Yes—cryopreserved red blood cells can be transfused, but never while they are still frozen. A blood bank thaws the unit, removes the glycerol used to protect the cells during freezing, washes and tests the cells, and issues the finished component as deglycerolized red blood cells. This is a regulated clinical process used mainly when a patient needs an unusual antigen profile, when suitable standard components are unavailable, or when blood supplies must be supplemented during a demand surge.
What “frozen blood” means
In transfusion medicine, “frozen blood” usually refers to red blood cells that have been cryopreserved. It does not mean that every component of whole blood—such as plasma, platelets and red cells—is frozen and later infused in the same form. Plasma and platelets have different storage methods and indications. The process described here concerns cryopreserved red-cell units.

Freezing red cells without protection would damage their membranes. A cryoprotectant, usually glycerol, is mixed with the cells before freezing. Glycerol limits injury from ice formation and allows the unit to remain in long-term frozen storage. Because glycerol would be harmful if infused in the finished component, it must be removed after thawing. The washed product is called deglycerolized red blood cells.
A patient therefore receives a prepared red-cell component, not a frozen bag. Selection, thawing, deglycerolization, compatibility testing and release are performed by trained blood-bank staff. A person should never attempt to freeze, thaw, wash or administer blood outside a licensed transfusion service.

How a cryopreserved unit becomes ready for transfusion
The exact equipment and wash sequence depend on the blood establishment, product label and jurisdiction. The clinical pathway generally follows these stages:
- Collection and testing. The red-cell unit is collected, typed and screened under the same regulated blood-safety system used for other components. If the unit has a valuable or uncommon antigen profile, the blood bank may select it for cryopreservation rather than use it immediately as a liquid component.
- Addition of glycerol. The cells are combined with glycerol in a controlled process. The cryoprotectant protects the cells during freezing but is not intended to remain in the transfusable product.
- Frozen storage. The prepared unit is stored at the required temperature, with its identity, expiration date and processing records maintained. Long-term dating is tied to the storage temperature and the applicable national rule.
- Thawing when selected. When the unit is needed, blood-bank personnel thaw it using validated equipment and procedures. The unit remains under the blood bank’s control; it is not handed to a patient or ward while frozen.
- Sequential washing. After thawing, the cells are washed to remove glycerol and other residual fluid. The Joint Trauma System’s August 5, 2024 guideline describes sequential washing after thawing. The number of washes and the equipment used are part of the approved processing system.
- Resuspension and inspection. The washed cells are suspended in an appropriate solution. Staff inspect the component and follow the product’s quality-control requirements before it can be issued.
- Compatibility and release. The transfusion service performs the required patient testing and compatibility checks, then labels and releases the deglycerolized unit for transfusion. Processing a frozen component and compatibility-testing a patient are related but distinct regulated activities.
The AABB Circular of Information for the Use of Human Blood and Blood Components (June 2024 edition) states that deglycerolized red cells contain at least 80% of the red cells from the original unit and have approximately the same expected post-transfusion survival as red cells generally. That statement describes the product standard; it is not a promise that every patient will have an identical clinical response.
How long can frozen red cells be stored?
In the United States, the cited federal dating rule allows cryopreserved red cells to be stored for up to 10 years at −65 °C or colder. This is a U.S. product-dating limit, not a universal rule for every country or every freezing system. The individual unit’s label and the manufacturer’s instructions control.

| Stage | Published U.S. limit | What controls the limit |
|---|---|---|
| Frozen red cells | Up to 10 years at −65 °C or colder | 21 CFR § 610.53, the approved processing method and the unit label |
| After deglycerolization, open system | 24 hours after the bag is entered | Federal dating rule and the product’s approved instructions |
| After deglycerolization, closed system with additive solution | 14 days after the bag is entered | Federal dating rule, the additive system and manufacturer instructions |
The short post-thaw window is important. Freezing can preserve a selected unit for years, but it does not create an indefinitely usable product after thawing. Once the bag has been entered and processed, the blood bank must follow the dating period for that particular system. A hospital may also apply a shorter limit if its validated procedure or the component label requires one.
Why would clinicians choose cryopreserved red cells?
Unusual antigen profiles
The AABB Circular says, “Frozen storage is especially suitable for red cells with unusual antigenic phenotypes.” Red-cell antigens are inherited markers on the cell surface. A patient who has antibodies against a common antigen may need donor cells lacking that antigen. If compatible units are rare, freezing a carefully characterized unit can preserve it until that patient—or another patient with the same need—requires it.
This does not mean that every patient with an uncommon blood type receives frozen blood. The transfusion service evaluates the patient’s antibody history, current testing, urgency and the units available through its network. Cryopreservation is one inventory strategy for difficult-to-match components.

When standard components are inappropriate or unavailable
Deglycerolized red cells are generally used when ordinary liquid components are not appropriate or cannot be obtained in time. The decision is clinical and inventory-based. A frozen unit is not automatically superior to a suitable liquid unit; it is selected because its characteristics solve a particular compatibility or supply problem.

Washing removes glycerol and also reduces residual plasma constituents. The AABB Circular notes that this may make deglycerolized red cells useful for patients who have had previous severe allergic reactions to transfusion. Washing is not a guarantee that a reaction cannot occur, and the patient’s medical history and transfusion plan still determine the safeguards used.
Periods of increased demand
The Joint Trauma System guideline issued August 5, 2024, describes frozen and deglycerolized red cells as a supplement to other blood sources during surges in transfusion demand. That use is especially relevant to organized emergency and military systems with validated storage and processing capacity. It should not be read as evidence that frozen units are routinely stocked in every civilian hospital.

Frozen red cells compared with standard liquid red cells
| Consideration | Cryopreserved red cells | Standard liquid red cells |
|---|---|---|
| Primary reason for selection | Unusual antigen profile, lack of a suitable standard unit, selected plasma-reduction needs, or supply supplementation during a surge | Routine transfusion when a compatible liquid component is available and appropriate |
| Preparation before infusion | Must be thawed, deglycerolized and washed, then checked and compatibility-tested | Does not require the cryopreservation and deglycerolization sequence |
| Long-term storage | Can remain frozen for the applicable long-term dating period; the cited U.S. limit is up to 10 years at −65 °C or colder | Uses the dating period assigned to its liquid-storage system |
| Post-processing dating | Depends on the processing system: the cited U.S. limits are 24 hours for an open system and 14 days for a closed system with additive solution | Not applicable to a unit that has not undergone deglycerolization |
| Availability | Depends on specialized inventory, equipment, staff and local agreements | Depends on the hospital’s ordinary inventory and blood-supplier access |
| General quality claim | Storage alone does not establish a broad outcome advantage; it is a solution for particular clinical or inventory needs | Often the appropriate choice when it meets the patient’s compatibility and clinical requirements |
Is frozen blood available at every hospital?
There is no reliable national civilian inventory figure that can answer this for every hospital. Availability varies with the institution’s blood-bank capabilities, the blood center or supplier it uses, the patient’s antigen requirements and the urgency of the transfusion. Some facilities may have frozen units on site; others may need to request a unit from a regional or specialized source, while some may not offer the service at all.
If a clinician believes a patient may need a rare or specially processed component, the hospital transfusion service can search its own inventory and contact other blood establishments. The treating team—not the patient—must coordinate thawing, washing, testing and transport because the component has a limited post-processing dating window.

What are the risks and limitations?
Inadequate deglycerolization
The AABB Circular warns of a risk of intravascular hemolysis if deglycerolization is inadequate. Hemolysis means red cells rupture and release their contents into the bloodstream. Validated washing procedures, equipment checks, visual inspection and laboratory quality controls are intended to prevent a nonconforming unit from being issued.
Processing does not remove all transfusion risks
Cryopreservation addresses storage and selected component characteristics. It does not eliminate ordinary transfusion risks such as incompatibility, bacterial contamination, transfusion reactions or errors in patient identification. Blood establishments and hospital transfusion services use testing, labeling and administration checks to reduce those risks.
Dating and labeling are product-specific
The figures in the table are U.S. federal dating standards cited in 21 CFR § 610.53. Other jurisdictions may use different rules, and a particular manufacturer’s closed system may carry instructions that are more restrictive. Staff must follow the label and validated procedure for the actual unit rather than rely on a generic number.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesNot a do-it-yourself treatment
Household freezers cannot provide the required temperature control, and thawing or washing blood without validated equipment can damage cells or leave glycerol behind. FDA materials distinguish blood establishments that process components from hospital transfusion services that perform compatibility testing and issue products. Those regulated roles are why a patient should never transport or prepare a frozen unit independently.
What should a patient or family member ask?
- What component is planned? Confirm whether the team is discussing cryopreserved red cells, a standard liquid red-cell unit or another blood component.
- Why is this component being selected? The reason may be an antibody or antigen-matching problem, a prior severe allergic reaction, or limited supply.
- Has compatibility testing been completed? Ask whether the transfusion service has completed the testing required for the patient’s blood type and antibody history.
- Where will the unit come from? The hospital can explain whether it is in local inventory or must be obtained from another blood establishment.
- What processing has occurred? The staff can tell you whether the unit has been thawed and deglycerolized and what dating period applies.
- What symptoms should be reported during transfusion? Follow the clinical team’s instructions and immediately report new chills, fever, back or chest pain, breathing difficulty, rash, nausea or a feeling that something is wrong.
Bottom line
Frozen red blood cells can be used for transfusion after a blood bank thaws and washes them to remove glycerol. Under the cited U.S. standards, a unit may remain at −65 °C or colder for up to 10 years, but its usable period after deglycerolization is much shorter and depends on whether an open or closed processing system was used. Cryopreservation is a specialized option for unusual antigen profiles, difficult compatibility situations, selected allergy histories and periods of increased demand—not a routine replacement for suitable liquid red cells. The hospital transfusion service can determine whether the product is available and appropriate for an individual patient.
