Can the iKnife Tell Whether Tissue Is Cancerous?

In research studies, the iKnife has rapidly classified tissue samples by analyzing the chemical profile of aerosol created during electrosurgery. That is not the same as a proven, routinely available cancer diagnosis: the evidence described here does not show that using its readings improves patient outcomes or replaces standard pathology.

How does the iKnife analyze tissue?

An electrosurgical knife uses electrical current to heat and cut tissue, producing aerosol—often called surgical smoke. In the iKnife setup, that aerosol is directed to a mass spectrometer. The instrument measures chemical signals, and software compares the resulting profile with reference patterns associated with different tissue types. The clinical literature calls this method rapid evaporative ionisation mass spectrometry (REIMS).

So “smelling” cancer is a metaphor: the system analyzes molecules in tissue-derived aerosol. It does not literally smell tissue or recognize cancer visually. Imperial College London’s account of the original system describes the knife-to-mass-spectrometer connection and a reference library used to classify readings.

What have studies found?

Early operating-theatre feasibility work

In 2013, Imperial reported using tissue from 302 surgery patients to build an initial reference library. In 91 operating-theatre tests, the iKnife’s tissue classifications matched postoperative diagnoses. Surgeons could not see the iKnife readings during those tests, so the study demonstrated classification feasibility—not whether acting on a reading would change treatment or outcomes. The initial library included samples from brain, lung, breast, stomach, colon and liver tumors; that breadth should not be mistaken for equal validation or an approved use in every cancer type. • 【User-Friendly Design for Veterinary Professionals】Designed with ease of use in mind, this diagnostic equipment features both bottom and side activation options, with a secure safety lock to prevent accidental firing. This setup makes handling more convenient, ensuring seamless operation in any veterinary environment.
  • 【Precision Positioning with Enhanced Imaging】The needle includes clear scale markings for precise depth control and an enhanced imaging marker for clear positioning, allowing veterinarians to perform highly accurate tissue collection procedures. This is essential for detailed diagnostic assessments.
  • 【Reliable and Durable for Repeated Use】Built for stability, this surgical device is durable and capable of multiple punctures with minimal vibration, providing a high sampling success rate. Paired with a coaxial needle, it allows for single puncture procedures, reducing complications and enhancing patient comfort for dogs, cats, and other animals.

Endometrial cancer pilot

A prospective, single-centre pilot studied Pipelle biopsy samples collected from patients undergoing clinical evaluation. The publication record reports 150 patients and 453 spectra. A 2022 NIHR Imperial Biomedical Research Centre summary reports 89% diagnostic accuracy and analysis in 1.8 seconds. Those figures describe this pilot and its model; they are not a general accuracy rate for the iKnife across cancers or care settings. The summary notes that larger datasets and more uniform sample conditions are needed. NIHR Imperial BRC, 2022 · Imperial College London publication record

Breast surgery trial

Cancer Research UK lists REI-EXCISE, a trial of iKnife assistance during breast cancer surgery. Recruitment ran from 2017 to 2019, and the trial page says results were reported in 2024. The page does not provide detailed outcome metrics, so it cannot establish a particular level of performance or patient benefit. Cancer Research UK: REI-EXCISE

Does a fast classification mean a diagnosis patients can rely on?

No—not on the evidence summarized here. A result produced in seconds is a measure of analysis speed under a study protocol, not proof that the system can replace histopathology, is approved and routinely available, or improves survival, recurrence rates, or treatment decisions. The early theatre tests kept readings from surgeons; the endometrial pilot reported accuracy for its specific sample set. Neither finding by itself establishes clinical benefit from using iKnife results to guide care.

Imperial’s 6 January 2023 report quoted Professor Sadaf Ghaem-Maghami describing the system’s potential for evaluating women referred with abnormal vaginal bleeding. That statement expresses a possible future role, not an achieved change in routine care. The sources reviewed here do not establish a regulator or standards-body endorsement. What the iKnife is—and is not

  • It is: an electrosurgical knife connected to mass spectrometry and classification software, tested as a way to classify tissue from its chemical profile.
  • It is not: a literal smell test, an imaging-based “virtual biopsy,” or, based on these reports, an established replacement for standard tissue diagnosis.

A separate 2024 Imperial report about an AI “virtual biopsy” concerns classifying lung tumors from medical scans; it describes a different technology, not the REIMS iKnife. Imperial College Healthcare NHS Trust, 2024
