Apple Watch Vitals can flag overnight readings that differ from your personal typical range, but it cannot tell you that you are getting sick or diagnose an illness. To give it the best chance of collecting consistent data, enable sleep tracking and Sleep Focus, wear the watch overnight for seven days to establish a typical range, and make sure it sits comfortably against your skin.
What Apple Watch Vitals can—and cannot—tell you
Vitals compares overnight measurements with a typical range built from your own collected data. Apple says illness is one factor that may affect those measurements. By default, a notification appears when at least two overnight metrics are outside their typical ranges; it may also mention possible contributing factors such as medication or elevation changes. That is a reason to review a pattern, not confirmation of an illness or its cause.

The overnight metrics are heart rate, respiratory rate, blood oxygen, wrist temperature and sleep duration. On Apple Watch Series 12 and Apple Watch Ultra 4, Vitals shows heart rate variability instead of sleep duration and also supports a daytime vitals view.

So if your watch showed unusual readings before you noticed symptoms, it is fair to describe that sequence as your experience—provided the timing and notification match what happened. It is not accurate to say that Vitals detected, predicted or diagnosed an infection.

How to set up Vitals for more consistent readings
- Update your devices. Apple’s setup checklist calls for an iPhone with the latest iOS and an Apple Watch with the latest watchOS.
- Turn on sleep tracking. In the Health app, set up Sleep and enable Track Sleep with Apple Watch. Use Sleep Focus for your scheduled sleep time.
- Wear the watch to bed for seven days. Apple says this gives Vitals the data it needs to establish your typical range. A baseline is personal, so the feature compares your readings with your own pattern rather than a universal normal value.
- Check the fit before sleeping. Wear the watch comfortably, with its back in contact with your skin. Apple warns that a watch that is too loose may not capture accurate readings; tightening it uncomfortably is not the solution.
These steps follow Apple’s collection guidance; they do not guarantee that every reading will be accurate or that Vitals will identify a health change.
Why might my Apple Watch say my vitals are out of range?
An out-of-range notice means multiple overnight metrics differed from your established typical range—not that Apple Watch has identified a specific problem. Apple says factors such as illness, medication and elevation changes may affect readings. The notification does not establish which factor is responsible.

Review the pattern in context rather than treating one alert as a diagnosis. On Apple Watch, Vitals history covers the last seven days. In the iPhone Health app, you can view daily, weekly, monthly and six-month charts.

- Wrist temperature: Apple lists support for Apple Watch Series 8 or later and all Apple Watch Ultra models. Wrist-temperature tracking is not the same as a direct body-temperature measurement.
- Daytime vitals: Apple lists this view for Apple Watch Series 12 and Apple Watch Ultra 4.
- Blood Oxygen: It is not available in Vitals for certain Apple Watch models purchased in the United States on or after January 18, 2024, with part numbers ending in LW/A or LS/A. Apple says those models analyze Blood Oxygen data on iPhone, with results shown in Health.

Apple states that “Vitals app measurements are not intended for medical use” and advises consulting a healthcare provider before making health decisions. Its Apple Watch User Guide also says, “Your Apple Watch is not a medical device.” If you are concerned about symptoms, seek appropriate clinical care rather than waiting for wearable data or treating a notification as a diagnosis.
