Short answer: a wrist wearable may notice physiological changes that sometimes precede flu, COVID-19 or another infection, but it cannot identify a pathogen or confirm that you are ill. Heart rate, heart-rate variability (HRV), sleep, activity, breathing rate and skin temperature are indirect signals that also change after alcohol, travel, stress, poor sleep, exercise and many noninfectious conditions. The LSTM result in this topic comes from a specific COVID-19 bracelet study; a larger influenza study used XGBoost instead and achieved only moderate discrimination.

What a wrist wearable can—and cannot—detect

A smartwatch or fitness tracker measures body responses, not viruses. An infection can raise resting heart rate, disturb sleep, alter HRV, increase respiratory rate or produce a temperature pattern. Machine-learning software can combine those changes into an alert that your current data differs from your usual baseline.

That alert is a prompt to check symptoms and consider a test. It is not a flu diagnosis, a COVID-19 diagnosis or proof that any pathogen is present. The same pattern can follow dehydration, strenuous training, emotional stress, jet lag, alcohol, medication, menstrual-cycle changes or an unrelated illness. No study summarized here validates a retail watch as a general infection screening test.

How an LSTM analyzes wrist signals

Signals collected by the bracelet or watch

  • Heart rate: resting and sleep heart rate can rise when the body is under physiological stress.
  • Heart-rate variability: changes in beat-to-beat timing may reflect altered autonomic regulation.
  • Sleep and activity: shorter sleep, more awakenings, fewer steps or a sudden change in routine can accompany illness.
  • Respiratory rate: breathing can become faster or irregular during a respiratory infection.
  • Skin temperature and perfusion: peripheral temperature and blood-flow patterns may shift, although they are not the same as a clinical fever measurement.

Why use a long short-term memory network?

An LSTM is a recurrent neural-network architecture designed for sequences. Instead of judging one heart-rate reading in isolation, it can learn relationships across hours or days—for example, a gradual rise in sleep heart rate combined with falling HRV and disrupted sleep. The model produces a probability or classification from that time series; it does not “see” influenza or coronavirus directly.

Model output depends on the population, sensors, labels and training procedure. A network trained on a research bracelet and a narrowly defined COVID-19 cohort should not be assumed to work on a different watch, pathogen or population without independent validation.

What the key studies actually found

The findings below answer different questions. They use different pathogens, devices, cohorts, inputs and performance measures, so their percentages are not interchangeable.

Study and setting Device/data and model Outcome and result Important qualification
COVI-GAPP, BMJ Open (2022) Sensor bracelet measuring respiratory rate, heart rate, HRV, wrist-skin temperature and skin perfusion; LSTM recurrent neural network COVID-19 detection up to two days before symptoms; recall 0.73 in training and 0.68 in testing Interim analysis; indexed details do not establish performance for flu or consumer watches
Farooq et al., JMIR (2024) Commercial Fitbit data (steps, heart rate and sleep) plus patient-reported symptoms; extreme gradient boosting (XGBoost) Independent HTRI evaluation AUC 0.75 for combined data; symptom-only AUC 0.74 Moderate discrimination in people with influenza-like illness; activity-only models were weaker
Natarajan et al., JAMA Network Open (2021) Research-grade noninvasive wrist sensors after controlled exposure; 39 adults in an H1N1 challenge and 24 in a rhinovirus challenge Up to 92% accuracy for H1N1 and 88% for rhinovirus; severity classification 24 hours before symptoms reached 90% and 89%, respectively Small, selected cohorts with known inoculation timing and dose; cross-validated challenge results, not everyday-watch guarantees
Aditya et al., Nature Medicine (online 2021; volume 2022) Smartwatch heart-rate, step-count and sleep signals; real-time alerting system Among 3,318 participants, 84 had SARS-CoV-2; alerts covered 67 infected participants (80%), with presymptomatic signals appearing a median of three days before symptoms Alerts also occurred with other respiratory infections and noninfectious stress events

The LSTM COVID-19 bracelet result

The COVI-GAPP interim report is the source for the title’s LSTM connection. Its bracelet combined several physiological streams and used an LSTM to detect COVID-19-related changes as much as two days before symptom onset. Reported recall was 0.73 on training data and 0.68 on testing data. Recall means the fraction of labeled cases the model found; it does not indicate how often an alert is correct in the general public.

Because the full article details available here are limited to the indexed record, fine-grained sampling, cohort composition and threshold choices should not be inferred. Most importantly, this result concerns COVID-19 and a particular research bracelet—not influenza and not a named retail smartwatch.

The larger commercial influenza evaluation

Farooq and colleagues used FluStudy2020 data for development and the Home Testing of Respiratory Illness (HTRI) study for independent testing. Diagnostic results were available for 953 HTRI and 925 FluStudy2020 participants; activity data existed for 848 (89%) and 840 (90.8%), respectively. Data were collected from December 2019 through October 2020.

The strongest independent model combined symptoms with wearable data and reached an AUC of 0.75. A symptom-only model reached 0.74, while activity-only performance was lower. Important combined features included cough, mean resting heart rate during main sleep, fever and time in bed. The result shows that reported symptoms contributed materially; it does not demonstrate universal presymptomatic screening in healthy consumers. The authors characterize the algorithm’s influenza accuracy as moderate and warn that research-grade performance may not translate to commercial devices.

Controlled H1N1 and rhinovirus experiments

The JAMA Network Open challenge study deliberately exposed healthy adults, then monitored them with high-fidelity wrist sensors. Known exposure timing, controlled dose, close observation and robust infection labels make the modeling task cleaner than ordinary life. The reported accuracy—up to 92% for H1N1 and 88% for rhinovirus, with severity estimates 24 hours before symptoms—therefore describes those cohorts and conditions.

In a household or workplace, exposure time is unknown, mixed infections occur, sensor wear is imperfect and baseline health varies. A percentage from a challenge experiment should not be advertised as the expected accuracy of a consumer watch.

Real-time smartwatch alerts for SARS-CoV-2

The Nature Medicine system followed 3,318 people and generated alerts for 67 of the 84 participants who became infected. Presymptomatic signals appeared at a median of three days before symptoms. However, the system also alerted around other respiratory infections and events such as stress, alcohol use and travel. Its strength was early anomaly notification, not pathogen identification.

How to read the performance numbers

  • AUC summarizes how well a model ranks cases above noncases across thresholds. An AUC of 0.75 is moderate discrimination, not a 75% chance that an individual alert is correct.
  • Accuracy is the share of classifications that are correct in a particular cohort and class balance. It can look high when a study population is unusually selected.
  • Recall (sensitivity) is the share of known cases detected. It says nothing by itself about false alarms.
  • Alert coverage, such as the fraction of infected participants receiving an alert, is different from specificity and positive predictive value.

Always ask what was labeled as infection, how many people were studied, whether testing was independent, what sensors were used and whether symptoms were supplied to the model.

Using a wearable responsibly as an early-warning signal

Choose it for reliable data collection, not diagnosis

For personal monitoring, look for a fitness tracker with heart-rate and sleep tracking or a smartwatch that records heart rate, sleep and activity consistently. The studies do not validate any particular retail model as an infection detector. Prioritize comfortable wear, a secure fit, regular charging and clear export or privacy controls over marketing claims about medical prediction.

Build a personal baseline

  1. Wear the device during normal sleep and daily activity for several weeks before interpreting deviations.
  2. Keep the fit and wrist position consistent; remove periods with obvious sensor gaps.
  3. Record confounders such as hard training, alcohol, travel, unusually short sleep, vaccination, medication changes and menstrual-cycle phase when relevant.
  4. Review trends over multiple nights rather than reacting to one high heart-rate reading.

Act on a change without overinterpreting it

  1. Check how the measurement was obtained and whether the device reports a data-quality warning.
  2. Take your temperature with a clinical thermometer if fever is possible, and note cough, sore throat, congestion, fatigue, breathing difficulty or gastrointestinal symptoms.
  3. Follow local guidance for an appropriate influenza, COVID-19 or other respiratory test when symptoms or exposure warrant it.
  4. Limit close contact and improve ventilation if you may be contagious, while avoiding claims that the watch has identified a specific pathogen.
  5. Seek urgent medical help for severe breathing trouble, chest pain, confusion, blue or gray lips, fainting, dehydration or rapidly worsening symptoms. People at higher risk should contact a clinician early because some treatments are time-sensitive.

Why false alerts and missed infections happen

  • Noninfectious stress: travel, alcohol, anxiety and strenuous exercise can alter heart rate, sleep and activity in the same direction as illness.
  • Sensor limitations: loose fit, skin contact, motion and missing nights reduce signal quality.
  • Population shift: a model trained on selected adults or people with influenza-like illness may not fit children, older adults, pregnant people or those with chronic disease.
  • Label limitations: a symptom report is not equivalent to a laboratory-confirmed pathogen, while a negative test early in illness can also miss infection.
  • Model drift: devices, firmware, behavior and circulating viruses change over time. Performance requires fresh, independent evaluation.

A missed alert does not rule out infection, and an alert should not trigger medication or isolation decisions by itself. Use it as one piece of evidence alongside symptoms, exposure history, validated tests and clinical advice.

Privacy and data-quality questions to ask

Wearable services may process continuous physiological data and symptom questionnaires. Before enabling an infection feature, check what is stored, whether data leaves the device, retention periods, deletion controls, research consent and whether your employer or insurer can access it. Disable sharing that is not necessary for the feature.

For anyone building a personal experiment, keep a dated log of wear time, missing data, symptoms, test results and confounders. This will not turn a watch into a diagnostic instrument, but it prevents selective interpretation of isolated readings.

What would make future LSTM systems more trustworthy?

Useful validation would require large, diverse prospective cohorts, laboratory-confirmed outcomes, independent testing on different devices and seasons, transparent thresholds, calibration, false-alarm rates and subgroup analysis. Researchers also need to report whether the model uses symptoms, how much lead time is measured, and how often users actually wear the sensor.

LSTM is one modeling option, not evidence of superiority. The influenza study’s XGBoost approach and the smartwatch anomaly system answer different operational questions. Better algorithms cannot compensate for weak labels, missing data or a population unlike the people who will use the product.

Bottom line for readers

Wrist sensors can provide an early nudge that your physiology has changed. The LSTM bracelet study found a COVID-19 signal up to two days before symptoms, while commercial influenza monitoring showed moderate AUC and depended substantially on reported symptoms. Controlled H1N1 and rhinovirus experiments produced higher accuracy under conditions that ordinary smartwatch users do not share. Treat every alert as a reason to check how you feel and consider testing—not as proof of flu, COVID-19 or any other infection.

Frequently Asked Questions

Can my smartwatch tell me that I have the flu?

No. It may detect an unusual pattern in heart rate, sleep, activity or related signals, but the evidence does not establish any consumer watch as a flu diagnostic test. Confirm suspected influenza with appropriate testing and medical guidance.

Is LSTM better than XGBoost for infection detection?

The available studies do not support that conclusion. LSTM was used in a specific COVID-19 bracelet study, whereas the commercial influenza cohort used XGBoost. Different pathogens, sensors, inputs and outcomes prevent a direct superiority claim.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How early can a wearable warn about illness?

Reported lead times vary by study: the COVID-19 bracelet analysis examined up to two days before symptoms, and a separate smartwatch alerting study reported a median three-day presymptomatic signal. These are cohort results, not guaranteed personal warning windows.

What should I do after receiving an unusual wearable alert?

Check for noninfectious explanations, review symptoms and exposure, measure temperature with a clinical thermometer, and follow local testing or clinical advice. Seek urgent care for severe breathing difficulty, chest pain, confusion, fainting or rapidly worsening illness.
