The strongest direct demonstrations use electricity generated inside the gastrointestinal (GI) tract: galvanic cells that use digestive fluid as an electrolyte, and enzymatic glucose biofuel cells (BFCs) that convert glucose and oxygen into power. A 2022 Nature Communications capsule combined a glucose BFC, glucose sensing and magnetic human-body telemetry in a 2.6 cm by 0.9 cm package.
Wireless RF or magnetic transfer, acoustic energy, externally delivered light, thermal gradients and mechanical motion are other possible sources. They are better understood as complete power-delivery architectures—not drop-in batteries—because an external transmitter, coupling losses, tissue-safety limits and very small power budgets determine whether they work. No validated consumer product currently matches this medical use case; documented systems are custom prototypes or component-level technologies.

What “battery-free” means in an ingestible sensor
Battery-free does not mean energy-free. The electronics still need a source that provides voltage and current, a way to regulate that supply, and usually a communications link. The difference is that energy is produced or delivered during operation instead of being stored in a conventional primary or rechargeable cell.

The most direct evidence for a self-powered ingestible device falls into two categories:
- GI galvanic cells: dissimilar electrodes generate current while gastric or intestinal fluid completes the electrochemical circuit.
- Enzymatic glucose biofuel cells: enzymes extract energy from glucose and oxygen in GI contents; the same electrical output can also encode glucose concentration.
RF, magnetic, acoustic, optical, thermal and mechanical harvesters can also power low-energy implantable electronics, but ingestible use requires an external coupling arrangement or a favorable physical gradient at the capsule’s exact location.
GI galvanic cells: electricity from digestive fluid
How the cell works
A galvanic cell places two dissimilar electrodes in contact with GI fluid. The fluid acts as the electrolyte, while the difference in electrode chemistry creates a voltage. Earlier ingestible demonstrations used magnesium–copper or zinc–copper couples. Magnesium–copper can provide a higher voltage; zinc–copper generally provides lower voltage but may last longer. The electrode materials are consumed or chemically altered over time, so the source is finite even though it contains no conventional battery pack.

Output depends on electrode area, material selection, pH, ionic concentration and how continuously the electrodes are wetted. A geometry optimized for the stomach should not be assumed to behave the same way in the small intestine. Dilution, food, mucus and transit time all change the local electrolyte. The capsule must also prevent unintended contact between the electrodes before deployment and must use materials compatible with the intended exposure period.
What has been demonstrated
A PLOS/PMC study published in 2017 reported an average power density of 0.23 μW per mm² of electrode area and an average operating period of 6.1 days for temperature measurements in the GI tract of pigs. Those are animal-study results, not a human performance guarantee. The figures describe the tested electrode area and device configuration; changing the area, materials, capsule position or measurement workload changes the available power and duration.
The result is important because the cell needed no external transmitter. It generated energy where the sensor operated and sustained a multi-day measurement task. It also illustrates the main trade-off: increasing electrode area can increase output, but area consumes scarce capsule volume and may increase the amount of material exposed to tissue and digestive fluid.

Where galvanic cells fit
- Best fit: low-duty-cycle measurements such as temperature, transit timing or occasional threshold detection.

Glucose biofuel cells that sense and power a capsule
Architecture of the 2022 capsule
A Nature Communications study in 2022 demonstrated a battery-free capsule using an enzymatic glucose BFC. The capsule measured 2.6 cm long × 0.9 cm in diameter. Glucose oxidase at the anode and bilirubin oxidase at the cathode were combined with mediators on carbon-nanotube-coated nickel foam. Glucose oxidation at the anode and oxygen reduction at the cathode produced the electrical output.
Recommended Free Tools
The capsule used an enteric coating that protected the device in the acidic stomach and dissolved in the small intestine. This is a functional design choice, not merely packaging: the coating controls when the electrodes contact GI contents and helps place the measurement in a region where the intended substrate and oxygen are available.
One signal provides both power and glucose information
The BFC voltage changes with glucose concentration. Instead of adding a separate powered glucose sensor, the circuit extracts glucose information from the source itself. The voltage is converted into a frequency signal, and the frequency is transmitted using magnetic human-body communication. The study describes a circuit that performs energy harvesting, biosensing and wireless telemetry through a power-to-frequency conversion scheme.
The reported communication band was 40–200 MHz. Magnetic human-body communication uses the body as part of the coupling path rather than radiating like a conventional long-range radio link. A receiver outside the body detects the frequency-coded signal while the capsule remains inside the GI tract.
What the porcine result does—and does not—show
The authors used a porcine model because pigs have anatomical and physiological similarities to the human GI tract. That makes the experiment more informative than a fluid beaker test, but it remains a pilot animal study. It demonstrates feasibility of simultaneous power generation, glucose measurement and telemetry; it does not establish human safety, a universal transit time, a therapeutic accuracy specification or commercial readiness.
Wireless RF and magnetic power transfer
RF harvesting is a system, not a component
RF energy harvesters normally include a receiving antenna, an impedance-matching network and an AC–DC rectifier. The rectified output then passes through power-management electronics before reaching the sensor and transmitter. A 2024 RF review presents this chain as an alternative for low-power implantable and wireless sensors.

For an ingestible capsule, the external transmitter must deliver enough field at the capsule’s changing depth and orientation. The receiving antenna, matching network, rectifier and regulator all occupy volume and introduce losses. Ambient RF harvesting avoids a dedicated transmitter, but the available field is weaker and depends on location and surrounding equipment. A deliberate RF link can provide predictable bursts of energy, yet it adds hardware, alignment and tissue-exposure constraints.
Magnetic coupling
Magnetic near-field transfer can use coils or other receiving structures and is generally easier to control over a short distance than ambient RF. The coupling falls as alignment and separation change, so a capsule moving through the bowel may receive power in bursts rather than continuously. Designers often pair the link with a storage capacitor or a duty-cycled workload: energy accumulates, the sensor wakes, a measurement is taken and data are sent before the supply collapses.
Human-body communication, as used by the glucose-BFC capsule, is distinct from powering the device through a large external magnetic field. In that demonstration, the BFC supplied the operating energy while the body-coupled magnetic link carried the encoded data. A future design could combine magnetic power transfer with body-coupled telemetry, but each added circuit must fit the capsule’s power and safety budget.

Acoustic and ultrasonic transfer
Ultrasound can cross tissue and focus energy on a receiver. An ingestible receiver would need an external transducer, alignment or tracking strategy, rectification and storage. The acoustic intensity, heating, cavitation risk and exposure duration require dedicated safety validation. This approach is therefore promising for controlled experiments but remains research-stage for ingestible medical products.
Optical and photovoltaic conversion
Photovoltaic receivers can convert delivered light efficiently, but light penetration through skin and abdominal tissue is limited. A capsule deep in the GI tract has no reliable line of sight to an external lamp. Optical powering is consequently more plausible for shallow implants or devices positioned close to an optical window than for free-moving intestinal capsules.
Thermal gradients
Thermoelectric harvesters need a sustained temperature difference across the device. Body temperature is relatively uniform, and the gradient between the capsule and its surroundings changes with location, meals and blood flow. A thermal design may contribute trickle power but is unlikely to provide a predictable supply unless the capsule has a deliberately engineered thermal boundary.

Mechanical motion and pressure
Peristalsis, contact with the intestinal wall and pressure changes can drive piezoelectric, triboelectric or other mechanical harvesters. Motion is intermittent and location-dependent. A device may receive useful energy during contractions and almost none while it is stationary in fluid. Mechanical harvesters are best paired with energy storage and a workload that tolerates long gaps.

Design constraints that dominate in the GI tract
Power budget and duty cycle
Start with the energy required per measurement, processing event and transmitted packet. A sensor that samples continuously and radios every reading will need far more energy than one that wakes once per hour and stores a threshold event. The source should be sized for the worst expected GI condition, not the average value measured in a favorable fluid.
Power-management electronics should include a cold-start path, voltage regulation and undervoltage protection. If the source rises slowly, the circuit must avoid repeatedly starting and collapsing. A storage capacitor can collect energy between measurements, while a brownout-safe state machine preserves calibration and prevents corrupted telemetry.
Changing chemistry and anatomy
Stomach acid, intestinal fluid, food particles and mucus present different ionic environments. Electrode output, enzyme activity and oxygen availability can all change after transit. A capsule should be characterized in the intended sequence of fluids and at realistic temperatures, then tested in an animal model that reproduces movement and residence time.
Encapsulation and controlled exposure
Encapsulation must protect electronics from fluid ingress while allowing the chosen energy source to contact the environment. A galvanic or glucose cell needs exposed or permeable electrode regions; the rest of the circuit must remain sealed. Enteric coatings can delay exposure until the small intestine, as in the 2022 glucose-BFC capsule. Coating thickness, dissolution time and mechanical integrity need batch testing because they directly affect when power becomes available.
Telemetry under a tiny energy budget
Sending data often costs more energy than sensing. Frequency-coded signaling, short bursts and local data reduction can lower the radio burden. Magnetic human-body communication is attractive when a receiver can be positioned outside the patient, but the link must be tested through tissue, at realistic capsule orientations and across the expected transit path.
Best Value
Materials and biological safety
Every electrode, enzyme, mediator, adhesive, coating and corrosion product requires a biocompatibility assessment for the intended duration. A galvanic cell that consumes magnesium, zinc or another metal introduces reaction products into the GI environment. Enzymes and mediators must remain immobilized, and the capsule must withstand mechanical forces without releasing particles. Safety validation also covers retention, obstruction, removal and electromagnetic, acoustic or thermal exposure from any external power source.
A practical workflow for choosing a battery-free architecture
- Define the clinical measurement. Specify the analyte or physical variable, accuracy, sampling interval, total transit window and whether data must be streamed or can be stored and relayed later.
- Map the target location. Decide whether the capsule must work in the stomach, small intestine, colon or several regions. Record expected pH, fluid availability, oxygen, residence time and likely orientation.
- Estimate the complete energy budget. Include sensor excitation, analog conversion, processing, memory, telemetry, voltage conversion and startup losses. Add margin for the lowest expected source output.
- Select an energy source. Choose a galvanic cell when autonomous in-body operation and multi-day low-duty sensing matter; choose a glucose BFC when glucose is present and source voltage can carry the measurement; choose wireless transfer when an external transmitter and controlled power bursts are acceptable.
- Design storage and power management. Add a capacitor or other approved storage element if harvesting is intermittent. Set startup, undervoltage and shutdown thresholds so the capsule fails safely rather than transmitting corrupted data.
- Co-design telemetry. Match the communication method to the receiver’s position and the available energy. A low-power frequency signal may be preferable to high-throughput radio, especially when the capsule only needs to report trends or alarms.
- Engineer exposure control. Use an enteric coating, selective membrane or sealed electrode window to determine when the source contacts GI contents. Verify that the coating dissolves at the intended site and that it does not peel or crack prematurely.
- Validate progressively. Test in synthetic gastric and intestinal fluids, then in benchtop motion and tissue-equivalent setups, followed by animal studies. Measure output, voltage stability, transit, telemetry reliability, material release and tissue response.
- Plan recovery and failure handling. Define what happens if the source stops, the capsule is retained, telemetry is lost or the device reaches the end of its chemical life. Clinical protocols need an independent method to locate or retrieve a nonfunctional capsule.
Common failure modes and fixes
| Observed problem | Likely cause | Engineering response |
|---|---|---|
| Power is adequate in the stomach but collapses in the intestine | Different pH, dilution, electrode wetting or oxygen availability | Characterize both regions, enlarge or redesign the active area, and add energy storage for transit gaps |
| The capsule repeatedly resets | Startup current or telemetry burst exceeds the harvested supply | Use staged startup, lower the transmission duty cycle and raise the storage threshold before wake-up |
| Glucose readings drift | Enzyme activity, mediator transport or substrate concentration changes | Calibrate across expected GI fluids and temperatures; monitor source voltage and flag out-of-range conditions |
| Wireless power is intermittent | Receiver misalignment, changing depth or tissue attenuation | Use a larger coupling margin, adaptive matching, burst operation or a storage capacitor; verify at realistic orientations |
| Telemetry is received only near the abdomen | Short-range coupling and body-position dependence | Define receiver placement in the protocol and transmit only essential data with error detection |
| Electrode output falls faster than expected | Finite reactants, passivation, fouling or reduced fluid contact | Change electrode chemistry or geometry, protect nonactive surfaces and test the full transit duration rather than a static soak |
Safety and readiness for human use
A battery-free design removes risks associated with a conventional cell, but it does not remove medical-device risk. Electrochemical sources can release reaction products; enzymes and mediators can leach; external RF, magnetic or acoustic systems can expose tissue to energy; and any capsule can be retained or obstruct the GI tract.
The porcine glucose-BFC demonstration is a feasibility milestone, not evidence that a human product is approved or clinically validated. Human translation requires material characterization, electrical and exposure testing, reliable transit and retrieval plans, manufacturing controls, and clinical studies that establish measurement performance and adverse-event rates.
For developers, the key safety question is not simply whether the source produces enough microwatts. It is whether the complete capsule—including source, coating, antenna or electrodes, electronics and telemetry—remains safe and predictable throughout the intended journey.
What is realistically available today?
There is no validated, ordinary consumer product that honestly matches the use case of a battery-free ingestible medical sensor. Generic batteries, capsule-endoscopy replacement parts and consumer NFC tags should not be represented as substitutes for a medical power system. The documented solutions are custom research prototypes, specialized energy-harvesting components or laboratory biofuel-cell and electrode assemblies.
For a laboratory project, galvanic electrodes and enzymatic BFC materials offer the most direct path to an autonomous GI proof of concept. For a system that must deliver predictable energy on demand, a deliberately designed RF or magnetic link may be more practical, provided the patient can use the external transmitter and the exposure limits are met. Acoustic, optical, thermal and mechanical methods remain highly dependent on anatomy and operating conditions.
Bottom line for selecting a source
Choose a GI galvanic cell when the device can tolerate changing chemistry and needs low-power measurements without an external transmitter. Choose an enzymatic glucose BFC when glucose is both the available fuel and the measurement target; the 2022 porcine capsule shows that one source can power sensing and encode glucose data for magnetic human-body telemetry. Choose RF or magnetic transfer when controlled external power is more valuable than complete autonomy. Treat acoustic, optical, thermal and mechanical harvesting as specialized options whose feasibility depends on location, coupling and safety validation.
