Can You Trust an AI Chatbot to Diagnose Symptoms? What 2026 Studies Found

Don’t use a general-purpose AI chatbot to decide whether a possible medical emergency can wait. Chatbots can explain health information, but 2026 evaluations found that their care-seeking advice can be inconsistent—and that ChatGPT Health under-triaged more than half of the emergency scenarios in one structured test. These studies do not show how often real patients are harmed, but they do show why a fluent answer is not a safe substitute for a clinician or emergency services.

Diagnosis and triage are different—and both can go wrong

Diagnosis means identifying a condition. Triage means judging how urgently someone needs care and where to get it. For a person worried about a serious symptom, triage is often the more immediate question: can this wait, or do I need help now?

A chatbot may produce a plausible explanation without reliably assessing urgency. A wrong answer can cause needless alarm, but under-triage—advice that makes a serious situation sound safe to wait on—can delay time-critical care.

What the 2026 studies tested and found

ChatGPT versions gave imperfect care-seeking recommendations

A 2026 longitudinal study evaluated 22 ChatGPT model versions against 45 validated patient vignettes. Each vignette was prompted ten times, producing 9,900 assessments. The highest overall accuracy reported was 74%, for o1-mini, and the researchers found no overall improvement trend across newer releases. They concluded that accuracy was not sufficient for standalone care-seeking advice. This was a repeated vignette benchmark, not a study measuring emergency outcomes in real patients. ChatGPT Health under-triaged many test emergencies

In a 2026 structured evaluation summarized by the Icahn School of Medicine at Mount Sinai, investigators tested ChatGPT Health with 60 scenarios spanning 21 specialties, across 16 contextual conditions for 960 interactions. Physicians classified the correct urgency; the system under-triaged more than half of the scenarios physicians designated as emergencies. It often handled clear-cut cases but struggled with nuance. In one asthma scenario, it identified signs of impending respiratory failure yet still advised waiting. This is a result under the study’s test conditions, not an estimate of how often chatbot users experience under-triage in everyday life. Mount Sinai’s study summary

Other patient-facing answers also raised safety concerns

A physician-led 2026 study in npj Digital Medicine assessed 888 chatbot responses to 222 patient-posed questions in internal medicine, women’s health, and pediatrics. Depending on the chatbot, 21.6% to 43.2% of responses were rated problematic, and 5% to 13% were rated unsafe. Those percentages describe the study’s particular question set and evaluation framework; they are not universal error rates for every chatbot or conversation. The study in npj Digital Medicine

Why these findings matter—and what they do not prove

The emergency evaluation is concerning because missing urgency can lead someone to postpone care. But it tested structured scenarios at one point in time; it did not establish that chatbot use caused a particular number of emergency visits, injuries, or deaths. The broader advice study likewise cannot predict the safety of every answer a chatbot might give.

AI systems are frequently updated, so findings about a tested release should not be treated as a permanent score for every later version. Nor do these studies establish that every dedicated symptom checker is safer than a general chatbot. They support a narrower, useful conclusion: current evaluations show enough inconsistency and under-triage to make a general chatbot unsuitable as the sole judge of urgency.

What to do if symptoms may be urgent

Do not ask a chatbot whether a possible emergency can wait. If symptoms are worsening or concerning, seek medical care directly. For a suspected emergency, contact your local emergency services or go to an emergency department rather than waiting for a chatbot response.

Mount Sinai’s U.S.-based guidance highlights chest pain, shortness of breath, severe allergic reactions, and changes in mental status as reasons to seek care directly. For thoughts of self-harm, it advises contacting the U.S. 988 Suicide and Crisis Lifeline or going to an emergency department. Where a chatbot can fit into non-urgent care

For a non-urgent question, a chatbot may help explain general health information or help you prepare questions for a clinician. Treat its response as a starting point for discussion—not as a diagnosis, a care plan, or confirmation that symptoms are harmless. The American Medical Association says chatbots are not a replacement for a doctor and should not be used for medical emergencies. AMA guidance on using AI chatbots for health questions
