Biological vs. Chemical Indicators for Steam Sterilization: What Each One Verifies

Biological indicators (BIs) and chemical indicators (CIs) are complementary, not interchangeable. A BI uses a standardized population of highly resistant spores to challenge the sterilization process and determine whether those spores were inactivated. A CI changes when it experiences specified sterilization conditions such as time, temperature, steam exposure, or a combination of parameters. An external CI shows that a package went through a process; an internal CI helps assess whether the package interior received the required conditions. A CI does not prove that microorganisms were killed, and even a negative BI does not prove that every item in a load is sterile.

Safe steam-sterilization monitoring combines mechanical readings, chemical indicators, and biological indicators. The Centers for Disease Control and Prevention (CDC) recommends mechanical and internal/external chemical monitoring for every load, biological monitoring at least weekly, and a BI in every load containing implantable items, with quarantine whenever possible until the BI is negative.

What each indicator actually verifies

The fastest way to distinguish the two is to ask what is being measured.

Question Biological indicator Chemical indicator
What responds to the cycle? A standardized population of resistant spores, typically Geobacillus stearothermophilus for steam. A chemical system designed to respond to one or more specified process parameters.
What does a satisfactory result mean? The biological challenge was inactivated under the test conditions. The indicator experienced its labeled process conditions; an internal CI also provides evidence that sterilant reached the package interior.
When is the result available? After processing and incubation according to the product instructions; rapid-readout systems are available for some products. Usually immediately after the cycle.
Can it prove that every item is sterile? No. A negative BI does not prove that every item in the load was sterile or adequately exposed. No. A CI does not establish microbial kill and cannot replace a BI.
Routine CDC role At least weekly; every load containing implantable items, with quarantine whenever possible until a negative result. Mechanical and internal/external chemical monitoring for each load, using an internal indicator in each package where applicable.

CDC describes BIs as “the only process indicators that directly monitor the lethality of a given sterilization process.” That directness is why a BI is the strongest biological check, but it is still one part of a monitoring program rather than a certificate that every object in a load is sterile.

CDC also notes that CIs are convenient and inexpensive and indicate exposure to the sterilization process. Their result is valuable because it is immediate and package-specific, but exposure is not the same as destruction of all microorganisms.

How the three monitoring layers fit together

Steam sterilization is monitored in three complementary ways. Each layer answers a different question, and a satisfactory result in one layer does not erase a failure in another.

Mechanical monitoring: did the machine run as programmed?

The sterilizer records or displays physical conditions such as cycle time, temperature, and pressure. The operator reviews those readings for the selected cycle and load. A deviation can reveal a cycle interruption, incorrect setting, equipment problem, or other condition that requires the load to be held for investigation.

Chemical monitoring: did the process reach the indicator where it was placed?

An external indicator, such as indicator tape or package marking, identifies a package that has been processed. An internal CI is placed inside the package and gives immediate feedback about conditions at that location, including whether steam and the required process conditions reached the package interior. CDC dental guidance emphasizes an internal chemical indicator in each package. If the internal CI is visible, CDC’s listed recommendation does not require an additional external indicator for that package.

Biological monitoring: did a resistant biological challenge survive?

A BI contains spores selected because they are resistant to the relevant sterilization process. For steam, CDC identifies Geobacillus stearothermophilus. The BI is processed in the sterilizer and then incubated or read using the method specified by its manufacturer. A negative result means the test spores were inactivated under those test conditions; a positive result means viable spores were detected and the sterilizer or its use must be investigated.

Biological indicators in steam sterilization

What a steam BI contains

A commercial steam BI contains a defined spore population on a carrier or in a self-contained system. It must be cleared for the intended use and labeled for the sterilizer type and cycle parameters. The resistant organism is a challenge: if the process is inadequate, the spores may survive even when a less resistant contaminant would not.

What a negative BI tells you

A negative BI indicates that the spores in that test device were inactivated under the conditions the device experienced. This is direct evidence about biological lethality at the test location. The result should be interpreted alongside the mechanical record, chemical indicators, load configuration, packaging, and the sterilizer’s instructions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A negative BI is not a guarantee for every item in the load. CDC’s glossary specifically cautions that it does not prove that all items were sterile or that all items were exposed to adequate sterilization conditions. A BI can be placed in a challenge location, while other packages may have different air-removal, steam-penetration, or loading conditions.

When the BI result becomes available

Read the device only by the incubation, readout, and time limits in its labeling. Some systems provide rapid readouts, while others require a longer incubation period. Do not release or interpret a BI outside the manufacturer’s stated procedure, and do not substitute a BI intended for another sterilizer type or cycle.

How often to use BIs

CDC recommends a biological indicator at least weekly, using an FDA-cleared commercial spore preparation intended for the sterilizer and cycle. Use a BI for every load containing implantable items and quarantine those items whenever possible until the BI is negative. These are monitoring recommendations, not measurements of a sterilizer’s failure rate or a claim that one indicator type is more accurate in every situation.

Chemical indicators in steam sterilization

External indicators

An external CI changes appearance after exposure to a defined sterilization process. It helps staff distinguish processed from unprocessed packages and provides a quick visual check before an item is opened. It does not show that the package interior reached the required conditions and does not demonstrate microbial kill.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Internal indicators

An internal CI is placed inside each package or tray so its response reflects conditions at the point where instruments are stored. A satisfactory internal response supports the conclusion that the labeled conditions reached that location. An unsatisfactory response means the package must not be used; investigate the cycle, packaging, loading, and indicator before reprocessing.

Single-parameter and multiparameter response

Different CIs are designed to respond to one parameter or to a combination of parameters such as time, temperature, and steam exposure. The exact response requirements, endpoint, and interpretation are product-specific. Read the indicator’s instructions rather than treating a color change as a universal proof of sterilization.

What a CI cannot tell you

A CI cannot establish sterility. It may show that a package was exposed or that conditions at the indicator location met the labeled endpoint, yet microorganisms could survive if the cycle, load, packaging, air removal, or sterilizer function was inadequate elsewhere. CDC therefore says CIs should be used with BIs, not instead of them.

Choosing the right monitoring combination for a load

  1. Verify the sterilizer and cycle. Confirm that the selected steam cycle, temperature, exposure time, drying phase, and load limits match the sterilizer and the items being processed.
  2. Prepare the load correctly. Use compatible packaging, avoid overloading, allow space for steam circulation, and position packages as directed by the sterilizer and packaging instructions.
  3. Place an internal CI. Put the appropriate internal indicator in each package or tray where it will represent the hardest-to-reach location. Use an external indicator when required by facility policy, packaging, or the applicable guidance; when the internal CI is visible, CDC’s listed recommendation does not require a separate external indicator.
  4. Apply mechanical monitoring. Record or review time, temperature, pressure, and any alarms or cycle interruptions.
  5. Run the BI at the required interval. Use a steam BI labeled for the exact sterilizer and cycle. Include one in every implantable-item load and quarantine those items whenever possible until the result is negative.
  6. Evaluate all results before release. A failed mechanical record, an abnormal internal or external CI, or a positive BI requires the load to be held and investigated. Do not release an item solely because another monitoring layer looks satisfactory.
  7. Document the decision. Record the cycle identification, load contents, mechanical readings, CI results, BI identification and result, operator, and any corrective action according to facility policy.

What to do when an indicator fails

External CI does not change as expected

  • Hold the package and do not use it.
  • Check that the correct indicator was used, that it was within its expiration and storage requirements, and that the package actually completed the cycle.
  • Review the mechanical record and inspect the package, seal, and load configuration.
  • Reprocess the item after the cause is resolved; do not treat another package’s satisfactory indicator as evidence that this package was processed.

Internal CI shows an inadequate response

  • Do not open the package for patient care or other use.
  • Hold the package and review the cycle record, packaging method, placement of the CI, load density, and steam-penetration conditions.
  • Follow the sterilizer, indicator, and facility instructions for reprocessing and documenting the event.

Mechanical readings are outside the required range

Consider the load unverified even if the CI changed. Check for the wrong cycle, an interrupted cycle, an alarm, incorrect loading, or equipment malfunction. Keep the affected items out of service until the deviation is resolved and the load is processed successfully.

Protocol for a positive steam BI

A positive steam BI is a specific signal that requires a controlled investigation, not an automatic assumption that every previous load must be recalled.

  1. Take the sterilizer out of service. Hold items associated with the positive result.
  2. Notify the responsible supervisor and infection-control function. Preserve the BI, mechanical record, chemical indicators, load information, and relevant package records.
  3. Check cycle settings and use. Verify that the correct cycle was selected, the load was prepared and positioned correctly, and the BI was used and interpreted as labeled.
  4. Check maintenance and steam supply. Investigate equipment condition, preventive maintenance, water or steam problems, and any recent service or change.
  5. Repeat BI testing in three consecutive cycles. Use the correct BI and follow the CDC protocol and product instructions for each test.
  6. Return the sterilizer to service only when the protocol permits. If all repeat indicators are negative and the investigation finds no unresolved defect, the protocol allows return to service.
  7. Escalate persistent positives. If repeat tests remain positive, treat implicated items as nonsterile, recall and reprocess them, and involve maintenance and the sterilizer manufacturer as needed.

CDC’s guidance does not make one positive steam spore test an automatic recall trigger for non-implantable objects when there is no identified sterilizer or procedural defect and the cycle settings were appropriate. Implantable items receive more conservative handling because CDC recommends quarantine whenever possible until a negative BI. Decisions should follow the current CDC protocol, facility policy, and applicable regulatory requirements.

Common misunderstandings

“The tape changed, so the instruments are sterile.”

False. External tape indicates exposure of the tape to a process; it does not verify conditions inside the pack or microbial kill.

“The internal CI passed, so a BI is unnecessary.”

False. An internal CI gives package-level process information. CDC recommends using chemical indicators with biological indicators, not replacing BIs with them.

“A negative BI proves the whole load was sterile.”

False. The BI directly challenges the process at its test location. CDC’s glossary says a negative result does not prove that every item was sterile or adequately exposed.

“A positive BI always means an immediate recall of every item processed.”

Not necessarily. Remove the sterilizer from service and follow the investigation and repeat-testing protocol. CDC distinguishes a single positive steam BI from persistent positives or an identified defect; non-implantable items do not automatically require recall solely because of one positive test when no defect or inappropriate setting is found.

“Any spore test works in any sterilizer.”

False. Select an FDA-cleared BI labeled for the sterilizer type and cycle parameters. A product intended for another sterilization process is not an appropriate substitute.

Practical selection and record-keeping checklist

  • Match every BI and CI to the sterilizer type, cycle, temperature, exposure time, and packaging system stated on its label.
  • Check expiration dates, storage conditions, package integrity, and the indicator’s instructions before use.
  • Place internal CIs where steam penetration is most difficult and where the indicator can be read without ambiguity.
  • Use an external indicator when required by the package system or facility procedure; do not treat it as a replacement for an internal CI.
  • Review mechanical readings for every load and retain them with the load record.
  • Use BIs at least weekly and in every implantable-item load, quarantining implantable items whenever possible until a negative result.
  • When any result is abnormal, hold the affected items, document the finding, identify the cause, and reprocess only after corrective action.
  • Keep the BI result, CI observations, mechanical record, cycle identifier, operator, and disposition together so an investigation can reconstruct exactly what happened.

Bottom line for infection-prevention decisions

A BI answers the biological question: did a standardized population of resistant steam spores survive? A CI answers a process-exposure question: did the indicator experience its specified conditions, and, when placed inside a package, did those conditions reach that location? Mechanical monitoring adds the machine’s time, temperature, and pressure record. None of the three alone proves that every item in a load is sterile. Using all three, interpreting each result within its limits, and following the CDC response to abnormal findings provides the defensible monitoring program for steam sterilization.
