Yes—sometimes. In selected childhood leukemias, researchers have found the same leukemia-associated DNA sequence in a newborn blood spot collected at birth and in the child’s leukemia years later. That shows a cancer-associated cell clone was already present before birth. It does not show that a fully developed, symptomatic cancer or tumor existed at birth, and it does not mean that all cancers begin prenatally.
What “started before birth” actually means
When scientists discuss a cancer that began before birth, they usually mean that an initiating genetic change occurred in a fetal cell or that a preleukemic clone—an expanding population of cells carrying a cancer-associated change—was already present in utero. This is different from saying the child had clinically recognizable cancer at birth.

Leukemia can develop through several steps. A founding alteration may create a preleukemic clone, while later genetic events or other influences are needed for that clone to become overt leukemia. The first change can therefore occur before birth, followed by a symptom-free interval lasting months or years before diagnosis.

The clearest evidence comes from archived newborn blood spots
Many newborns have a small blood sample collected shortly after delivery for screening. In research, some of these specimens—often called Guthrie cards or neonatal blood spots—remain archived. After a child is diagnosed with leukemia, investigators can identify a molecular marker in the leukemia and then look retrospectively for that same marker in the stored birth sample.
A matching sequence supports the conclusion that the leukemia-associated clone existed by the time of birth. The approach is retrospective: the later leukemia diagnosis reveals which patient-specific sequence researchers need to search for in the earlier specimen.
The 1997 MLL-AF4 study
A primary study published in 1997 examined children with acute lymphoblastic leukemia (ALL) carrying an MLL-AF4 fusion. Using polymerase chain reaction (PCR)-based testing, the researchers detected patient-specific MLL-AF4 fusion sequences in newborn blood samples from children who were diagnosed between 5 months and 2 years of age.
Because the identical leukemia-associated sequence was present in a sample collected at birth, the study provided direct evidence that the initiating leukemia clone arose prenatally in those cases. It did not establish that every child with ALL has a prenatal origin, nor that the complete leukemia was already clinically present on the day of birth.
The 2002 t(12;21) study
A 2002 study looked at nine children with B-precursor ALL carrying the t(12;21) chromosomal translocation. Clone-specific fusion sequences were found in the birth-card DNA of three children. Their leukemia diagnoses occurred at 2 years 11 months, 3 years 4 months, and 5 years 8 months.

Those findings show how a prenatal clone can persist for years before additional changes lead to diagnosed disease. The small sample and the specific leukemia subtype limit the conclusion: the result supports prenatal initiation in those three cases, not a universal rule for all ALL or all childhood cancers.
| Study evidence | Marker sought in the birth sample | What was found | Age at later diagnosis | What it supports |
|---|---|---|---|---|
| 1997 primary study | Patient-specific MLL-AF4 fusion sequence | The sequence was detectable in archived newborn samples from children later diagnosed with ALL | 5 months to 2 years | Prenatal presence of a leukemia-associated clone in those cases |
| 2002 study | Clone-specific sequence associated with t(12;21) in B-precursor ALL | Three of nine children had the sequence in birth-card DNA | 2 years 11 months; 3 years 4 months; 5 years 8 months | Prenatal initiation in the three children with matching birth samples |
How researchers detect the earlier clone
The laboratory logic is straightforward, although it is not a general-purpose screening test.

- Identify the leukemia marker after diagnosis. Testing of the child’s leukemia reveals a fusion or other sequence that is specific enough to track.
- Retrieve the archived newborn specimen. Researchers locate the blood spot collected around birth and stored in an archive.
- Extract DNA from the spot. The available DNA is analyzed for the same molecular sequence found in the leukemia.
- Use a targeted PCR assay. PCR amplifies the selected fusion sequence so that investigators can determine whether it is detectable in the neonatal sample.
- Compare the sequences. A matching, patient-specific sequence links the later leukemia clone to cells present at birth.
This strategy is powerful because it tests the person’s own earlier biological sample. It is not the same as predicting cancer in a healthy newborn from a broad panel of possible mutations.
Three kinds of evidence—and what each one proves
| Evidence type | What it can show | What it cannot establish by itself |
|---|---|---|
| Direct retrospective detection in an archived birth sample | A leukemia-associated clone or sequence was present by birth in the individual tested | That a fully developed tumor, symptoms, or clinically diagnosable cancer existed at birth |
| Developmental or embryonal-origin evidence | Some childhood cancers may begin through genetic events during fetal development and then acquire further changes | That every pediatric cancer, every leukemia subtype, or any adult cancer has a prenatal origin |
| Embryonic mosaicism | A mutation arose early enough in development to occur in only some cells and may alter later cancer susceptibility | That the person’s eventual tumor was already present at birth or descended directly from the detected mosaic mutation |
Why a prenatal clone may not cause cancer until years later
Reviews of childhood ALL describe a multistep model. A first genetic event in a developing blood-forming cell can create a preleukemic population. That population may remain small or inactive for a considerable period. A later mutation, an alteration in cell regulation, or another biological influence may then allow the cells to expand into clinically apparent leukemia.
This model explains why a child can have a positive birth-card result but a normal early childhood. The birth sample records the presence of the founding lineage; it does not measure how large that clone was, whether it was growing, or whether it would inevitably progress. A prenatal initiation is therefore a statement about timing of origin, not a prediction that leukemia was unavoidable.

Embryonic mosaic mutations are related but not the same claim
Some cancer-related mutations arise very early in embryonic development. Because the mutation occurs before all tissues have finished forming, it may be present in only a subset of cells—a pattern called mosaicism. Depending on which tissues carry the change, mosaicism can alter a person’s susceptibility to later cancer.

The investigators estimated that the frequency in that cohort was around 1 in 1,000, while noting that the true frequency could be higher.
That figure is not the percentage of all cancers that began before birth. The study concerned selected susceptibility genes in a clinical cohort. A mosaic mutation can increase risk without being the mutation that created a particular person’s later tumor, and it does not prove that a tumor was present at birth. This distinction is important when interpreting genetic testing or counseling results.
Which cancers can be traced back to birth?
The strongest direct examples in the evidence are specific childhood leukemia subtypes, especially cases in which a distinctive fusion sequence can be sought in an archived blood spot. The MLL-AF4 and t(12;21) findings demonstrate prenatal initiation in selected ALL cases.
Researchers and reviews discuss developmental origins in other childhood cancers, but the existence of a plausible fetal origin is not equivalent to direct proof from a newborn sample. Evidence cannot automatically be extended from the studied leukemias to brain tumors, sarcomas, lymphomas, adult cancers, or cancer in general. Each disease and molecular subtype requires its own evidence.

Is there a routine test that tells whether someone’s cancer began before birth?
No routine consumer or population-screening test has been established by these studies. The blood-spot analyses were retrospective research methods performed after a leukemia diagnosis, using a marker already identified in that person’s cancer. The cited work does not provide general sensitivity, specificity, or performance data for screening healthy newborns or adults.
A test can only search a sample for a sequence that the laboratory knows to target. If no archived newborn specimen exists, or if the cancer lacks a suitable patient-specific marker, this particular approach may not be possible. A negative result in an old sample would not automatically prove that the cancer began later; it could reflect the limits of the marker, the specimen, or the assay.
Likewise, finding a cancer-associated mutation in blood or another tissue does not by itself date the origin of a tumor. Clinical geneticists must determine whether a finding represents a tumor mutation, an inherited variant, or mosaicism, and then interpret it in the context of the person’s diagnosis and family history.
What a prenatal-origin finding means for patients
For an individual, a matching sequence in a birth sample would answer a historical question: the leukemia-associated clone was present by birth. It would not, by itself, identify a preventable cause, show that a parent caused the cancer, or predict how the disease will respond to treatment.

In clinical practice, treatment decisions are based on the current leukemia’s subtype, genetic features, extent, response, and the patient’s overall condition. Developmental-origin information may be valuable for understanding disease biology and for research, but the studies described here do not establish a separate treatment pathway based solely on prenatal initiation.
Best Value
When mosaic or inherited susceptibility is suspected, clinical genetics or genetic counseling can help distinguish risk from proof of tumor origin. That interpretation may affect monitoring or family counseling, but it is a different question from whether a specific cancer clone was detectable at birth.
Common misunderstandings to avoid
- “A prenatal mutation means cancer was present at birth.” A mutation or preleukemic clone may precede a diagnosable cancer by years.
- “If childhood ALL can start before birth, all ALL does.” The direct studies involve selected subtypes and small groups, so they do not establish a universal rule.
- “A newborn blood spot can routinely predict future cancer.” The published approach is retrospective and marker-specific, not validated population screening.
- “Mosaicism proves the tumor began in the embryo.” Mosaicism can indicate altered susceptibility without proving that the later tumor descended from the detected change.
- “A negative archived sample rules out prenatal origin.” Failure to detect a sequence may reflect the biology of the cancer or the limits of the available assay and specimen.
How strong is the conclusion?
The conclusion is strong for the individual cases in which a leukemia-specific sequence is found in both the later leukemia and the archived birth sample. In those cases, the clone existed by birth. The conclusion becomes less direct when evidence comes from developmental biology or from a susceptibility mutation rather than from the person’s own tumor-associated sequence.
The studies also have important scope limits. The 1997 and 2002 investigations focused on particular molecular forms of childhood ALL, and the 2002 group included only nine children. The NCI mosaicism estimate came from a selected cancer cohort and a defined set of susceptibility genes. None of these findings supplies a universal answer for every cancer or every patient.
The most accurate summary is therefore precise rather than sweeping: some childhood leukemia clones can be shown, after diagnosis, to have been present before birth; a prenatal clone is not the same as a fully developed cancer at birth; and embryonic mosaicism may influence risk without proving prenatal tumor origin.
