A wedge-shaped cushion can be especially helpful for some people, while a doughnut cushion is not automatically the best choice. Fit depends on where you hurt, your body, and the chair or vehicle seat.
A cushion can make sitting more tolerable, but it cannot identify or treat the cause of pain. NHS guidance recommends a specially designed coccyx cushion and good posture with lower-back support. Mayo Clinic likewise says a pressure-reduction cushion may help and notes that some people find a wedge shape most helpful.

What a tailbone cushion is meant to do
Your coccyx is the small bony structure at the bottom of the spine. Sitting can concentrate load at the back of the pelvis, particularly on a hard, flat or poorly shaped seat. A coccyx cushion changes the contact pattern: it supports the sitting bones and thighs while reducing direct contact under the tailbone.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsThe cutout is important. A rear U-shaped notch or a posterior wedge removes material where the coccyx would otherwise press into the seat. The goal is pressure redistribution, not to suspend your entire pelvis in the air. If the cushion is too soft, too short, or badly positioned, your weight may still settle on the painful area.

NHS advice is to use a specially designed coccyx cushion when sitting and to maintain good posture with lower-back support. Mayo Clinic describes pressure-reduction cushions as a possible aid and says some people find a wedge-shaped cushion most helpful. Neither source presents a cushion as a diagnosis or a guaranteed cure.
Choose the cutout geometry first
U-shaped coccyx cushion
A U-shaped cushion has a notch opening toward the rear. With the open end aligned to your tailbone, the left and right sides support the pelvis while the coccyx sits over the gap. This is the most direct design for someone whose pain is centered at the tailbone and worsens when the rear of a flat seat presses upward.
Check the width and depth of the opening rather than assuming every U shape is equivalent. A narrow notch may miss the painful spot; an extremely wide notch can remove too much support and make the cushion feel unstable. The back edge should remain behind your coccyx when you sit normally, not slide forward under your thighs.

A wedge typically slopes from a higher rear section to a lower front section, often with a rear relief area. The slope can rotate the pelvis slightly forward and reduce the tendency to slump backward onto the coccyx. Mayo Clinic specifically notes that some people find a wedge-shaped cushion most helpful.
A wedge can work well at a desk when the seat height and desk height allow your knees and feet to remain stable. It may be less suitable if it makes your hips substantially higher than your knees, pushes you toward the steering wheel, or causes your feet to lose firm contact with the floor.
Doughnut or ring cushion
A doughnut cushion has a continuous ring with a central opening. It can reduce contact in the middle, but the ring may concentrate pressure on the surrounding buttocks and can feel restrictive. The opening is also often centered, whereas coccyx pain is at the rear of the pelvis. If you consider a ring design, sit on it long enough to check whether the rear rim is pressing on the painful area.

Do not choose by shape alone. The correct geometry is the one that removes pressure from your pain location without creating a new pressure point at the thighs, hips or pelvic floor.

A cushion that is too small can leave part of your pelvis unsupported; one that is too large can overhang the seat and tip. Measure the usable part of your chair or vehicle seat, including the distance between armrests or the side bolsters that limit the cushion’s width.
| Measurement | Why it matters | How to check it |
|---|---|---|
| Width | Supports both sitting bones and stays stable. | Compare cushion width with the flat portion of the seat. Leave enough clearance for armrests and side bolsters. |
| Front-to-back depth | Determines whether your thighs are supported without forcing the cushion against the seat back. | Measure from the seat back to the front edge where your legs bend. |
| Overall height | Changes hip, knee, desk and steering-wheel position. | Check your sitting height with the cushion installed, not just its thickness in a product listing. |
| Relief opening | Must sit behind the coccyx while the support areas remain under your sitting bones. | Compare the opening’s position and width with the distance from your tailbone to your sitting bones. |
OPTP documents one example, its Coccyx Pillow, at 17.5 by 14.25 inches, with height tapering from 3 inches at one end to 1.25 inches at the other. Those dimensions are a reference point, not a universal standard. Verify the current dimensions of any model and compare them with your own seat before buying.
Height is the most commonly overlooked specification. Raising yourself several inches can make a desk effectively too low, bend your wrists upward, or place your knees below the ideal angle. In a car, it can change your eye line and distance from the pedals and steering wheel. If you need a tall cushion, plan to readjust the seat and steering wheel or use it only where those adjustments are safe.
Decide how firm the foam should be
Firmness is a balance between support and pressure relief. Very soft foam may feel comfortable for a few minutes but compress until the seat pan or hard base is felt underneath. Very firm foam can preserve its shape yet create concentrated pressure at the edges of the cutout.

Pick the symptom - the matching free tool is one click away.
Signs the cushion is too soft
- Your hips sink until the relief opening no longer lines up with the tailbone.
- You feel the chair’s hard surface through the cushion.
- The front edge collapses and your thighs are unsupported.
- The cushion develops a permanent depression quickly.
Signs the cushion is too firm
- Pain shifts from the coccyx to the edges of the opening or the upper thighs.
- You cannot make small posture changes without feeling a hard ridge.
- You tense your muscles to stay balanced.
Foam type, density and construction vary widely, and listings do not always use comparable firmness scales. Look for a model that states its support material clearly and has enough structural resilience for your weight and typical sitting duration. OPTP describes its documented cushion as using premium foam; construction details for other models must be checked individually.

Look for a stable base and washable cover
A cushion is only useful if it stays in place. A non-slip underside, grippy fabric or a secure seat surface can prevent the cushion from sliding backward when you stand or forward when you move. On leather or smooth vinyl, test for movement because the same cover can behave differently than on fabric.
Inspect the cover before purchase. A zipper or other removable closure lets you wash the cover without washing the foam. OPTP documents a machine-washable mock-mesh cover. Follow the care label: washing foam when the instructions allow only spot cleaning can damage its shape or drying time.
- Prefer a cover that can be removed without stretching or tearing the fabric.
- Check whether the base has a non-slip treatment and whether it is suitable for your seat material.
- Keep a spare cover or clean towel available if you use the cushion daily.
- Do not place a loose towel over the relief opening; it can fill the gap and restore pressure under the tailbone.
Set up the cushion correctly
Position matters as much as the foam. Use this sequence each time you move the cushion to a new chair.
- Place the cushion fully on the seat with the relief opening toward the back. Do not let the rear notch hang over the edge.
- Sit down slowly and slide your hips back until your lower back can rest against the chair’s back support.
- Check that the coccyx is over the opening or lower-relief area, while both sitting bones are supported.
- Keep both feet flat or firmly supported. If your feet dangle, lower the chair or add a stable footrest rather than allowing the cushion to tilt.
- Adjust the backrest so your lower back is supported without forcing you to recline onto the tailbone.
- Take a brief standing break and reassess after several minutes. A correct setup should not create a new sharp pressure point.
Do not judge a cushion solely by the first minute of softness. Check alignment after ordinary movements such as reaching, turning and standing. If the relief opening moves away from your coccyx whenever you shift, the dimensions or grip are wrong for that seat.
Fit the cushion to common places you sit
Desk chair
Measure the chair pan and then recheck your desk height after adding the cushion. A cushion that raises you can leave your elbows below the keyboard and force your shoulders upward. Adjust the chair first; if the chair cannot go low enough, a thinner cushion or a different desk setup may be safer. Keep your monitor and keyboard positions consistent with your new sitting height.
Car seat
Use a cushion only if you can maintain full control of the vehicle. After installation, confirm that your feet reach the pedals naturally, your view is unobstructed, the seat belt lies correctly, and your hands reach the steering wheel without stretching. A cushion that slides or changes your driving position unexpectedly should not be used while driving.

Hard dining or waiting-room chair
Portability and a stable underside matter more than elaborate adjustability. A compact U-shaped cushion can be useful when the seat has a flat, firm pan, but check that the cushion does not overhang the chair and tip when you lean back.
Travel and temporary seating
Confirm the folded or carried size before buying. A cushion that fits an office chair may be too large for an airplane seat or a narrow bench. If you cannot keep the relief opening aligned, it will not provide the intended benefit. Avoid placing it where it blocks a safety restraint or creates an unstable sitting surface.
Use this buying checklist
- Purpose: The design is intended for coccyx pressure relief rather than general seat comfort.
- Geometry: The U-notch or wedge places the relief area behind your tailbone.
- Measurements: Width and depth fit the usable seat area, and height will not destabilize your legs or work position.
- Support: Foam remains supportive under your weight without bottoming out.
- Base: The underside stays put on your actual chair material.
- Cover: It removes for cleaning and the care instructions suit your routine.
- Documentation: The seller states dimensions, materials and care rather than relying only on photographs.
- Returns: You can exchange or return it if the opening, firmness or height is unsuitable.
Compare the main options at a glance
| Design | Pressure-relief approach | Potential advantages | Potential problems | Best starting point when |
|---|---|---|---|---|
| Rear U-notch | Removes contact directly behind the sitting bones. | Intuitive alignment for pain focused at the coccyx; commonly available. | Can feel unstable if the notch is too wide or the base slides. | Your pain is clearly at the rear center and you use a flat seat. |
| Wedge | Changes pelvic angle and may include a rear relief area. | Can discourage backward slumping; Mayo Clinic notes some people find it most helpful. | May raise the hips enough to disrupt desk or vehicle ergonomics. | You need a modest forward tilt and can adjust the seat around it. |
| Doughnut/ring | Uses a central opening to reduce middle contact. | May feel comfortable when pressure is broadly central. | Rear rim can press on the coccyx; ring edges may create new pressure. | The ring shape matches your body and does not load the rear rim. |
Common buying mistakes and fixes
Choosing by appearance
Two cushions can both look U-shaped while differing in opening location, height and firmness. Read the dimensional drawing and confirm which end is higher before ordering.
Ignoring the chair
A cushion cannot compensate for a seat that is too narrow, deeply bucketed or unable to support its edges. If the cushion overhangs or rocks, move it to a flatter seat or select a smaller model.
Using extra padding to fill the cutout
Adding a blanket or soft pad across the notch defeats the pressure-relief design. If the opening feels too aggressive, choose a different geometry or a smaller relief area instead.
Assuming thicker is better
More height can worsen knee, desk and steering-wheel alignment. Select the lowest height that gives adequate support and keeps your feet and working position stable.

Keeping a cushion that causes new pain
Discomfort at the thighs, hips or one side of the pelvis is a fit signal, not something to endure indefinitely. Recheck orientation, then try a different width, opening or firmness while the return period is still active.
Why the return policy matters
Pressure distribution is personal, and a cushion can feel different after an hour than after two minutes. Before purchase, read the seller’s rules for opened or used cushions, the return window, shipping responsibility and hygiene exclusions. Marketplace terms change, so confirm the policy on the listing you are actually buying from rather than relying on an old review.
Keep the packaging until you have tested the cushion in your main chair. Test it on a clean surface, retain the receipt and record the date you began using it. If the cushion is unsuitable, these steps make an exchange more likely to be straightforward.

When a cushion is not enough
A pressure-relief cushion may reduce sitting discomfort, but tailbone pain can have different causes. NHS guidance advises seeking medical help if pain has not improved after a few weeks or affects daily activities. Mayo Clinic also advises evaluation when pain persists. Arrange medical assessment sooner for severe or rapidly worsening pain, pain after a significant injury, fever, new weakness or numbness, or problems controlling your bladder or bowel.
Stop using a cushion that causes increasing pain, numbness, skin injury or loss of balance. A clinician or physical therapist can assess your symptoms and sitting posture and advise whether a cushion, activity changes or another treatment is appropriate.
A practical decision path
- Locate the pressure: If it is directly under the rear center, start by comparing a U-notch with a wedge that has rear relief. If pressure is elsewhere, do not assume a coccyx cutout is the answer.
- Measure the seat: Record usable width, depth and available clearance before choosing dimensions.
- Set the height limit: Decide how much your desk, feet or vehicle position can tolerate, then eliminate cushions that exceed it.
- Check construction: Confirm foam support, a stable underside and a removable washable cover.
- Plan a trial: Buy only with a return or exchange policy that covers a poor fit.
- Reassess alignment: Install it with the notch to the rear, verify support under both sitting bones and stop if new symptoms appear.
The Bottom Line
Start with a correctly sized U-shaped or wedge coccyx cushion, then judge it by alignment, stability and sustained comfort in your actual chair. The best model is the one that keeps direct pressure off your tailbone without disturbing your feet, posture or work and driving position; return it if it cannot do that.
