Blood Biopsies for Cancer: What Liquid Biopsies Can—and Can’t—Tell You

A “blood biopsy” usually means a liquid biopsy: a lab test that looks in blood or another body fluid for cancer cells or material released by a tumor. It can help clinicians study a known cancer or, in some settings, look for signs of cancer—but it is not a universal blood test that can rule cancer in or out. What a result means depends on why the test was ordered and what it was designed to detect.

What is a liquid biopsy?

The National Cancer Institute (NCI) defines a liquid biopsy as a laboratory test of blood, urine, or another body fluid that looks for cancer cells or molecules released by tumor cells. These may include DNA, RNA, or other molecules. In blood tests, one commonly discussed target is circulating tumor DNA (ctDNA): small fragments of DNA shed by tumor cells.

A liquid biopsy does not remove a piece of tissue. It samples material circulating in a fluid, which can be collected with a blood draw and, in some cases, repeated over time. NCI notes that repeated samples may help clinicians follow molecular changes in a tumor.

What can a cancer blood test be used for?

The purpose matters: a test intended to guide treatment after a cancer diagnosis is not the same thing as a screening test for someone without symptoms. NCI describes potential liquid-biopsy uses that include helping find cancer, informing treatment planning, assessing response to treatment, and checking for recurrence. The test’s intended use and the patient’s clinical situation determine how a result should be interpreted.

Profiling a cancer that is already known

For some people with a known solid cancer, clinicians may use blood-based genomic profiling to look for tumor changes that could help guide consideration of a targeted drug or immunotherapy. The result is one part of treatment planning; it does not by itself establish that a particular medicine will work for an individual. A potentially relevant treatment may be FDA-approved for that use, used off-label, or available through a clinical trial.

In an October 2020 account, NCI described FDA approvals for specified genomic-profiling uses of Guardant360 CDx and FoundationOne Liquid CDx in people with solid cancers. Those are examples reported at that time, not a complete list or confirmation of current labeling.

Monitoring over time

Because a blood sample can be collected again, clinicians may use some liquid biopsies to look for molecular changes during or after treatment. Whether that is appropriate—and whether a result changes care—depends on the cancer, the specific test, and the clinical question.

Screening people without symptoms

Multi-cancer early detection tests are intended to look for possible signs of cancer in people who have not been diagnosed. NCI distinguishes these screening tests from liquid biopsies used to guide treatment in people with known cancer. NCI has described major unresolved questions about whether multi-cancer screening provides a net benefit, including whether it reduces cancer deaths and what harms it may cause. A positive screening result is not, by itself, a confirmed cancer diagnosis.

How does a blood biopsy compare with a tissue biopsy?

A tissue biopsy removes abnormal tissue for examination. A liquid biopsy looks for tumor-derived material in a fluid sample. They can provide different kinds of information and are not automatically interchangeable: a blood result may help answer a molecular question, while tissue sampling may be needed to examine the tumor itself or resolve an uncertain blood result.

Approach What is sampled What it may help with Important limitation
Liquid biopsy Blood or another body fluid; the test may look for tumor cells or molecules such as ctDNA Selected molecular profiling, monitoring, or other uses specified for the test A tumor may release too little detectable material into the sampled fluid; results depend on the test and clinical context
Tissue biopsy A piece of abnormal tissue removed for examination Examining tumor tissue, including molecular analysis where appropriate Obtaining tissue may be difficult or unsafe in some situations; a tissue sample represents the material collected from the sampled site

Blood testing can be useful when tissue is difficult or unsafe to obtain, but convenience does not mean the blood test will provide the same information. Tumor location, size, biology, and the amount of tumor material shed into blood affect whether a signal can be detected. Tumor-derived DNA can also be a small fraction of all DNA fragments in a sample.

What does a negative liquid-biopsy result mean?

A result that does not find a cancer-related alteration does not necessarily mean the tumor lacks that alteration or that cancer is absent. There may not have been enough tumor-derived material in the blood for the test to detect it. NCI’s 2020 discussion describes a non-detect result as potentially inconclusive and notes that tissue analysis may be considered as follow-up when appropriate.

The next step depends on the test’s purpose and the person’s clinical situation. A clinician may consider tissue testing, imaging, or another assessment; a negative result alone should not be treated as proof that cancer or an actionable tumor change is ruled out.

Can a blood test screen for every cancer?

No single claim about “a cancer blood test” applies to every test, cancer, or patient. Multi-cancer tests are a distinct screening use, and NCI says key questions about benefits and harms remain. Potential harms include false-positive results that lead to follow-up procedures, as well as overdiagnosis or overtreatment. A screening result needs appropriate clinical evaluation rather than being treated as a diagnosis on its own.

A specific colorectal-screening example

NCI reported in July 2024 that the FDA had approved Shield as a blood test for primary colorectal cancer screening in people at average risk. That dated example is specific to the named test, the colorectal screening purpose, and the described population. It does not establish that blood tests screen for every cancer or that the test is appropriate for every person. Regulatory labeling and screening recommendations can change.

What to ask a clinician about a liquid biopsy

  • What question is this test meant to answer? Clarify whether it is being used for screening, diagnosis, profiling a known cancer, or monitoring.
  • What can this specific result establish? Ask which markers or material the test examines and how the result could affect care.
  • What would happen after a positive, negative, or inconclusive result? Depending on the situation, follow-up might involve tissue testing, imaging, colonoscopy, or another clinical assessment.
  • Is the test’s intended use relevant to my situation? A test authorized or studied for one population or purpose should not automatically be assumed to apply to another.
  • What are the possible burdens and harms? Discuss false results, downstream procedures, overdiagnosis, and any practical or financial implications that apply to the specific test.
