A 2017 research study showed that a specialized imaging test could detect whether urinary tract infection bacteria responded to antibiotics in under 30 minutes from loading a urine sample. That is a promising proof of concept—not a routine home test or a guarantee that every infection, bacterium, or antibiotic can be assessed that quickly.
What did the under-30-minute test actually show?
In a 2017 Proceedings of the National Academy of Sciences study, researchers used a custom microfluidic chip and phase-contrast microscopy to measure how individual bacteria grew after antibiotic exposure. The authors reported an end-to-end time of less than 30 minutes from sample loading to diagnostic readout. The measured bacterial response itself took 3 to 11 minutes, depending on the antibiotic. The authors’ significance statement says: “We show that it is possible to determine if a urinary tract infection is caused by resistant bacteria within 30 min of loading a urine sample, even if the bacterial concentration in the urine is very low.” This describes what their research demonstrated; it does not establish that the test is routinely available to patients.

The test did not look for a visible resistance marker or identify a resistance gene. It measured a phenotype: whether antibiotic exposure changed bacterial growth. The chip trapped individual bacteria in two rows of 2,000 cell traps. One row received antibiotic-free reference medium, while the other received medium containing the antibiotic under assessment.

Time-lapse phase-contrast images tracked how the cells extended. Researchers compared the treated cells’ normalized growth response with the untreated reference. Because changes in growth could be measured without waiting for colonies to become visible, the assay produced a result much sooner than conventional colony-based testing can.
How strong was the clinical-isolate comparison?
The study’s blinded clinical-isolate comparison focused on ciprofloxacin susceptibility in uropathogenic Escherichia coli (UPEC). Of 50 collected isolates, one did not grow during preparation and was excluded. For the 49 included isolates, the chip’s ciprofloxacin classifications agreed with the hospital’s disk-diffusion results within 10 minutes.

- TRUST THE SAME TYPE OF UTI TESTS MANY DOCTORS USE: Two tests in one; our UTI test measures both Leukocytes and Nitrites, two common indicators of a UTI. It is also the same urinary tract infection test used in many doctor's offices
- HOW TO USE: Use the test at home - designed to make it easy and sanitary to use, just hold the test strip vertically to read UTI test results
- ACCURATE RESULTS: AZO UTI Test Strips offers you reliable, over-the-counter UTI results at home you can rely on. Test results are ready to read in just two minutes
- CONVENIENT, EASY TO USE TEST STRIPS: Our UTI Test Strips give you results quickly and our suite of urinary health products including our AZO Cranberry Softgels and Cranberry Gummies can help cleanse and protect the urinary tract
That is complete agreement within this particular study set—not a population-wide sensitivity or specificity guarantee. The researchers also measured response times for nine antibiotics used for UTIs and reported supplementary work involving additional UTI pathogens. Those results should not be read as equivalent clinical validation for every organism–antibiotic combination.

The ciprofloxacin comparison used clinical UPEC isolates, but that result alone does not show that the chip was deployed as a routine test directly in clinical care. The research system required a specialized microfluidic chip, fluid handling, microscopy, and image analysis; it was not an off-the-shelf home diagnostic.

How does it compare with later microscopy-based testing?
A separate 2024 study reported another microscopy-based approach: a multipad agarose plate that measured minimum inhibitory concentration (MIC) from microcolony growth rates within three hours of antibiotic incubation. The study used laboratory monocultures of E. coli K-12 MG1655 and tested nine antibiotics. It is related work, not a replication of the 2017 chip’s under-30-minute result. Read the 2024 study in the Journal of the Royal Society Interface.

Quick Recap
What should a reader take from the result?
- Single-cell imaging can assess antibiotic response by measuring growth, rather than by visually identifying resistance or detecting a selected gene.
- The strongest clinical-isolate result reported for the 2017 test was a blinded ciprofloxacin comparison in 49 UPEC isolates.
- The under-30-minute result is a research demonstration using specialized equipment, not evidence of a universal or currently accessible rapid test.
