Sometimes—but only for specific gene–medicine combinations. Pharmacogenetic testing looks for inherited genetic differences that can affect how the body processes or responds to particular medicines. When a result is covered by reliable evidence and a clinical guideline, it may help a prescriber decide about a medicine, dose, or monitoring. It cannot identify one universally “best” medicine or replace a clinician’s assessment.
What a pharmacogenetic test tells you
Pharmacogenetics studies how inherited genetic differences may affect a medicine’s metabolism, transport, or effects. A test reports results for particular genes or variants; a clinician then considers whether those results are meaningful for the medicine being considered.

The practical question is not simply whether a DNA result is associated with a drug response. It is whether that specific result has a sufficiently established interpretation and a supported action for that particular medicine. The Clinical Pharmacogenetics Implementation Consortium (CPIC) publishes evidence-based, peer-reviewed guidance on how to use available genetic results in drug therapy. Its guidelines address how results may inform treatment—not whether every patient should be tested. CPIC says its guideline resources are freely available.

Actionability is specific to the gene, the medicine, and often the clinical indication. Depending on the evidence, guidance may support a change in medicine selection, starting dose, titration, or monitoring—or it may support no genetic adjustment. A gene–drug association alone does not prove that changing treatment on the basis of a test improves outcomes.

For selected serotonin reuptake inhibitor antidepressants, CPIC’s 2023 guideline includes recommendations involving CYP2D6, CYP2C19, and CYP2B6. It does not provide clinical recommendations based on HTR2A or SLC6A4 because evidence for clinical validity or utility is mixed or insufficient. The guideline also calls for consideration of other patient characteristics and drug interactions.
Clopidogrel is another example, but its CYP2C19 guidance is tied to the clinical indications covered by CPIC’s 2022 update; it should not be generalized to every patient or medicine. For G6PD-related medication decisions, CPIC’s 2022 guideline discusses genotype-based considerations while noting that genetic results may not be enough to resolve every clinical question.

When genetic findings do not justify a change
A measurable association is not automatically a reason to alter treatment. CPIC’s 2024 CYP2B6–methadone guideline describes associations with some methadone pharmacokinetic measures, including S-methadone levels, but finds insufficient evidence to change methadone prescribing or ECG monitoring based on CYP2B6 genotype. For the groups addressed, the guideline supports standard dosing, titration, and monitoring rather than a genotype-based adjustment.

Why a “negative” or broad-panel result may not settle the question
Genetic tests differ in what they examine. A targeted genotyping assay checks specified variants and may miss rare or novel variants outside its coverage. CPIC’s 2022 clopidogrel guideline cautions that CYP2C19 results can be incomplete and that clinicians should know which variants the assay included. The result is therefore best interpreted alongside the test’s stated coverage, not as proof that every relevant variant has been ruled out.

For G6PD-related decisions, some tests cover only common alleles, so a negative genetic result may not rule out deficiency. Depending on the genotype and clinical question, enzyme activity testing may also be needed. A prescriber can determine whether the test used is adequate for the decision at hand.
What else affects how a medicine works?
Genes are one part of prescribing. Drug interactions can alter the relationship between a genotype and predicted enzyme activity. Age, kidney and liver function, diet, substance use, health conditions, previous medication response, and tolerability may also affect the choice or dose. A genetic report does not account for all of these factors on its own.

Questions to ask before acting on a result
- Does this result apply to the specific medicine and clinical situation being considered?
- Which variants did the test examine, and could its coverage have missed relevant variation?
- Does current clinical guidance recommend an action for this gene–medicine combination, or is the evidence insufficient?
- Could my other medicines, health conditions, organ function, or past response change how the result should be interpreted?
- Would additional testing, such as an enzyme activity test, be relevant to this decision?
Discuss results with a prescriber or pharmacist before making medication decisions. CPIC cautions that its information is not intended for direct diagnostic use or medical decision-making without review by a healthcare professional. Do not start, stop, or change a medicine based solely on a genetic test result.
