Short answer: A causal link between Ozempic or other GLP-1 medicines and nail problems has not been established. A 2025 analysis found reports of nail changes in a spontaneous-safety database, but those reports cannot show how frequently nail changes occur in treated patients or prove that a medicine caused them.
Ozempic’s current prescribing information does mention postmarketing alopecia (hair loss), while the cited postmarketing section does not list nail changes. If your nails change after starting or increasing a GLP-1 drug, contact the prescribing clinician and ask whether a board-certified dermatologist should evaluate you.

What the evidence actually shows
Ozempic’s official label
The current DailyMed Ozempic (semaglutide) prescribing information lists alopecia under postmarketing skin and subcutaneous-tissue reactions. The label explains that these reports were submitted voluntarily by a population of uncertain size. Because there is no reliable denominator, the frequency cannot be estimated, and a causal relationship cannot be established. In the postmarketing passage cited here, nail changes are not listed.
Alopecia is a hair finding, not a nail finding. Its presence on the label should not be treated as evidence that Ozempic causes brittle nails, ridges, discoloration, separation, or any other nail condition.

The 2025 nail-change report analysis
An April 2025 analysis in the Journal of Drugs in Dermatology tabulated 71 reports coded as nail changes among the reactions analyzed for the included GLP-1 agonists. Those reports represented 0.61% of the analyzed reactions—not 0.61% of people taking the medicines. The study used spontaneous adverse-event reports, so it cannot establish incidence, compare risk between drugs with different exposure levels, or prove that a GLP-1 medicine caused an individual’s nail change. The publication is available as a PDF of the April 2025 analysis.
The basic biology question is still open
A 2025 dermatology review found that no research had specifically investigated whether GLP-1 receptors are present in human nail tissue or what role they might play in nail biology. Without that work, there is no confirmed biological pathway that explains a GLP-1-specific nail effect. The review is published at Karger.
Why a safety signal is not proof of a side effect
Spontaneous-report systems are designed to detect possible safety signals. A patient, clinician, manufacturer, or other reporter can submit an event after a medicine was used. Such systems are valuable for noticing unusual patterns, but they do not record every person who took the drug without a nail problem. They also lack a consistent count of exposed patients, consistent examination of the nails, and a control group of people who did not take the medicine.
For that reason, “71 reports” means 71 submitted reports that met the analysis criteria. It does not mean 71 confirmed cases, and it cannot be converted into a percentage of users. The 0.61% figure has the same limitation: it is the share of analyzed adverse-event reactions coded as nail changes, not a population risk.

Reports can also contain incomplete medical histories, uncertain timing, duplicate submissions, or other medicines and illnesses that could affect the nails. A signal warrants better study; it does not settle causality. A controlled study with a defined group of treated patients, a comparison group, standardized nail examinations, and a known follow-up period would be needed to estimate risk more reliably.
What “nail problems” can mean
People use the phrase for many different observations. A clinician may ask you to describe or photograph changes such as:
- New brittleness, splitting, peeling, or breakage.
- Horizontal or vertical ridges and changes in surface texture.
- White, yellow, brown, green, or dark discoloration.
- Thickening, thinning, distortion, or a change in growth rate.
- The nail lifting from the nail bed or shedding.
- Redness, swelling, tenderness, drainage, or changes in the surrounding skin.
These descriptions are not diagnoses. Similar-looking changes can have very different causes, and a photograph or symptom timeline cannot determine whether a medicine is responsible.

No. Noticing a nail change after starting, stopping, or increasing a GLP-1 medicine establishes a timeline, not causation. Nails grow slowly, so a visible change may reflect an event that occurred weeks or months earlier. At the same time, other health changes, treatments, injuries, or exposures may occur during the same period.

- DIRECTIONS FOR USE: Apply 2 coats over dry, clean nails and let each coat dry before applying another. Use with nail color as base and top coat. For longest-lasting protection, apply another coat every 3 days. Remove easily with nail polish remover
Do not attribute a symptom to the medicine solely because it appeared afterward, and do not dismiss it solely because the label does not list it. The useful next step is a medication-and-health review that considers the exact onset, which nails are affected, the pattern of change, recent illness or injury, other medicines, and any relevant medical history.
What to do if your nails change while taking a GLP-1 drug
1. Record the change
Take clear, dated photographs in consistent lighting. Note whether one nail or many are involved, whether fingernails or toenails are affected, when you first noticed the change, and whether the appearance is progressing. Keep a list of the GLP-1 product, dose, dose changes, start date, and other prescription or over-the-counter medicines.
2. Contact the prescribing clinician
Tell the prescriber about the finding rather than making a dose change on your own. The clinician can review the reason you take the medicine, your treatment response, other possible explanations, and whether an examination or testing is appropriate. Do not stop a medication that controls diabetes or another condition without a plan for continuing that care.

3. Ask whether dermatology evaluation is appropriate
The American Academy of Dermatology (AAD) advises people to communicate with their medical team and work with a board-certified dermatologist for skin, hair, or nail side effects. A dermatologist can examine the nails directly and decide whether the pattern needs additional evaluation.

4. Avoid unproven self-treatment
The available evidence does not identify a particular supplement, nail hardener, tool, or retail product as a treatment for suspected GLP-1-associated nail changes. Starting several products at once can irritate the nail or obscure the original pattern. Ask a clinician before taking supplements, especially if you have another medical condition or use medicines that could interact with them.
When a nail change deserves prompt medical attention
Arrange prompt care for significant pain, rapidly increasing swelling, pus or other drainage, fever, a nail that is lifting after an injury, or a new dark streak or pigment that is changing. These findings can require assessment regardless of whether you use a GLP-1 medicine. A clinician can tell you how urgently you need to be seen based on the appearance and your health history.

An appointment usually begins with a detailed timeline and inspection of the affected nails and surrounding skin. Bring your photographs and complete medication list. The clinician may ask about injuries, manicures or other repeated trauma, recent illnesses, skin conditions, and whether the change affects several nails or only one.

The clinician may simply monitor growth, or may choose targeted tests or a dermatology referral when infection, inflammation, medication effects, or another medical cause is possible. What is known about GLP-1 drugs and skin, hair, and nails
The AAD describes a mixed dermatologic picture for GLP-1 medicines. Steven Daveluy, MD, FAAD, told the organization: “We now know that GLP-1 medications can have an impact on the skin and hair. There are some skin conditions that may improve after starting a GLP-1, like psoriasis and hidradenitis suppurativa. There are other conditions, such as hair loss or facial appearance, which may get worse. Understanding these risks and benefits can help you make a more informed choice when thinking about starting a GLP-1.”
That statement addresses skin and hair broadly; it is not a finding that GLP-1 drugs cause nail problems. The same distinction matters when reading reports of alopecia in the Ozempic label: a hair-related postmarketing report does not establish a nail-related adverse effect.
Should you switch or stop the medicine?
There is no evidence in the cited material that one GLP-1 medicine is safer for nails than another. Raw report counts cannot rank individual drugs because they do not provide comparable exposure denominators. A decision to continue, adjust, or change treatment should weigh the medicine’s established benefits, the severity and progression of the nail finding, alternative explanations, and your overall medical needs.

Questions to take to your appointment
- Could the appearance represent a nail disorder that needs examination or testing?
- Does the timing fit any known effect of my medicines, or should we look for another cause?
- Should I continue the current dose while this is evaluated?
- Would a board-certified dermatologist add useful expertise?
- Which changes should prompt an urgent call?
- Are any supplements or topical products appropriate for me, and what risks do they carry?
Bottom line on Ozempic and nail problems
Current evidence does not establish that Ozempic or other GLP-1 drugs cause nail problems. The 2025 report analysis is a signal for further investigation, not an incidence estimate or proof of causality. Ozempic labeling includes postmarketing alopecia but does not list nail changes in the cited section, and direct research on GLP-1 receptors in human nail biology is lacking. New or worsening nail symptoms should be documented and discussed with the prescribing clinician, with dermatology evaluation when appropriate.
