Some supplements have also increased cancer risk in particular trial populations, and supplements may interact with cancer treatment. A supplement recommended by a clinician for a documented deficiency is a separate medical use—not cancer prevention or treatment.
What the evidence says about vitamins and cancer
The National Cancer Institute (NCI) says there is not enough evidence to support multivitamins, minerals, or individual vitamins and minerals as ways to prevent cancer. An association observed between a nutrient and cancer in a population does not prove that taking a supplement will prevent cancer. NCI’s Cancer Prevention Overview puts it plainly: “There is not enough proof that taking multivitamin and mineral supplements or single vitamins or minerals can prevent cancer.”

Prevention, treatment, mortality, and recurrence are different outcomes. A study finding fewer cancer deaths does not necessarily mean fewer people developed cancer; neither result by itself proves that a supplement treats a diagnosed cancer or prevents it from returning.

Antioxidants include nutrients such as beta-carotene and vitamin E. Trial results show why it is unsafe to assume that an antioxidant supplement is beneficial simply because the nutrient occurs in food or has antioxidant properties.

Beta-carotene trials in people at elevated risk
In the ATBC trial, 29,133 Finnish male chronic smokers aged 50–69 received beta-carotene (20 mg daily), alpha-tocopherol (50 mg daily), both, or placebo for 5–8 years. The beta-carotene groups had higher lung cancer incidence (relative risk 1.18; 95% confidence interval 1.03–1.36) and total mortality (relative risk 1.08; 95% confidence interval 1.01–1.16), according to the NCI’s Lung Cancer Prevention PDQ.
In CARET, a combination of beta-carotene (15 mg daily) and retinol (25,000 IU daily) increased lung cancer risk and all-cause mortality among people at high risk because of smoking history or asbestos exposure. The trial ended early. These findings concern supplements tested in specific high-risk groups; they do not show that beta-carotene-containing foods cause cancer. NCI summarizes these findings in its Antioxidants and Cancer Prevention material.

Vitamin E and prostate cancer risk
In extended follow-up of SELECT, taking 400 IU of vitamin E daily was associated with a statistically significant excess risk of prostate cancer compared with placebo (hazard ratio 1.17; 99% confidence interval 1.0004–1.36; P=.008), as summarized in NCI’s Cancer Prevention Overview. This result is about the dose and study population in that trial, not a universal estimate for every person or every vitamin E product.
Does vitamin D prevent cancer or lower the risk of dying from cancer?
Most randomized trials summarized by NCI did not find that vitamin D, with or without calcium, lowered cancer incidence. In VITAL, which included more than 25,000 participants, vitamin D at 2,000 IU daily did not reduce cancer deaths overall. Analyses that excluded deaths in the first few years reported a reduction, but that secondary finding is not the same as the overall trial result. See NCI’s Vitamin D and Cancer fact sheet for its discussion of the evidence.

Should someone diagnosed with cancer take antioxidant supplements?
There is no reliable basis for using vitamins or supplements as cancer treatment. NCI states in Diets, Supplements, and Cancer: “But there is no proof that any nutrition trend, food, vitamin, mineral, dietary supplement, herb, or combination of these can slow cancer, cure it, or keep it from coming back.”
Evidence about antioxidant supplements during chemotherapy or radiation is mixed; some studies have found worse outcomes. That does not establish that every supplement interferes with every treatment. Whether a particular product could affect a particular therapy depends on the product and regimen. NCI discusses these issues in its patient guidance on Cancer Therapy Interactions With Foods and Dietary Supplements and its professional PDQ summary.

Tell the oncology team about every supplement you take or are considering, including its name, dose, and timing. Do not start, stop, or change cancer treatment based on a supplement claim; ask the treating team about the specific product and therapy.

A clinician may recommend a supplement to correct a documented deficiency or address another medical need. That is different from taking it to prevent cancer, treat a tumor, or prevent recurrence. Follow the clinician’s advice for the indication it addresses, and ensure the oncology team knows about it if you are receiving cancer care.

How to judge a vitamin-and-cancer claim
- Check the outcome: Does the evidence concern cancer incidence, cancer deaths, treatment response, or recurrence? They are not interchangeable.
- Check who was studied: A result in Finnish male smokers or people with smoking or asbestos exposure does not automatically apply to other populations.
- Check dose and formulation: Trial doses and combinations are evidence about those tested conditions, not consumer dosing recommendations.
- Distinguish trials from associations: Observational links do not establish that a supplement caused a lower or higher cancer risk.
- Ask the oncology team about treatment: Interaction questions are specific to the supplement and regimen; do not assume either that all supplements are harmful or that they are harmless.
