Urea, lactic acid, and salicylic acid are all used in creams for keratosis pilaris (KP), but none is a proven cure or a universal best choice. They can help loosen the buildup of surface skin cells that contributes to rough bumps; moisturizer helps with the dryness that often accompanies KP. The best option is one you can use as directed without irritating your skin.
What is the best ingredient for keratosis pilaris?
There is no established winner for every person. The American Academy of Dermatology (AAD) lists urea, lactic acid, and salicylic acid among keratolytic options for KP. These ingredients help loosen accumulated surface cells, while moisturizers address dry skin. The evidence does not establish which ingredient works best for a particular skin type, severity, body area, or age.

For a practical comparison, consider what was actually studied, how your skin tolerates the product, and whether its formulation is suitable for regular moisturizing. A study result for one concentration and schedule does not predict what every product—or every person—will achieve.

Urea: supportive but preliminary evidence
Urea-containing creams are a dermatology-recommended option. In a 2024 open-label, noncomparative study, 30 adults aged 18–65 who completed the study used 20% urea cream once daily for four weeks. The study reported significant improvement in measured skin smoothness and texture, with no significant adverse events. Because it was small, had no comparison group, and followed participants for only four weeks, it does not show that 20% urea is right for everyone or establish how well it compares with other ingredients. Journal of Drugs in Dermatology, 2024.

Lactic acid: one direct comparison used 10%
Lactic acid is an alpha hydroxy acid and a keratolytic option for KP. In a randomized 2015 comparison, participants applied 10% lactic acid cream twice daily for three months. The study reported a mean lesion reduction of 66% from baseline. That figure describes the study’s formulation and conditions; it is not a personal forecast, and the trial does not establish long-term results or individual suitability. Kootiratrakarn, Kampirapap, and Chunhasewee, 2015.
Salicylic acid: another keratolytic option
Salicylic acid is also listed as an option by the AAD and Mayo Clinic. In the same 2015 randomized comparison, 5% salicylic acid cream was applied twice daily for three months. The study reported a mean lesion reduction of 52% from baseline. This result should not be read as proof that salicylic acid is less effective for everyone: it reflects one comparison of particular formulations under particular conditions. Kootiratrakarn, Kampirapap, and Chunhasewee, 2015.

Is urea or lactic acid better for keratosis pilaris?
The available evidence does not directly compare urea with lactic acid, so it cannot establish which is better. The 2024 urea study and the 2015 acid comparison differed in design, duration, and treatment schedule; their results should not be compared as though they came from a single head-to-head trial.

How to use KP creams without irritating your skin
- Choose one product and follow its label. Check its directions and age guidance. Do not apply more often or in greater amounts than directed; overuse of keratolytics can leave skin raw and irritated.
- Test a new product on one affected area. Mayo Clinic recommends trying a new product on a small area first. Acid-containing creams can sting or inflame skin.
- Moisturize after using a keratolytic. The AAD recommends applying moisturizer after the treatment. For routine moisturizing, it advises a thick, oil-free cream or ointment on damp skin within five minutes of bathing.
- Keep skin care gentle. Avoid vigorous scrubbing and picking. Scrubbing tends to irritate skin and can worsen KP; stacking several strong exfoliants is not a reliable shortcut.
- Pause if your skin becomes irritated or excessively dry. The AAD advises stopping for a few days if irritation or dryness develops, then following product directions before resuming. If the reaction is severe or does not settle, ask a clinician for advice.
Mayo Clinic notes that acidic creams are not recommended for young children. Follow the specific product’s age directions and seek individualized guidance when choosing treatment for a child.
Why maintenance may be necessary
KP can persist, may improve over time, and may return after treatment stops. Treatment can improve its appearance but does not cure the condition; the AAD notes that ongoing maintenance is often needed. A clinician can help determine an appropriate product strength and frequency if over-the-counter use is not working or is difficult to tolerate.

When to seek medical advice
Consider seeing a dermatologist if home treatment does not help or the bumps are bothersome. Get professional advice if bumps are painful, markedly inflamed, spreading, or if you are unsure that the rash is KP. A clinician can assess the skin and advise on suitable treatment rather than relying on appearance alone.
