professional’s interpretation. The intended scan type, target finding, patient population and clinical setting determine whether the supporting evidence is relevant to your examination.

Triage and notification

FDA describes “Radiological Computer-Assisted Triage and Notification Software” as software that prioritizes time-sensitive images for review by specified clinical users. It does not provide image-analysis information beyond triage and notification. An alert can mean that a study was moved nearer the front of a worklist; it is not a final diagnosis and does not tell you, by itself, what is visible on the scan.

Did AI read my scan, or did it just flag it?

The fastest way to find out is to ask the radiology or treating team which function was used. A typical workflow can include several distinct stages:

  1. Image acquisition: The X-ray, CT, MRI, mammogram, ultrasound or other study is produced.
  2. Software processing: An approved tool may enhance images, make measurements, detect or characterize a finding, or assign a study a triage priority.
  3. Clinical review: The designated clinical user reviews the images and the software output. For lesion-detection and diagnosis software, the clinical user makes the primary diagnostic and patient-management decisions.
  4. Report and care plan: The clinician’s interpretation is documented and combined with your history, symptoms and other results to decide whether follow-up, another test or treatment is appropriate.

Hospitals can use more than one software function in a workflow, so “AI was used” does not identify which stage it affected. Ask whether the program produced a finding, changed the image, supplied a measurement or only sent a priority notification.

Software function What the output does Who uses it What it means for a patient
Image processing Changes or improves how image data are displayed or reconstructed Clinical user interpreting the images Better visualization does not equal a disease diagnosis
Detection, localization or characterization Marks or describes a potentially suspicious lesion or feature Clinical user making the primary decision A finding requires professional interpretation and, when appropriate, follow-up
Diagnostic support Provides measurements or pattern-based information to support a clinical judgment Specified clinical user The output is one part of the diagnostic assessment
Triage and notification Prioritizes a time-sensitive study and notifies specified users Clinical users managing the review queue An alert is not a diagnosis and does not itself describe the abnormality

Is a radiologist still reviewing the scan?

For the FDA lesion-detection and diagnosis category, yes: the classification states that primary diagnostic and patient-management decisions are made by the clinical user. Triage software is also designed to prioritize images for specified clinical users rather than replace their review. That does not tell you exactly how your facility used a particular product, so ask who reviewed the full study and who signed or confirmed the interpretation.

If a report mentions an automated mark, score or alert, ask the clinician to explain whether it changed the reading priority, supplied a measurement or influenced the written impression. You can also ask what uncertainty remains and whether the recommended follow-up depends on the software output, the images themselves or both.

How accurate is AI for my type of scan?

General FDA information does not establish one accuracy percentage for all AI scan systems or all patients. A number from one study cannot be transferred safely to a different scan type, disease, patient population or clinical workflow.

When someone presents an accuracy figure, check that it concerns the same:

  • Intended task: triage, image processing, detection, measurement, characterization or diagnostic support.
  • Scan and finding: for example, a particular CT protocol and a defined type of lesion, rather than “all CT scans.”
  • Clinical user: the professionals who receive and act on the result.
  • Outcome: what counted as correct, such as detection, localization, characterization or a management decision.
  • Population and setting: the people, equipment, image quality and workflow represented in the evaluation.

FDA says it evaluates whether studies are appropriate for a device’s intended use and technological characteristics. That is why comparing products by a single headline number can mislead: the studies may have used different populations, reference standards, scan protocols or outcomes.

What “FDA-cleared” or “FDA-authorized” means

The FDA regulates qualifying medical devices that use AI; it does not regulate “AI” as an abstract concept. Depending on the device and its intended use, a manufacturer may use a 510(k) clearance, De Novo classification or premarket approval pathway. These are different regulatory routes, not interchangeable grades of how well a result will apply to you.

The FDA reported that it had authorized more than 1,600 AI-enabled medical devices for marketing in the United States as of September 2026. That count includes varied technologies and functions. It is not a count of 1,600 interchangeable systems that diagnose scans in the same way.

For listed devices, FDA says the applicable premarket requirements include a focused review of overall safety and effectiveness and an evaluation of whether the supporting studies fit the intended use and technological characteristics. The agency also cautions that its public list is not comprehensive, that its summaries are not all-inclusive and that the list is updated periodically. Authorization therefore does not mean every output is correct for every patient, nor does it replace the clinician’s review.

FDA’s oversight discussion also follows a device through development, validation, deployment, monitoring, maintenance and modification. Those lifecycle considerations matter because a system’s performance depends on how it is used and maintained, not only on the fact that it passed a premarket pathway.

Does an AI finding mean I have cancer?

No. In the FDA lesion-software category, the software is intended to help a clinical user detect, localize or characterize lesions suspicious for cancer; the classification assigns the primary diagnostic and management decisions to that user. “Suspicious” describes why a finding may need attention, not a confirmed cancer diagnosis.

A flagged area may require the radiologist to compare prior images, obtain additional views, recommend another scan, or correlate the finding with your history. The appropriate next step depends on the complete clinical assessment. Ask what the finding is, how confident the clinician is, what alternatives were considered and what follow-up is recommended.

Questions to ask your care team

These questions translate FDA’s emphasis on intended use, clinical users and transparency into a conversation about your own result:

  • What AI tool or software function was used, and what is it designed to do?
  • Did it detect or characterize a finding, improve the image, measure something or prioritize the scan for review?
  • Who reviewed the full scan, and who signed or confirmed the interpretation?
  • How does the AI result fit with my symptoms, prior imaging and medical history?
  • What uncertainty remains, and what follow-up do you recommend?
  • Was the software output included in the written report, and can you explain any score, mark or alert in plain language?

If the answers are unclear, ask whether the result came from the radiologist’s interpretation, a triage notification or a separate decision-support feature. Keeping those categories distinct prevents an alert from being mistaken for a diagnosis.

Why transparency matters

In a June 13, 2024 statement about machine-learning-enabled medical devices, Troy Tazbaz, director of the FDA’s Digital Health Center of Excellence, said: “Health products, including those that are medical devices, powered by artificial intelligence and machine learning (AI/ML) use software algorithms that can potentially learn from real-world use, and in some situations, may use this information to improve the product’s performance. But they also present unique considerations due to their complexity and the iterative and data-driven nature of their development.”

For patients, the practical implication is to ask what the system was intended to do, how its output reached the clinical user and how the clinician accounted for it. The word “AI” does not reveal those details.

What patients outside the United States should know

The FDA pathways, device count and public-list qualifications above are specific to the United States. Other countries have their own medical-device regulators and authorization processes. If your scan was performed elsewhere, ask which national authority assessed the device and what that authorization covers. Regardless of country, the useful questions remain the same: what task did the software perform, what evidence supports that task and who made the final clinical decision?

A practical way to judge the result you received

  1. Identify the function. Write down whether the software enhanced images, detected or characterized a feature, supplied diagnostic support or triaged the study.
  2. Identify the decision-maker. Confirm which clinician reviewed the complete scan and finalized the interpretation.
  3. Match the evidence. Ask whether performance information applies to your scan type, target finding, patient group and clinical setting.
  4. Put it in context. Discuss symptoms, prior imaging, history and any additional tests that affect the meaning of the finding.
  5. Clarify the plan. Leave the appointment knowing what remains uncertain and what follow-up is recommended.

That process gives AI a useful, bounded role: it can assist image interpretation or workflow, while a qualified clinical user remains accountable for deciding what the result means for you.
