The work ranges from a sensitive laboratory assay to small feasibility studies and a newer microfluidic assay comparison—not a validated consumer screening product.
What the “spit test” research actually measures
The central example is C-reactive protein (CRP), a marker of inflammation. In 2005, Christodoulides and colleagues used a microchip assay to measure CRP in human saliva. The study reported sensitivity of 10 pg/mL when saliva was diluted 1,000-fold. That is a result about the assay’s ability to measure a biomarker under the study’s conditions; it is not a threshold for diagnosing heart disease.

The authors discussed potential relevance to chronic inflammatory conditions, including cardiovascular and periodontal disease. They also reported higher salivary CRP in people with chronic periodontal disease. Such an association does not show that a person with an elevated saliva CRP has heart disease: inflammation can have more than one cause, and the study did not establish a stand-alone cardiac diagnosis from saliva.
What later studies found—and what they did not
Acute heart attack feasibility study
A 2009 feasibility study measured 21 proteins in serum and unstimulated whole saliva from 41 people with acute myocardial infarction and 43 apparently healthy controls. In that study’s sample and analysis, a salivary panel of CRP, myoglobin, and myeloperoxidase had an area under the curve (AUC) of 0.85. Combining the panel with an electrocardiogram (ECG) yielded an AUC of 0.96.

These are preliminary findings from a particular study population, not a demonstrated accuracy rate for general screening or an at-home test. The higher AUC reported for the panel with ECG also illustrates that the finding involved a combination with a clinical test, rather than saliva alone. Its results should not be treated as interchangeable with the 2005 assay’s sensitivity figure; the studies measured different things in different settings.
Rapid CRP assay study
A 2011 study evaluated a rapid salivary CRP assay in 55 healthy volunteers and 28 cardiac patients. It reported higher mean salivary CRP in the cardiac group and a correlation between paired serum and saliva results. The authors proposed further investigation for screening. Group differences and correlation in this exploratory study do not establish that saliva results can reliably diagnose an individual patient.

Microfluidic assay work reported in 2024
A 2024 paper described an integrated microfluidic assay for salivary CRP, interleukin-6 (IL-6), and procalcitonin. For real saliva samples, the authors reported R² = 0.952 when comparing results with standard ELISA. This indicates agreement in that assay comparison; it does not establish clinical performance for detecting heart disease, regulatory authorization, or availability as a consumer product. The paper presented household cardiovascular screening as an opportunity, not as an established use.

Why measuring a biomarker is not the same as diagnosing heart disease
A diagnostic test has to do more than detect a molecule. It needs to be evaluated for the clinical decision it is intended to support, in the relevant patient population, against an appropriate reference standard. The studies described here cover technical measurement, comparisons between sample types or assays, and early feasibility work. They do not establish a saliva-only test’s ability to rule in or rule out cardiovascular disease in routine care.

- CRP is an inflammation marker. A saliva CRP result by itself does not identify the cause of inflammation or prove heart disease.
- Study performance is not consumer-test performance. The reported AUCs came from a small feasibility study with defined groups, not a general-population screening program.
- Assay agreement is not clinical validation. Agreement with ELISA shows how measurements compared in the reported samples; it does not establish that the assay improves diagnosis or outcomes.
- A device under development is not a finished product. A 2005 Sandia Lab News report described a portable diagnostic device in development and said the team had tested healthy saliva samples for gum disease, with tests on diseased samples expected to follow. That report does not document a completed heart-disease test.
What to do if you are worried about your heart
Do not use a saliva biomarker result—or the promise of a future saliva test—to delay medical assessment. If you think you may be having a heart attack, seek emergency medical care immediately. For non-emergency concerns about cardiovascular risk, ask a clinician which established assessment is appropriate for your symptoms and health history. The studies summarized here do not provide a home result that can replace that assessment.

The cited studies describe research assays and feasibility work. They do not establish that a clinically authorized, commercially available saliva-based heart-disease test exists. A proposed use in a paper or a device described as under development should not be mistaken for a product that consumers can use to diagnose a cardiac condition.
