Weight regain is common after stopping GLP-1-based medicines used for weight management, but the amount varies. In a tirzepatide withdrawal trial, participants switched to placebo gained an average 14.0% of their body weight over 52 weeks. That figure is a change in body weight—not 14% of the weight they had lost—and it cannot predict one person’s result.
How much weight do people regain after stopping?
There is no single amount that applies to everyone. The American Diabetes Association (ADA) says sudden discontinuation of semaglutide or tirzepatide results in regaining about one-half to two-thirds of the weight lost within a year. A 2026 systematic review and meta-regression estimated that 60% of treatment-related weight loss was regained at one year. That estimate pooled data from six randomized trials; it is an average, not an individual forecast.

The figures describe different evidence syntheses, but both point to substantial average regain during the first year. Neither means every person regains the same amount—or all the weight lost.

The SURMOUNT-4 randomized withdrawal trial helps explain why the percentage can be confusing. In the trial, 783 adults with overweight or obesity began a 36-week tirzepatide period. Of those, 670 who completed that period were randomized to continue treatment or switch to placebo for another 52 weeks. Participants did not have diabetes, had tolerated 10 or 15 mg, and received lifestyle counseling.

Participants had lost an average 20.9% during the initial 36 weeks. The +14.0% result is the placebo-switch group’s average change in body weight from the point of randomization—not 14% of the weight they had lost. The trial describes a selected group who completed the lead-in and tolerated treatment; its averages do not tell an individual how much or how quickly they may regain weight. Why the estimates do not predict one person’s outcome
People differ, and study results depend on who took part, which medicine they used, how long they took it, and how regain was measured. SURMOUNT-4 studied tirzepatide in adults without diabetes who completed a treatment lead-in. The 2026 review combined six randomized trials for its one-year trajectory estimate; estimates beyond 52 weeks were modeled rather than directly observed in long-term randomized follow-up. Most studies included in that review were judged to have moderate risk of bias.

So these findings support a general expectation of regain for many people after stopping, not a precise timeline or personal prediction. The longer-term modeled estimates should not be treated as measured outcomes. See the 2026 systematic review and meta-regression.

The ADA recommends continuing obesity medicines after treatment goals are reached to maintain health benefits, because stopping often leads to weight recurrence and the return or worsening of cardiometabolic risks and obesity-related conditions. Its 2025 guidance also calls for shared decision-making about long-term treatment, including continuing at the lowest effective dose or stopping with close weight monitoring.

A decision to continue, pause, or stop should be made with the prescribing clinician. Relevant considerations include:
- Expected weight-maintenance benefit and the person’s treatment goals and preferences.
- Tolerability and other health considerations.
- Access and cost.
- Whether close follow-up and other support are available if treatment stops.
The trial evidence cited here does not establish a universal tapering schedule or show that tapering prevents regain. Do not change a prescribed medicine based on a group average; discuss the options with the clinician managing treatment. Read the ADA’s 2025 Standards of Care guidance.

Planning support and monitoring after stopping
If stopping is necessary or chosen with a clinician, plan follow-up rather than treating the last dose as the end of care. The ADA recommends close weight monitoring when discontinuing. An international expert consensus also recommends monitoring for regain, active dietary support, long-term weight-maintenance care, and referral to a registered dietitian as part of multidisciplinary support.
Monitoring can help make changes in weight visible to the person and clinician, but a scale is only a tracking tool. The consensus recommendations support ongoing care; they do not prove that a particular diet, exercise routine, product, or post-treatment program reliably prevents regain.
