For a mild reaction limited to the sting area, move somewhere safe, remove a visible stinger promptly, wash the skin, take off nearby rings or tight jewelry, and apply a cold compress wrapped in cloth. Elevate an affected arm or leg if you can. Hydrocortisone cream or calamine may ease local itching and swelling; an oral antihistamine may help itching. Call your local emergency number now for trouble breathing or swallowing, swelling of the face or throat, faintness, or other signs of a severe allergic reaction. If the person has a prescribed epinephrine autoinjector, use it as directed and seek emergency care.
How long can swelling and itching last?
Some pain, redness, swelling, and itching around a sting are local reactions. A mild reaction may settle within hours. A stronger local reaction can grow over the first day or two before improving.

Mayo Clinic says a moderate reaction may worsen over the next day or two and last up to seven days. The American College of Allergy, Asthma & Immunology (ACAAI) describes a large local reaction as swelling that extends beyond the sting site; it says this swelling often peaks two to three days after the sting and may last a week or longer. How severe a previous sting reaction was does not reliably predict how severe a future one will be. Judge what to do by the symptoms happening now.

What to do for local swelling and itching
- Move away from the bees. Get to a safe place to avoid additional stings.
- Remove a visible stinger promptly. Scrape or brush it sideways with a fingernail or the edge of a card. NHS guidance advises against tweezers because squeezing may release more venom. NHS: Insect bites and stings.
- Wash the area with soap and water.
- Remove rings or tight jewelry near the sting before swelling increases.
- Apply a cold compress wrapped in cloth. Mayo Clinic suggests 10–20 minutes; NHS advises at least 20 minutes for swelling. Do not put ice directly on the skin.
- Raise an affected arm or leg if possible.
- Consider symptom relief. Hydrocortisone cream or calamine may help local itching and swelling. An oral antihistamine may help itching, but some products cause drowsiness. Follow the product label and ask a pharmacist or clinician if you are unsure what is suitable.
- For pain, a nonprescription pain reliever may be appropriate for some people. Check its label and consider individual contraindications.
Avoid scratching, rubbing the sting, applying heat, or putting mud or bicarbonate of soda on it. These local measures and medicines are not treatment for anaphylaxis.

Call your local emergency number immediately for possible anaphylaxis or another severe reaction. Warning signs include:

- Swelling of the face, lips, tongue, or throat.
- Fainting, or dizziness together with other symptoms.

Do not wait to see whether these symptoms improve. An antihistamine pill is not enough to treat anaphylaxis. If the person has a prescribed epinephrine autoinjector, use it as directed and get emergency care even if symptoms improve. Mayo Clinic notes that home treatment is enough for most bee stings, but that reassurance does not apply when severe or systemic symptoms are present. Ask for prompt advice based on symptoms or sting location
Contact a local medical service promptly if the sting is in the mouth or throat or near an eye, if there were multiple stings, or if symptoms are getting worse or not improving. Also seek advice for vomiting with abdominal pain, dizziness, fever with swollen glands, or a previous serious allergic reaction. The NHS gives urgent GP or NHS 111 advice for these concerns; those services and pathways are specific to the UK. Mayo Clinic highlights multiple stings in children, older adults, and people with heart or breathing problems as a reason for immediate care. Follow the pathway where you live. Get advice about possible infection or persistent symptoms
Seek clinical advice if the area appears infected, symptoms persist, or the person feels unwell. NHS suggests pharmacist advice if the sting area is hot, red, painful, swollen, or producing pus or fluid. Mayo Clinic advises arranging a healthcare visit if symptoms have not gone away within three days or if other allergic symptoms occurred. These are separate recommendations from different health systems, not one universal cutoff.

After a severe reaction, make sure a healthcare professional is involved if emergency assessment did not happen. Mayo Clinic says referral to an allergy specialist may follow. ACAAI describes venom immunotherapy as specialist preventive treatment for selected people with insect-sting allergy. A clinician can assess whether allergy testing or preventive treatment is appropriate; do not try to diagnose a venom allergy or obtain prescription epinephrine without medical advice.
