The realistic option depends on the subtype and cause. Acetazolamide may be considered when surgery is not possible, and lumbar puncture or drainage can temporarily lower pressure or predict whether a shunt may help. Neither is an established long-term cure.
Obstructive hydrocephalus may sometimes be treated with endoscopic third ventriculostomy (ETV), which creates another CSF pathway but is still an operation. For idiopathic normal-pressure hydrocephalus (iNPH), shunting remains the best-supported disease-directed treatment.

Why the type of hydrocephalus matters first
Hydrocephalus is a problem of CSF buildup and flow. “Without surgery” can mean several different things: taking medicine, having fluid removed temporarily, receiving rehabilitation, or avoiding an implanted shunt by choosing ETV. These approaches do not have the same purpose or evidence.
Obstructive hydrocephalus
In obstructive hydrocephalus, a blockage prevents CSF from moving normally through the ventricles or outflow pathways. If the blockage is the main cause, ETV may create an alternate route for CSF. The NHS says ETV “is not suitable for everyone, but it could be an option if the build-up of CSF in your brain is caused by a blockage.” ETV is therefore a possible alternative to a shunt for selected patients, not a medicine-based treatment and not a guarantee that hydrocephalus will resolve permanently.

Communicating hydrocephalus and idiopathic normal-pressure hydrocephalus
In communicating hydrocephalus, the ventricles remain connected but CSF absorption or circulation is impaired. Idiopathic normal-pressure hydrocephalus, or iNPH, is a communicating form associated with a characteristic pattern of gait difficulty, cognitive change and urinary symptoms. The treatment evidence for iNPH differs from that for a clear blockage: ETV is not routine therapy, while CSF shunting has the strongest disease-directed support.
What “without surgery” can realistically accomplish
A treatment can be useful without being curative. The key question is whether it lowers pressure briefly, provides diagnostic information, supports function, or supplies durable CSF diversion.
| Approach | Main purpose | How durable is the effect? | Important limitation |
|---|---|---|---|
| Acetazolamide or another carbonic-anhydrase inhibitor | Possible pressure or symptom management when a person is not a surgical candidate | Not established as a permanent correction | Evidence does not support it as a dependable long-term substitute for CSF diversion |
| Lumbar puncture (tap test) | Remove CSF temporarily and observe symptoms; help predict shunt response | Temporary, if symptoms improve at all | A negative test does not reliably exclude benefit from a shunt |
| External lumbar drainage | Longer drainage trial to assess likely response, especially in iNPH evaluation | Temporary and performed under medical supervision | It is a test or bridge, not a permanent replacement for diversion |
| Supportive physical, occupational or continence care | Improve mobility, safety and day-to-day function | Depends on the underlying condition and rehabilitation response | Does not remove the CSF-flow problem |
| Endoscopic third ventriculostomy | Create an alternate CSF route in selected obstructive cases | Can provide durable diversion for some people | It is still an operation and is unsuitable for many patients |
Medication: where acetazolamide fits
Carbonic-anhydrase inhibitors such as acetazolamide may be considered when a patient is not a surgical candidate. The clinical review informing current guidance describes this as a limited role, not proof that the medicine cures hydrocephalus or reliably replaces a shunt or ETV.
Medication should therefore be viewed as an individualized option under specialist supervision. A clinician must first establish the likely hydrocephalus subtype, review brain imaging and consider other causes of the symptoms. Taking a drug without confirming the diagnosis can delay treatment of a blockage or another urgent neurological condition. Prescription treatment also requires monitoring for adverse effects and for signs that pressure or neurological function is worsening.

Lumbar puncture and lumbar drainage: useful tests, not a cure
The tap test
A lumbar puncture can remove a measured amount of CSF, after which clinicians assess walking, balance, thinking or bladder-related symptoms. In suspected iNPH, improvement after the tap test can increase the likelihood that a shunt will help. The result is predictive rather than curative: the fluid is removed temporarily, while the underlying CSF-circulation problem remains.
Why a negative tap test is not decisive
Failure to improve after one lumbar puncture does not reliably rule out shunt benefit. The American Academy of Neurology (AAN) guideline notes that a positive response to external lumbar drainage or repeated lumbar punctures may increase the chance of response, but a negative test does not exclude it. Clinicians may therefore combine the response with the symptom pattern, imaging and overall medical risk instead of treating one test as an absolute yes-or-no answer.
External lumbar drainage
External drainage can provide a longer, closely observed trial than a single tap. It is still temporary and involves a medical procedure, so it is not a home treatment or a permanent alternative to diversion. Its role is to provide additional evidence when deciding whether an implanted shunt is reasonable.
ETV: an alternative to a shunt, but not a nonsurgical treatment
During ETV, a neurosurgeon uses an endoscope to create an opening that lets CSF bypass a blockage. Because no shunt tubing is implanted, patients sometimes describe ETV as “avoiding surgery,” but it remains an operation with procedural risks and follow-up requirements.

ETV is most relevant when imaging shows obstructive hydrocephalus and the blockage can be bypassed safely. The NHS cautions that it is not suitable for everyone. Age, the exact obstruction, prior procedures, anatomy and the clinician’s assessment all affect candidacy.
ETV should not be presented as routine treatment for classic iNPH. A Cochrane review found very-low-quality evidence for ETV in iNPH and concluded: “Doctors and patients should be aware of the limitations of the evidence on the effectiveness and safety of this operation.” That uncertainty is why an ETV discussion for iNPH differs from an ETV discussion for a clearly demonstrated obstruction.
What the evidence says about shunting in iNPH
For people with probable iNPH, the best-supported disease-directed treatment is CSF shunting. A shunt diverts CSF through an implanted system so that pressure and flow can be managed over time. It is not risk-free, and the decision should be individualized rather than based on symptoms alone.

The 2015 AAN guideline reported a 96% chance of subjective improvement and an 83% chance of improvement on a timed walk test at six months in the selected iNPH study populations it reviewed. It also reported an 11% risk of serious adverse events. These are evidence estimates from study groups, not guarantees for a particular person. The AAN recommendation is that clinicians may offer shunting for subjective iNPH symptoms and gait while carefully weighing the significant adverse-event risk.

The 2021 Japanese guideline states that CSF shunt intervention is effective for iNPH and recommends a programmable-pressure valve. It says an anti-siphon or gravity mechanism can be added when intracranial hypotension is detected. Valve choice and adjustment are specialist decisions; they do not turn a shunt into a nonsurgical treatment.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How specialists decide which option is appropriate
A safe plan is based on several pieces of evidence rather than a single symptom or scan.
- Confirm the syndrome and urgency. The clinician reviews the symptom pattern, neurological examination and the speed of change. Rapid deterioration requires urgent assessment rather than an outpatient trial of medication.
- Identify the subtype and cause. Brain imaging is used to look for obstruction, ventricular enlargement and other findings that distinguish obstructive disease from communicating hydrocephalus or iNPH.
- Assess likely benefit. In suspected iNPH, gait testing and cognitive or bladder assessments can be compared before and after a tap test, repeated lumbar punctures or external lumbar drainage.
- Review medical and surgical risk. Other illnesses, frailty, medications, prior operations and the person’s ability to participate in follow-up affect whether a shunt, ETV or conservative plan is reasonable.
- Set a measurable goal. The team should specify whether the target is walking speed, balance, thinking, bladder function, headache control or prevention of further deterioration. A defined goal makes a temporary drainage response easier to interpret.
- Plan follow-up. Medication trials, drainage tests, ETV and shunts all require reassessment. Worsening symptoms or a lack of the expected response should prompt the treating team to revisit the diagnosis and treatment plan.
Limits of supportive and “natural” approaches
Physical therapy, occupational therapy, continence support and home-safety changes can help a person function more safely while the medical evaluation proceeds or after treatment. They may address weakness, falls, transfers or daily routines, but they do not reopen a blocked CSF pathway or correct impaired absorption.

When symptoms need urgent medical assessment
Hydrocephalus can worsen quickly, and symptoms alone cannot establish the diagnosis. Seek urgent medical care for a rapidly worsening headache, repeated vomiting, increasing drowsiness, confusion, a sudden decline in walking, new visual symptoms or another rapid neurological change. Do not attempt to manage these signs with self-directed medication or repeated fluid removal.
For slower changes in gait, thinking or bladder control, arrange a clinical assessment rather than assuming they are normal aging or that a lumbar puncture will solve the problem. A specialist can determine whether hydrocephalus is present, which subtype is most likely and whether a temporary test, ETV, shunt or supportive plan offers the best balance of benefit and risk.
Bottom line for the main questions
Can hydrocephalus go away without surgery?
Usually not through medication, diet or exercise. Temporary CSF removal may relieve symptoms or provide useful diagnostic information, but durable correction generally requires CSF diversion.
Can medication treat hydrocephalus?
Acetazolamide may be considered for selected people who are not surgical candidates. It is not established as a cure or reliable long-term replacement for a procedure.
Can a lumbar puncture cure hydrocephalus?
No. A tap test or lumbar drain removes CSF temporarily. A positive response can support a later shunt decision, while a negative response does not reliably rule out benefit.
Is ETV an alternative to a shunt?
For some cases of obstructive hydrocephalus, yes. ETV is still an operation and is not suitable for everyone; evidence for its routine use in iNPH is very limited.
Can normal-pressure hydrocephalus be treated without surgery?
Supportive care and temporary drainage can help with evaluation and daily function, but shunting has the strongest disease-directed evidence. The expected benefit and the significant adverse-event risk should be weighed for the individual.
