Yes, retinal features may be associated with cardiovascular risk, and AI analysis of retinal photographs is being studied as a way to flag people who may need a standard risk assessment. But an eye exam or retinal scan cannot diagnose heart disease, show that a coronary artery is blocked, or replace evaluation by a clinician.
What can a retinal scan show about heart risk?
A retinal image captures structures at the back of the eye, including small blood vessels. Imaging the retina is non-invasive and already routine in eye care. Researchers have found associations between retinal features and systemic health: the National Eye Institute’s 2024 summary describes links between thinning in retinal layers and increased future risk across several disease categories, including cardiac disease. An association can be useful for research or risk estimation, but it does not establish that an individual currently has heart disease.

A retinal photograph is not a heart scan. It does not directly show coronary arteries, detect plaque in them, measure cholesterol, or take a blood-pressure reading. A concerning eye finding may be a reason to seek follow-up, not a diagnosis.

What the 2026 CLAiR study found
A prospective evaluation reported by the American College of Cardiology (ACC) on March 30, 2026, tested CLAiR, an AI system that analyzes retinal photographs to identify people likely to meet a threshold for elevated 10-year atherosclerotic cardiovascular disease (ASCVD) risk. The study enrolled 874 adults aged 40–75 at 10 U.S. eye-care and primary-care sites. Participants were not taking lipid-lowering medication and did not have known atherosclerosis.
#1 Best Overall
- Eye Contact Technology: Built-in design helps simulate direct eye contact, enhancing engagement and professionalism during calls and recordings
- 4K Ultra HD Video Quality: Crystal-clear resolution delivers sharp, detailed visuals for meetings, streaming, and content creation
- Noise-Cancelling Microphone: Reduces background noise for clear, natural audio in any environment
- Wide Compatibility: Zoom Certified and fully compatible with Microsoft Teams, Google Meet, and other major video platforms
- Professional Video Communication: Supports working from home, hosting webinars, and creating content with clarity, confidence, and connection
The comparison was with a standard risk-estimator classification—not confirmed heart disease. That estimator used age, sex, smoking status, blood pressure, and cholesterol. In the study sample, 26% had an estimated 10-year ASCVD risk of at least 7.5%. For identifying that classification, CLAiR had 91.1% sensitivity and 86.2% specificity. These figures describe agreement with the risk-estimator category; they are not accuracy rates for diagnosing existing coronary disease or predicting who will have a heart attack. The 26% figure describes this selected study sample, not the U.S. population.
The ACC report says 94% of images acquired in the study were usable by CLAiR. Imaging took about five minutes and the algorithm returned a result in about 30 seconds in the reported workflow. Those are study details, not guarantees for every camera, clinic, or patient. The cohort included 19% Black or African American participants and 26% Hispanic participants, but results should not automatically be generalized to populations or circumstances the evaluation did not establish.
Routine eye imaging and AI risk screening are different
| Approach | Purpose | What the result means |
|---|---|---|
| Routine retinal imaging or eye examination | Assess eye health and retinal structures as part of eye care. | Eye findings; not a cardiovascular diagnosis. |
| AI analysis of retinal photographs for cardiovascular risk | Emerging approach that compares image patterns with a cardiovascular risk category. | A possible risk flag that needs standard clinical assessment; not proof of existing heart disease. |
The distinction matters because the CLAiR evaluation used a standard risk estimate built from clinical information as its reference. It did not compare the retinal photograph with a direct examination of the heart or coronary arteries.

Discuss the result with a primary-care clinician, who can assess cardiovascular risk using standard clinical information and decide whether preventive care or further evaluation is appropriate. Do not start, stop, or change medication based on a retinal AI result alone. The ACC says the approach would not replace standard cardiovascular risk evaluation and that clear pathways are still needed to connect people with elevated results to clinical assessment and preventive treatment.
Blood pressure is one of the inputs used in standard risk estimation. If you monitor it at home, use the readings as information to share with your clinician; a home monitor does not diagnose heart disease or reproduce a retinal-screening algorithm.

The CLAiR study involved adults aged 40–75 who were not taking lipid-lowering medication and had no known atherosclerosis. The system was not designed for pregnant people or people with advanced eye disease. Its reported performance should not be assumed for younger people, treated patients, people with known cardiovascular disease, or those excluded from its intended use.
Other research helps explain why retinal imaging attracts interest, but it is not a substitute for validating a screening tool. An American Heart Association report in 2023 described a separate UK Biobank analysis of 1,101 people with prediabetes or type 2 diabetes followed for a median of 11 years: cardiovascular events occurred in 8.2% of the low-risk group, 15.2% of the moderate-risk group, and 18.5% of the high-risk group. These are outcomes in that cohort, which was predominantly white, not universal individual probabilities. The National Eye Institute also reported 259 genetic loci associated with retinal thickness in 2024; that is a finding about genetics and retinal structure, not a heart-screening statistic.
An NHLBI workshop roadmap identifies continuing needs including standardized, high-quality image capture; diverse longitudinal cohorts; improved measurement of retinal biomarkers; and better integration with clinical data and privacy protections. The American Academy of Ophthalmology’s 2022 educational material described AI prediction of cardiovascular and neurological disease from eye images as promising but not ready for clinical application at that time. The 2026 prospective evaluation adds newer evidence for risk classification, but does not by itself settle questions about real-world implementation or health outcomes. The ACC reported that the evaluation would support an FDA submission; the sources cited here do not establish current FDA clearance or authorization, or subsequent outcome trials.
Retinal imaging may also involve an additional fee, and the ACC notes that it is not covered by all U.S. vision plans as part of a standard visit. Ask the clinic and your plan about coverage before choosing an optional scan.
