Not by default. An AI chatbot can produce a useful explanation, outline, or list of leads, but fluent wording and apparent confidence are not evidence that an answer is true. Treat every important response as an unverified draft: separate it into claims, open and inspect the cited sources, check dates and jurisdiction, and obtain qualified human review before making a consequential medical, legal, financial, safety, or employment decision.
OpenAI’s Help Center puts the central warning plainly: “Confidence isn’t reliability: The model may express high confidence even in incorrect answers.” Its guidance also warns that a response can contain wrong dates or definitions, fabricated quotes and studies, nonexistent citations, and overconfident answers to ambiguous questions. Read the full guidance.

What does it mean to trust an AI answer?
Trust is not a single yes-or-no property. You are deciding whether a particular claim is reliable enough for a particular use. A chatbot may be perfectly adequate for brainstorming names or explaining a familiar concept, yet unsuitable for deciding which medicine to take or how to respond to a legal notice.

Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Assess five things before acting:
- Stakes: What could happen if the answer is wrong?
- Recency: Could the fact have changed after the model’s training data or the source’s publication date?
- Traceability: Can you open a source and match it to the exact claim?
- Specificity: Is this a broad explanation, or an obscure number, quotation, instruction, or recommendation?
- Independent confirmation: Does a relevant authoritative source or qualified professional support it, rather than the chatbot merely repeating itself?
These are practical decision axes, not a validated scoring system. A response that performs well on all five still deserves normal judgment; one that fails several should not be used as the basis for action.

Why can a wrong answer sound so convincing?
Fluent text is not a fact-check
Language models generate text that fits the prompt and the patterns learned from data. That process can produce a coherent sentence when the underlying information is missing, mixed together, or ambiguous. OpenAI describes hallucinations as “plausible but false statements generated by language models.” A polished paragraph therefore tells you about presentation, not truth.
Guessing can be rewarded
OpenAI’s September 5, 2025 explainer says accuracy-only evaluations can encourage a model to guess instead of admitting that a question cannot be determined. It distinguishes three outcomes: a correct answer, an error, and an abstention. The explainer identifies unavailable information, limited reasoning, and ambiguous prompts as reasons a question may not have a determinable answer.
One reported SimpleQA example illustrates why a single accuracy number cannot stand in for “AI reliability.” OpenAI reported these results for specific models in that evaluation:
| Model and evaluation | Abstention rate | Accuracy rate | Error rate |
|---|---|---|---|
| gpt-5-thinking-mini, SimpleQA example | 52% | 22% | 26% |
| OpenAI o4-mini, SimpleQA example | 1% | 24% | 75% |
These figures come from that particular benchmark and models, as reported by OpenAI in 2025. They are not general accuracy rates for ChatGPT, every chatbot, or ordinary everyday tasks. They do show why willingness to abstain can matter: a system that says it cannot establish an answer may be safer than one that confidently fills the gap.

How do I know if a chatbot is telling the truth?
Use this workflow whenever an answer could influence a decision, publication, purchase, or public statement.
- Break the response into checkable claims. Mark every factual assertion, number, date, quotation, study, technical detail, instruction, and recommendation. Ignore connective prose until the underlying claims are checked.
- Ask what would change your action. If only one sentence matters—for example, a dosage, deadline, eligibility rule, or compatibility requirement—verify that sentence first. This prevents a long, confident answer from distracting you from the decisive detail.
- Open each citation yourself. Confirm that the link works, that it leads to the source named by the chatbot, and that the source actually supports the wording. A list of references is not self-validating. OpenAI specifically advises visiting links directly when accuracy matters.
- Compare the original wording. For a quote, statistic, or technical claim, read the original publisher or document. Check the exact words, units, definitions, date, sample or scope, and any conditions attached to the figure. A source can be genuine while the chatbot’s summary is still wrong.
- Check time and place. Look for the publication or update date and confirm that the rule applies to your country, state, institution, contract, product version, or other jurisdiction. Current prices, laws, health guidance, software behavior, and events need current authoritative sources.
- Find independent confirmation when the consequences justify it. Prefer a primary regulator, court, government agency, manufacturer document, peer-reviewed paper, or professional body. Two websites that copy the same press release are not independent corroboration.
- Resolve ambiguity before verification. Ask the chatbot to state assumptions, then verify those assumptions. If the answer depends on an unspecified age, operating system, contract term, diagnosis, or location, supply the missing context or stop.
- Record what you checked. For work or decisions, save the source URL, title, date accessed, relevant passage, and your assumptions. This makes later review possible when a page changes.
What should I verify first?
Prioritize claims by the combination of potential harm and likelihood of change.
| Answer type | Main risk | Minimum verification | Before acting |
|---|---|---|---|
| Brainstorming, naming, rewriting | Low direct harm; unsuitable ideas | Read for tone, bias, and omissions | Edit and judge yourself |
| Stable background explanation | Simplification or an occasional factual error | Check key definitions and one authoritative reference | Use as a draft, not a sole source |
| Current event, price, product feature, or software procedure | Outdated information | Open a current source and confirm its date and version | Follow the current publisher or vendor documentation |
| Statistic, quotation, study, or technical specification | Fabrication, wrong context, units, or attribution | Read the original document and match the exact passage or figure | Do not publish or implement until it matches |
| Medical, legal, financial, safety, or employment decision | Material personal, legal, or monetary harm | Current authoritative guidance plus qualified review | Do not rely on the chatbot alone |
Can AI make up citations, quotes, or studies?
Yes. Official guidance lists fabricated quotes, studies, and citations among possible errors. A citation-shaped answer can therefore fail in three different ways: the link may not exist, it may lead to a different document, or the real document may not support the chatbot’s claim.

- Copy the title and author into the source site or a library catalogue rather than trusting the chatbot’s hyperlink text.
- Check that the publication, date, volume, page, section, or case number exists where relevant.
- Search the original document for the quoted phrase and compare surrounding context.
- For a statistic, confirm the population, time period, denominator, measurement method, and whether the number is an estimate or a one-time result.
- For a technical instruction, follow the documented version and prerequisites; screenshots or menu names may differ by plan and enabled tools.
If you cannot locate the source or the source does not contain the claimed evidence, treat the claim as unverified—not as a minor formatting error.

How important is recency?
Training data may not include events or changes that occurred later. Even a correct historical answer can become wrong when a regulation, price, product, public-health recommendation, court ruling, or software interface changes. For current information, use search or a research tool when available, then inspect the dated source yourself. Tool availability and interfaces vary by plan and enabled tools, so do not assume every user sees the same search controls.
When checking a changing claim, write down three facts together: the source’s publication or update date, the date the rule or information takes effect, and the location or edition to which it applies. A page updated yesterday may still describe a policy that starts next month; a current page may describe a different jurisdiction.
Should you trust AI for medical, legal, or financial advice?
Use a chatbot for questions to ask a professional, plain-language explanations of documents, or a checklist of issues to investigate. Do not use it as the final authority for a diagnosis, treatment or dosage, legal strategy or deadline, investment or tax decision, insurance coverage determination, workplace action, or safety-critical procedure.
OpenAI Academy’s responsible-use guidance, dated April 10, 2026, says: “For important decisions, such as in health, legal, or financial matters, always check with a qualified expert or defer to your organization’s policies before acting.” Its broader recommendations include human review for important work, attention to bias and perspective, and use of search or deep research when recency matters. Medical example
If a chatbot suggests that a symptom is harmless, verify the advice against a current health service or medical organization and contact a licensed clinician. Emergency symptoms, medication interactions, pregnancy, allergies, and changes in a chronic condition raise the bar further; do not wait for a chatbot to resolve uncertainty.
Legal example
If it gives a filing deadline or says a clause is enforceable, check the current law, the relevant jurisdiction, the complete document, and a qualified lawyer or approved legal service. A deadline calculated for one court or country may be wrong for another, and missing a procedural detail can have consequences even when the general explanation sounds reasonable.
Financial example
If it recommends an investment, tax treatment, loan action, or benefits choice, verify current rules and figures with the responsible regulator, provider, or qualified adviser. Ask which assumptions drive the answer and what would invalidate them; do not treat a confident forecast as a guarantee.

Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallHow should you respond when the chatbot is uncertain?
Uncertainty is information. A clear “I don’t know,” a request for missing context, or several conditional answers can be preferable to an unsupported single conclusion. Ask follow-up questions such as:
Best Value
- “Which parts of that answer are uncertain or depend on assumptions?”
- “What is the source and date for each number?”
- “What evidence would disprove this conclusion?”
- “Does this apply to my country, state, product version, or plan?”
- “Give me the exact passage in the original source, not a paraphrase.”
Do not confuse a disclaimer with verification. A chatbot can say “check with a professional” and still contain an incorrect diagnosis, citation, or calculation. Check the substantive claims using the workflow above.
Common mistakes that make verification fail
- Checking only the link: A real URL does not prove that the page supports the claim.
- Accepting a plausible quotation: Search the exact wording and read the surrounding passage.
- Using multiple summaries as confirmation: They may all derive from one error or press release.
- Ignoring units and definitions: “Annual,” “per user,” “before tax,” and “in laboratory conditions” can change the meaning of a number.
- Checking an old page: Record dates and version numbers for information that changes.
- Letting urgency lower the standard: If a decision is urgent, contact the appropriate professional or authority rather than treating speed as evidence.
- Publishing unverified text: Human review means checking the claims, not merely proofreading grammar.
A practical stop-or-proceed rule
Proceed with a chatbot’s output when the task is low-stakes, the needed facts are stable, and you have checked the important claims. Pause and verify when the answer contains a surprising detail, an obscure citation, a precise number without context, a recommendation that affects another person, or information that may have changed. Stop and escalate to a qualified expert or your organization’s policy when the decision could materially affect health, legal rights, money, safety, or employment.
OpenAI Academy summarizes the everyday habit well: “Always double-check critical facts with trusted sources, and feel free to mark errors with the thumbs-down button.” Its responsible-use guidance also recommends careful review for bias and perspective. The reliable way to use a chatbot is therefore not to demand certainty from it, but to use it to generate possibilities and then verify the claims that matter.
