What the 2026 paper actually says
“Tool or Companion? Reframing Conversational AI to Prevent Psychological Harm,” by Asia Maurich Novelli, Sukhwinder Shergill, and Andreia Sofia Teixeira, is a synthesis and argument drawing on research in AI, psychiatry, psychology, and network science. It discusses prior research and reports rather than measuring how often chatbot use leads to harm in a new population-level study.
The authors describe both possible benefits, such as short-term reductions in loneliness and improved mood, and adverse outcomes reported in clinical and nonclinical settings, including emotional dependence, worsening mental-health symptoms, and self-harm. Those reports warrant attention, but they do not establish a prevalence rate or show that the same effects occur for every user.
How a chatbot might affect emotional well-being
Social cues can make software feel like a relationship
The paper discusses features such as a distinct personality, warm or apparently empathic language, and fluent conversation. These cues may encourage a user to experience an AI system as a social partner. The authors treat this as a possible mechanism of concern, not proof that any single feature causes harm in every case.

The relationship is not reciprocal
A person may disclose private feelings while a chatbot responds in a supportive style. But the interaction is structurally asymmetric: the system does not have human vulnerability, mutual needs, or personal accountability. The authors argue that this difference matters when people rely on a chatbot for emotional support.
Effects can differ between people
Risk should not be inferred from a chatbot’s mere presence in someone’s life. As first author Asia Maurich Novelli put it: “The effects of these systems are unlikely to be the same for everyone.” The paper does not offer a validated consumer checklist that can predict which individual will benefit or be harmed.

Companion apps and general-purpose assistants are not the same use case

Is an AI chatbot a substitute for therapy or human support?
The authors’ central recommendation is to treat conversational AI primarily as a tool that supports human systems, not as a substitute for human relationships. That is a framing proposed by the paper, not a finding that any chatbot is validated or safe as mental-health care. The viewpoint does not provide clinical guidance for choosing a therapy service or establish that chatbot use alone can treat a mental-health condition.
For readers considering how they use a chatbot, a practical distinction is whether it helps with a limited task or becomes the main place they turn for emotional support. If use is displacing contact with trusted people or qualified care, that is a reason to involve human support rather than assuming the chatbot can fill that role.
