Can You Exercise With Knee Pain? Common Questions Answered

Yes, many people can remain active with knee pain—but the right answer depends on the cause, the activity and how your symptoms respond. For diagnosed knee osteoarthritis, major orthopedic and public-health guidance supports strengthening and low-impact aerobic exercise. A painful knee after a fall or twist, a hot swollen joint, or pain that is severe or worsening needs assessment rather than a generic “push through it” rule.

Use the guide below to decide when to modify exercise, when to stop a particular movement and when to seek medical care. It is general information, not a diagnosis or an individualized rehabilitation plan.

What knee pain means for exercise

Knee pain is a symptom, not a diagnosis. It can follow an injury, overuse, osteoarthritis or another condition, and the safest activity can differ greatly between those causes. A person with established osteoarthritis may benefit from a carefully adjusted program, while someone who cannot bear weight after a twist may need an examination before exercising.

The American Academy of Orthopaedic Surgeons (AAOS) recommends that people with symptomatic knee osteoarthritis take part in self-management, strengthening, low-impact aerobic exercise, neuromuscular education and physical activity consistent with national guidelines. Its third-edition guidance also says supervised, unsupervised and aquatic exercise are preferable to no exercise for improving osteoarthritis pain and function. These recommendations are for knee osteoarthritis; they do not clear every painful knee for exercise.

A useful distinction is between modifying an activity and continuing a specific activity that is causing substantial pain. You may be able to keep moving by reducing duration, slowing the pace, lowering resistance, changing the surface or choosing a different exercise. You should not ignore pain that is severe, progressively worse or associated with warning signs.

First check whether you need medical advice

Do not rely on an exercise article if the pain began with a significant fall, twist or other injury, or if the knee is rapidly deteriorating. NHS advice identifies several situations needing urgent assessment.

What you notice What to do
Very severe pain, inability to move the knee or inability to put weight on the leg Seek urgent medical advice.
Marked swelling, a changed shape or an obvious deformity Seek urgent medical advice.
A hot, red knee together with fever or feeling generally unwell Seek urgent medical advice.
Pain that is worsening or has not improved over a few weeks Arrange a routine assessment with a doctor or physiotherapist.

These thresholds come from NHS public guidance on knee pain and joint pain. Local care routes differ, but the warning signs are reasons not to test the knee with a workout. If you are unsure whether a recent injury is significant, have it assessed before returning to exercise.

When exercise is part of knee-osteoarthritis care

For diagnosed knee osteoarthritis, exercise is not merely allowed; it is a core part of recommended non-surgical care. AAOS describes strengthening, low-impact aerobic activity and neuromuscular education as strongly recommended elements, alongside self-management. The goal is to improve function and tolerance, not to prove that pain can be endured.

The U.S. Physical Activity Guidelines for Americans, second edition (published in 2018), state that most people with osteoarthritis can usually tolerate 150 minutes or more of moderate-intensity activity per week. The same guidance emphasizes that the type and amount must match an individual’s ability and the severity of the condition. Treat 150 minutes as broad public-health context, not a starting prescription for a painful knee. Someone currently inactive may need much shorter bouts and gradual progression.

AAOS’s patient conditioning material is intended to be used under a doctor’s supervision. It advises asking a doctor or physical therapist which exercises fit your goals and says to talk with them if exercise causes pain. The program suggests a 5–10 minute warm-up using low-impact activity such as walking or a stationary bicycle. A stationary bike is optional; you do not need to buy equipment to follow the general advice.

Which exercises are low impact for bad knees?

Federal physical-activity guidance names walking, swimming, tai chi and many strengthening exercises as low-impact, low-risk examples for people with osteoarthritis. “Low impact” describes the stress pattern of the activity, not a guarantee that it will be painless for every person. Begin at a level you can tolerate and observe how the knee responds during the session and afterward.

Activity Why it may be useful How to make it more tolerable
Walking Named as a low-impact aerobic option for osteoarthritis. Shorten the route, slow the pace or split activity into brief bouts.
Swimming or aquatic exercise Water-based exercise is identified as a low-impact option; AAOS also recommends aquatic exercise over no exercise for osteoarthritis. Choose a duration and stroke that do not increase symptoms; stop if pain becomes substantial.
Tai chi Listed among low-impact activities for osteoarthritis. Use a controlled range of motion and reduce the session length if symptoms rise.
Strengthening exercises AAOS strongly recommends strengthening for symptomatic knee osteoarthritis. Use a resistance and range of motion you can control; obtain individualized instruction when needed.
Stationary bicycle AAOS gives it as one example of a low-impact warm-up. Keep resistance low initially and treat it as an option, not a requirement.
Running NHS advises against running while the knee hurts. Pause running and use assessment and a graded return rather than running through symptoms.

The activity examples and adaptation principle come from the Physical Activity Guidelines for Americans. They are options to discuss with a clinician, not a ranking of universally safe exercises.

How to decide whether to continue, modify or stop

Use symptom response as feedback. Before each session, consider whether the knee is already swollen, whether you can walk and bear weight normally, and whether the planned movement resembles the activity that triggered the pain. During exercise, distinguish mild, stable discomfort from pain that is substantial or escalating.

Continue with a modification when

  • You have no urgent warning signs and the activity causes only tolerable symptoms.
  • You can reduce speed, resistance, depth, impact or duration and the knee settles.
  • Your movement remains controlled rather than limping or compensating.

Stop that activity when

  • Pain becomes moderate to severe, keeps increasing or changes your movement.
  • Swelling, heat or instability appears during or after the session.
  • The knee is noticeably worse later that day or the next day and does not settle with ordinary rest.

AAOS’s conditioning guidance uses the instruction “Do not ignore pain” and tells readers to discuss pain during exercise with a doctor or physical therapist. AAOS also notes that temporary increases in knee pain or muscle soreness can occur in osteoarthritis exercise programs. The Arthritis Foundation’s consumer guidance advises stopping for moderate to severe joint pain. Taken together, these points support adjusting a tolerable program—not treating worsening pain as a test of toughness.

Can you walk, swim or strengthen with knee pain?

Walking

Walking is a practical low-impact option when you can bear weight and your symptoms are stable. Start with a short, easy route. If a continuous walk provokes pain, divide the same general activity into shorter bouts separated by rest. A worsening limp, increasing swelling or inability to bear weight is a reason to stop and seek advice, not to shorten the route indefinitely.

Swimming and aquatic exercise

Water can make movement feel easier by reducing weight-bearing load, and aquatic exercise is included in AAOS’s recommendation for knee osteoarthritis. It still needs to be matched to your ability: a forceful kick, push-off or awkward pool entry can aggravate an injured knee. Use a stroke and duration that remain comfortable and follow facility safety procedures.

Strengthening

Strengthening is specifically recommended for symptomatic knee osteoarthritis because improving the muscles that support movement can help function. The exact exercises, loads and range of motion should be selected with a doctor or physical therapist when diagnosis, weakness, swelling or prior injury is uncertain. Use controlled repetitions and stop an exercise that produces substantial joint pain rather than simply adding rest between painful sets.

Should you stop exercising if your knee hurts?

You do not automatically have to stop all physical activity because one exercise hurts. First remove or modify the provoking movement and choose a tolerable alternative, such as a shorter walk, aquatic exercise or a different strengthening pattern. However, continuing the same painful activity is not appropriate when pain is moderate to severe, worsening, associated with swelling or deformity, or causing you to limp.

Keep a simple record for several sessions: the activity, duration, pain during exercise, swelling and how the knee feels later that day and the next morning. This does not diagnose the problem, but it can reveal whether a smaller dose is tolerated or whether symptoms are progressively worsening. Share the pattern with a clinician if it persists.

Can you run with knee pain?

Running deserves a stricter answer than general low-impact exercise. NHS guidance says not to run with knee pain. If the pain remains after a week of rest, it advises consulting a GP or physiotherapist. That is not a universal promise that one week makes running safe; the appropriate return depends on the cause and severity of the pain.

While running is paused, you may be able to maintain activity with a lower-impact option that does not reproduce symptoms, provided you have no red flags. Do not use a substitute workout to conceal a serious injury. Before resuming, obtain advice if pain followed an injury, keeps returning, causes swelling or changes your gait. A clinician can help determine whether a graded return is appropriate.

Read the NHS guidance on knee pain and other running injuries for its running-specific advice.

A practical, safer way to restart activity

  1. Clarify the situation. Note when the pain began, whether there was a twist or fall, where it hurts and whether you can bear weight. Seek urgent advice first if you have severe pain, inability to move or bear weight, major swelling or deformity, or a hot red knee with fever or feeling unwell.
  2. Choose a lower-impact test. For stable, diagnosed osteoarthritis, consider a brief walk, gentle aquatic session, tai chi or controlled strengthening movement. Do not treat these examples as clearance after an acute injury.
  3. Warm up briefly. AAOS suggests 5–10 minutes of low-impact activity such as walking or a stationary bicycle. Keep the pace easy enough that symptoms do not escalate.
  4. Use a small dose. Start below the amount that previously triggered pain. You can shorten the session, take breaks or lower resistance. The 150-minute weekly guideline is a longer-term public-health reference, not a requirement for the first session.
  5. Monitor during and after. Stop if pain becomes moderate to severe, increases steadily, causes limping or is accompanied by swelling or instability. Check the knee later that day and the next morning.
  6. Progress only when the response is stable. Increase one variable—time, frequency or resistance—gradually. If symptoms worsen, return to the last tolerable level and arrange clinical advice if they do not settle.

When a professional assessment is worthwhile

A doctor or physiotherapist can assess the cause, check strength and movement, and tailor an exercise plan. Arrange an appointment for persistent pain that has not improved within a few weeks, repeated swelling, recurrent giving-way, pain that limits ordinary walking, or uncertainty about how to exercise after an injury. AAOS specifically advises discussing painful exercise with a doctor or physical therapist and says its conditioning program is not individualized medical advice.

The direct answer

Many people with knee pain can exercise by changing the activity, duration or intensity. For diagnosed knee osteoarthritis, strengthening and low-impact aerobic exercise are recommended, with walking, swimming, tai chi and aquatic activity among the supported options. Do not run while the knee hurts under NHS advice, and do not exercise through severe, worsening or unexplained pain. Urgent symptoms, a significant injury or persistent problems call for medical assessment before you increase activity.
