Broken Wrist vs. Sprained Wrist: Symptoms and When to Seek Care

A broken wrist and a sprained wrist can both cause pain, swelling, bruising, tenderness, and difficulty moving or gripping. You cannot reliably tell them apart from symptoms alone. If a fracture is possible, arrange prompt medical assessment; numbness or tingling, an open injury, heavy bleeding, an obvious deformity, or changes in hand color or circulation need emergency care.

What is the difference between a broken wrist and a sprain?

A wrist fracture is a crack or break in one or more wrist bones. It may involve the radius near the wrist or one of the small carpal bones. A fall onto an outstretched hand is a common cause, and a distal radius fracture is among the most common wrist fractures, according to Mayo Clinic.

A sprain is an injury to the ligaments or joint capsule that support the wrist. Falls can cause either injury. The tissue involved differs, but the symptoms may not: both injuries can cause pain, swelling, bruising, tenderness, stiffness, and reduced movement.

Symptoms: why you cannot diagnose it at home

A fracture may cause significant pain, tenderness, swelling, bruising, stiffness, or difficulty moving the fingers or thumb. A sprain may hurt more when you move the wrist, grip, or twist. But these patterns are not reliable tests. Being able to move the wrist, having little bruising, or seeing no deformity does not rule out a fracture.

The NHS cautions that “It can be hard to tell if a wrist is broken, dislocated or badly sprained.” An examination and often an X-ray are needed to check for a break. Read the NHS broken arm or wrist guidance and Mayo Clinic’s wrist sprain overview for more on assessment.

Arrange prompt medical assessment

Seek medical advice as soon as possible if you suspect a fracture, or if pain is severe or worsening, you cannot use the wrist, swelling or bruising is substantial or getting worse, or movement is very limited. An examination and often an X-ray can establish whether a bone is broken; do not wait for an obvious deformity to appear.

Get advice for symptoms that persist or return

For wrist pain without emergency signs or a suspected fracture, contact a GP if it interferes with normal activities, worsens or returns, or has not improved after two weeks of home care. Do not wait that long if you think the wrist may be broken or any emergency sign is present. See NHS wrist pain guidance.

What to do while waiting for assessment

These steps may help protect the arm while you arrange care; they do not diagnose or treat a fracture:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Support the arm in a towel sling and avoid unnecessary movement.
  • Apply a cold pack wrapped in a towel for up to 20 minutes every 2 to 3 hours.
  • Remove rings and watches before swelling increases, if you can do so without forcing them.
  • If there is bleeding, use a clean pad or dressing if possible. Do not press on exposed bone.

The NHS also advises not driving yourself to A&E. Follow local emergency guidance and any instructions from a clinician. These interim measures come from the NHS broken arm or wrist guidance.

How clinicians confirm the injury and choose treatment

A clinician may ask how the injury happened, examine the wrist and hand, and arrange an X-ray. X-rays can help identify a fracture and may also be used when assessing a sprain. If deeper joint structures or tendons need evaluation, MRI or ultrasound may be considered. The appropriate tests depend on the examination and the concern; see Mayo Clinic’s wrist sprain overview and Mayo Clinic’s wrist pain diagnosis and treatment guidance.

Treatment depends on what the assessment finds and how serious the injury is. A fracture may need realignment and immobilization with a cast or splint. A sprain may be protected with a splint while it heals. A splint is support, not a way to tell whether the injury is a fracture, and the right treatment plan should come from a clinician.
