Hydrocortisone, a medication form of cortisol, is being studied in two different ways: soon after trauma to try to prevent later post-traumatic stress disorder (PTSD), and alongside therapy for people who already have PTSD. Neither use is established routine treatment. Early-prevention results are mixed and preliminary; for established PTSD, the 2023 VA/DoD guideline recommends against hydrocortisone as a stand-alone treatment and says evidence is insufficient to recommend for or against adding it to psychotherapy.
What “stress hormone treatment” means
Cortisol is a hormone produced by the body. Hydrocortisone is the pharmaceutical medication used in the clinical studies discussed here. The evidence does not establish that over-the-counter cortisol products or home cortisol measurements treat PTSD or help choose a treatment. The distinction matters: results from supervised clinical studies of hydrocortisone cannot be assumed to apply to supplements or consumer hormone tests.

Can hydrocortisone prevent PTSD after trauma?
Some studies have reported benefits when hydrocortisone was given soon after trauma, but findings are not consistent, and the participants and treatment approaches differ. The studies do not establish a preventive treatment for trauma survivors generally.

Pooled evidence is promising but preliminary
A 2021 systematic review and meta-analysis combined eight studies, nine effect sizes, and 362 participants. Across the included trials, hydrocortisone was associated with fewer PTSD symptoms (standardized effect d = 0.96; 95% CI 0.22–1.69; p = 0.011) and lower PTSD incidence (reported log risk ratio = 0.85; 95% CI 1.12–1.59; p = 0.023). The review reported a significant effect in preventive studies (d = 1.50; 95% CI 0.30–2.69; p = 0.014), but not in the subgroup treating established symptoms (d = 0.28; 95% CI −0.11 to 0.66; p = 0.161). The authors cautioned that few studies qualified and methodological quality was limited, so these pooled estimates are not a basis for routine care. Read the 2021 review and meta-analysis.

Individual trials do not show a uniform result
A randomized trial involving 64 adults treated after traumatic physical injuries tested a 10-day, low-dose hydrocortisone course begun within 12 hours. The authors reported fewer PTSD and depression symptoms and better health-related quality of life in the hydrocortisone group during the first three months. Its small sample and specific population limit how far the result can be generalized. Read the traumatic-injury trial.
By contrast, a randomized, placebo-controlled emergency-department study enrolled 118 people with acute stress symptoms and tested one intravenous dose within six hours of trauma. At 13 months, the hydrocortisone and placebo groups did not differ overall in PTSD prevalence or symptom severity. A reported interaction involving timing of trauma was a subgroup finding; it does not change the null overall result. Read the emergency-department trial.

A separate 2019 review of 19 randomized trials concluded that most included studies were methodologically weak. Its positive evidence for hydrocortisone prevention came from people with severe physical illness, which further limits generalization to other trauma survivors. Read the 2019 review.

The question is different once PTSD is established. In trials of prolonged exposure, hydrocortisone was given before therapy sessions as an addition to psychotherapy, not as a replacement for it. The larger cited trial did not find an overall benefit from this augmentation.

The larger veteran trial found no overall added benefit
A randomized, double-blind trial involving 60 U.S. veterans with PTSD compared 30 mg oral hydrocortisone with placebo before prolonged exposure sessions. Symptoms improved over time in both groups, but the intention-to-treat analysis found no measurable overall improvement in symptoms or secondary outcomes from adding hydrocortisone. Exploratory subgroup findings suggest questions for further study, not a general treatment benefit. Read the 2021 trial.
A small pilot result is not confirmation
A 2014 pilot trial randomized 24 veterans to hydrocortisone or placebo before prolonged exposure sessions and associated hydrocortisone with greater symptom reduction. The authors attributed the result to better treatment retention. With a small sample and a later, larger trial showing no overall augmentation benefit, the pilot remains preliminary. What current PTSD guidelines recommend
The 2023 VA/DoD guideline gives a strong recommendation against hydrocortisone as monotherapy for PTSD, citing the lack of strong efficacy evidence and/or known adverse-effect profiles and associated risks. It separately finds insufficient evidence to recommend for or against hydrocortisone augmentation. Those statements have different scopes: the recommendation against stand-alone treatment does not mean every research question about adding hydrocortisone to therapy is settled. The guideline recommends specific trauma-focused psychotherapies over pharmacotherapy, including prolonged exposure, cognitive processing therapy, and eye movement desensitization and reprocessing (EMDR). It also recommends paroxetine, sertraline, or venlafaxine as medication options. A 2019 American Psychological Association guideline summary likewise strongly recommended several psychotherapies and conditionally recommended a set of medications; it does not establish stress hormones as routine treatment. How to judge claims about cortisol and PTSD
When assessing a study or treatment claim, check what it actually tested rather than treating all “stress hormone” findings as interchangeable:
- Timing: Was hydrocortisone given shortly after trauma to attempt prevention, or to someone with established PTSD?
- Population: Were participants acutely ill or physically injured, or do the findings concern a broader group of trauma survivors?
- Role: Was hydrocortisone studied on its own or as an addition to psychotherapy?
- Evidence scale: Is the result from a small pilot, a larger randomized trial, or a pooled review with limited and heterogeneous studies?
- Outcome and follow-up: Did the study measure symptoms, PTSD diagnosis or incidence, treatment retention, or another outcome—and for how long?
These distinctions explain why an encouraging early-intervention result cannot establish hydrocortisone as treatment for existing PTSD, and why a subgroup observation cannot outweigh a trial’s overall result.
Discussing treatment options with a clinician
For someone seeking care for PTSD, the guideline-supported approaches are a more appropriate starting point for a clinician conversation than cortisol supplements, home testing, or self-directed hydrocortisone. A qualified clinician can discuss trauma-focused psychotherapy and guideline-supported medication options in light of the person’s circumstances. Clinical guidelines inform decisions; they do not replace individualized clinical judgment.
