Apple’s documentation describes individual estimates and records, not an overall “health age.” The metric most likely to be mistaken for one is Cardio Fitness, Apple’s estimate of VO₂ max. It can help you follow a fitness trend, but it is not a diagnosis or a clinical measure of biological age.
What does Apple Watch cardio fitness mean?
Apple defines cardio fitness as a measurement of VO₂ max: the maximum amount of oxygen your body can consume during exercise. Apple Watch estimates it from heart-rate and motion-sensor data during certain outdoor workouts; it does not directly measure your health age.

Apple says the estimate is available on Apple Watch Series 3 or later during an Outdoor Walk, Outdoor Run, or Hiking workout recorded in the Workout app. Indoor workouts do not count. The calculation also uses personal information including age, sex, weight, height, and medications that may affect heart rate. See Apple’s cardio fitness guidance.
Apple documents a supported VO₂ max estimate range of 14–65 mL/kg/min. Cardio-fitness classifications are available to users aged 20 or older. These are product limits and availability details—not a scale that converts your result into a health age or a population benchmark.

How does Apple Watch estimate cardio fitness?
The watch needs qualifying outdoor activity and sensor readings to produce an estimate. Apple says an initial estimate may require at least 24 hours of wear, several qualifying outdoor workouts, and passive measurements. Push workouts are not supported.
Because the estimate depends on a particular activity and the watch’s sensor inputs, it is best understood as an estimate to follow over time, not as a direct laboratory measurement. Apple says a low estimate relative to a person’s age and sex may prompt a notification. It also warns that some conditions or medications that limit heart rate can cause an overestimate.

Are Vitals and sleep data part of a health-age score?
Apple documents other health-related information separately. The Vitals app can show overnight heart rate, respiratory rate, wrist temperature, blood oxygen where available, and sleep duration. It compares measurements with ranges based on your own data and can notify you when multiple metrics are outside those ranges. Factors such as medication, elevation, or illness may provide context for changes.

Is Apple Watch VO₂ max accurate?
It is an estimate, and its usefulness depends on the quality of the information and readings behind it. Apple advises keeping personal details up to date and wearing the watch so it fits properly. Calibration can also improve measurement accuracy. Apple’s guidance on getting accurate measurements notes that heart-rate readings can be affected by skin perfusion, cold, tattoos, and movement; rhythmic movement tends to produce better sensor results than irregular movement.
These qualifications matter when interpreting a change: it may reflect a genuine fitness trend, but conditions and sensor inputs can affect the estimate. The documentation cited here does not establish a clinical validation result or support treating the number as a diagnosis.

What the estimate can—and cannot—tell you
- It can: provide an estimate of cardio fitness based on supported outdoor workouts and related sensor data, which you can follow over time.
- It cannot: tell you a single, definitive “health age” or independently establish a medical diagnosis.
- Interpret cautiously: missing or inaccurate personal details, sensor conditions, and heart-rate-affecting conditions or medications can affect the estimate.
Apple’s user guidance states: “Your Apple Watch is not a medical device.” If you have concerns about your health or fitness, use the watch’s information as context rather than a substitute for a clinician’s assessment. Apple’s readiness-score guidance includes this reminder.
