Some observational studies link blood vitamin D levels with pancreatic cancer outcomes, but those associations do not show that taking supplements changes cancer risk, treatment response, or survival.
Does vitamin D help pancreatic cancer?
It helps to distinguish three things that studies measure: vitamin D taken as a supplement, vitamin D consumed in food, and the blood marker 25-hydroxyvitamin D [25(OH)D]. They are not interchangeable. A link between blood levels and cancer outcomes does not establish that supplements cause those outcomes.

The National Cancer Institute (NCI) says there is no proof that vitamins or supplements can slow or cure cancer. Vitamin D and the analogue paricalcitol have been studied alone or alongside other treatments in pancreatic cancer, but the existence of research does not make either an established cancer treatment. Can vitamin D pills prevent pancreatic cancer?
Prevention has not been established. NCI says single vitamins and minerals, including vitamin D, have not been shown to reduce cancer risk. That does not mean supplements have been proved to cause pancreatic cancer; it means the evidence does not support taking them to prevent it. NCI: Cancer Prevention Overview.

What the incidence studies found
A 2013 meta-analysis of nine studies with 1,206,011 participants found no statistically significant association between the highest and lowest vitamin D categories and pancreatic cancer risk: pooled odds ratio (OR) 1.14 (95% confidence interval [CI] 0.896–1.451; P=0.288). The authors reported substantial variation across studies (I²=70.9%). Their analysis covered dietary vitamin D or circulating 25(OH)D, not a randomized test of supplements. 2013 meta-analysis.
A pooled analysis of nine case-control studies by the Pancreatic Cancer Case-Control Consortium reported higher risk with higher dietary vitamin D intake: OR 1.13 per 100 IU/day (95% CI 1.07–1.19), and OR 1.31 for intake of at least 230 versus under 110 IU/day (95% CI 1.10–1.55). The authors said more research was needed to determine whether the association was causal. This observational dietary finding neither proves supplements cause cancer nor shows they prevent it. A 2024 meta-analysis of 16 observational studies (10 on incidence and six on mortality), covering 529,917 participants, found no statistically clear association between circulating 25(OH)D and incidence in case-control studies: pooled OR 0.98 (95% CI 0.69–1.27). 2024 systematic review and meta-analysis.
Do higher vitamin D levels improve survival?
The same 2024 review reported that higher circulating 25(OH)D was associated with lower pancreatic cancer mortality: pooled hazard ratio (HR) 0.64 (95% CI 0.45–0.82) in cohort studies and HR 0.78 (95% CI 0.62–0.95) in case-control studies. These are observational associations, not results showing that supplements improve survival. People with different blood levels may differ in other ways, and the studies do not establish that supplementation caused the outcomes. Is vitamin D safe to take during pancreatic cancer care?
Tell your oncologist, nurse, or dietitian about supplements you take or are considering. Supplements can affect how cancer treatment works, and evidence about interactions is limited for many products. The general interaction guidance does not establish a specific interaction between vitamin D and any particular pancreatic cancer drug. Avoid self-prescribing high doses
NCI notes that excessive vitamin D can cause hypercalcemia and calcification in soft tissues. It lists 100 micrograms (4,000 IU) per day as the adult upper intake level; this is a safety limit, not a cancer-treatment dose or a target to take. Do not increase sun exposure to raise vitamin D, because ultraviolet exposure raises skin-cancer risk.
