Auth0 as an Identity Broker for Epic SMART on FHIR: Architecture, Setup, and Validation

By Freedom251 Team · · 13 min read

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Short answer: In a normal Epic SMART launch, Epic is the authorization server that issues the access token for Epic FHIR APIs. Auth0 can authenticate users and authorize your application, but its documented OIDC and PKCE features do not by themselves establish a drop-in replacement for Epic’s SMART authorization server or a universal Epic-token brokerage pattern.

Use Auth0 as a separate application identity layer only when that matches your workflow, and keep the Epic authorization request, launch context, scopes, code exchange, audience, and Epic-issued token explicit. A combined Auth0/Epic design must be validated against the specific Auth0 tenant, Epic customer, selected APIs, and environment before implementation.

Start with the system boundary

Epic’s SMART authorization server controls access to Epic FHIR data in the usual EHR-launch flow. Your app is registered with Epic, sends an OAuth 2.0 authorization request to Epic, receives an authorization code, and exchanges that code at Epic’s token endpoint. The resulting token is the credential used at the intended Epic FHIR base URL. Epic’s launch documentation describes this authorization-code sequence, including the redirect, state, launch context, and resource-server audience parameter aud where required. See Epic’s launch guide.

Auth0 can perform a different job: signing users into your application, issuing tokens for an Auth0-protected API, or connecting to an external OIDC identity provider. Those capabilities do not automatically make an Auth0 token acceptable to an Epic FHIR server. Treat the two authorization legs as separate until the exact topology, token audience, exchange mechanism, and customer configuration have been verified.

What each system is responsible for

Concern Epic SMART authorization server Auth0 application identity layer
FHIR access token Issues the token used for the configured Epic FHIR APIs after the Epic code exchange. Not established by the reviewed documentation as a replacement issuer for that Epic token.
User sign-in May authenticate and authorize the EHR user as part of the SMART launch. Can authenticate users for your application and issue Auth0 tokens for an Auth0 API.
Launch context Supplies the EHR launch context and determines what contextual information is available. Must preserve the incoming context if an application login is inserted; it cannot silently recreate it.
Scopes Evaluates requested SMART scopes and returns the scopes actually granted for the user and app configuration. Evaluates scopes for its own authorization flow. Its scope names and API audience are not automatically Epic scopes.
Audience aud identifies the Epic FHIR resource server when required by that environment. An Auth0 API audience identifies an Auth0-protected API and must not be copied into an Epic request without an explicit, validated design.
Operational approval Epic customer configuration controls available APIs, launch placement, and environment-specific behavior. Tenant settings control connections, callbacks, applications, APIs, and token behavior.

Choose the workflow before configuring either platform

Write down the user journey and data path first. The answers determine whether Auth0 is necessary, where it can sit, and which authorization server owns each token.

EHR launch

An EHR launch starts from an Epic-controlled context, such as a configured launch entry in the customer’s EHR interface. Your app receives iss and launch values, discovers the relevant SMART configuration as required by the implementation, and starts authorization at Epic. The launch value is not a generic login hint; it carries context that must survive any intermediate application flow.

Standalone launch

A standalone app can begin authorization without an EHR-supplied launch value. It still needs Epic’s registered client details, redirect URI, authorization endpoint, token endpoint, selected APIs, and an appropriate scope set. Decide whether the user first signs in through Auth0, Epic, or both, and document which system ultimately authorizes FHIR access.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

User and resource requirements

List the user type, FHIR resources, actions, and identity claims required by the feature. Separate clinical data access from application identity. For example, a dashboard might need a patient context and a current-user identifier, while an administrative screen might need only application authentication and no patient data. Do not request broad scopes merely because they are available.

Prerequisites and registration records

Before coding, create an environment-specific record for each deployment. At minimum, capture:

  • Epic production and non-production client IDs, with the non-production ID reserved for the Epic sandbox.
  • The exact redirect URI registered at Epic and the exact callback URI configured in Auth0, if Auth0 is used in the application flow. Scheme, host, path, and trailing-slash differences can matter.
  • The Epic launch URL, selected APIs, supported SMART version, authorization endpoint, token endpoint, and FHIR base URL obtained from the target environment’s SMART configuration.
  • The Auth0 application, connection, callback, logout, API audience, and allowed scopes, if those components are part of the design.
  • The workflow’s minimum resource, launch-context, and identity scopes.
  • Whether the app is browser-based, a native mobile app, a confidential server application, or another client type.

Epic states that its sandbox uses OAuth 2.0 and that developers should use their non-production client ID there. Sandbox behavior is useful for protocol testing, but it does not configure a customer’s production launch entry or prove that every customer has identical settings.

Implement the Epic SMART leg step by step

  1. Accept and preserve launch inputs

    For an EHR launch, receive iss and launch at the launch URL. Validate their format and bind them to the user session or a short-lived server-side transaction. Do not put unvalidated values into a redirect or use a launch value from one session in another.

    Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  2. Discover the target SMART configuration

    Use the target Epic environment’s documented SMART discovery process to identify the authorization and token endpoints and the FHIR base URL. Keep the discovered issuer and resource server associated with the same transaction. A production customer may expose values that differ from the sandbox.

  3. Register the SMART client with Epic

    Record the Epic client ID, redirect URI, selected APIs, and launch availability. If the customer requires a MyChart launch, its IT team may need to create a custom MyChart menu item; an application cannot create that menu entry from its own code.

  4. Generate state and, where appropriate, PKCE values

    Generate a high-entropy, single-use state value and verify the exact value on return. For a PKCE-capable client, create a verifier and its S256 challenge, retain the verifier for the transaction, and send the challenge in the authorization request. Epic says its implementation has supported PKCE since the August 2019 version and recommends PKCE for native mobile app integrations. Read the Epic OAuth 2.0 tutorial for the versioned implementation statement.

  5. Build the Epic authorization request

    Use the endpoint and parameters required by the target SMART configuration. A conceptual request contains the following values; replace the placeholders with discovered, registered values rather than hard-coding an endpoint:

    Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
    response_type=code
    client_id=<epic-client-id>
    redirect_uri=<registered-redirect-uri>
    scope=<minimum-smart-scopes>
    state=<single-use-state>
    aud=<epic-fhir-resource-server-if-required>
    launch=<preserved-launch-value-for-ehr-launch>
    code_challenge=<base64url-s256-challenge>
    code_challenge_method=S256

    Whether aud is required depends on the Epic version and configuration. Treat it as the FHIR resource-server identifier, not as an Auth0 API audience.

    • What You Get: Anker USB-C Hub (7-in-1), welcome guide, 18-month warranty, and our friendly customer service.
  6. Handle the callback

    Accept the authorization code and returned state only over the registered redirect URI. Reject missing, mismatched, expired, or already-used state values. Preserve the original transaction’s issuer, launch context, verifier, and requested scopes while processing the callback.

  7. Exchange the code at Epic

    Send the code to Epic’s token endpoint with the client authentication required by the registered app and the PKCE verifier when PKCE was used. Store the response according to your threat model, record the granted scopes and contextual fields, and associate the token with the correct Epic FHIR base URL. Do not substitute a token minted for an Auth0 API.

  8. Validate before calling FHIR

    Check the token response, issuer and audience expectations, expiry, granted scopes, patient or encounter context, and the identity claims required by the feature. Then call only the authorized endpoints at the matching Epic FHIR base URL. Log identifiers and outcomes needed for diagnosis without logging bearer tokens or sensitive clinical payloads.

    Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  9. Coordinate environment promotion

    Repeat registration and validation for the customer’s non-production and production environments. Confirm client IDs, redirect URIs, selected APIs, launch placement, consent behavior, and customer-specific differences instead of assuming that sandbox values transfer unchanged.

Where Auth0 can fit—and what must be proven

There are several possible arrangements, but the reviewed documentation does not verify a universal end-to-end Auth0-as-Epic-SMART-broker configuration. Treat each arrangement below as an architecture hypothesis that needs tenant- and customer-specific validation.

Arrangement What happens Questions to validate
Epic authorization plus separate Auth0 application login The app obtains its application identity from Auth0 and separately completes the Epic SMART flow for FHIR access. How is the Auth0 user linked to the Epic user or launch transaction? Where are both sessions stored? Does the extra login preserve iss, launch, state, and the original redirect?
Auth0 as an OIDC connection to another identity provider Auth0 delegates user authentication to an OIDC provider and receives identity claims for the Auth0 transaction. Does the connection provide the required openid scope and ID-token claims? Is this identity flow independent of Epic’s FHIR authorization?
Auth0-mediated token exchange or broker A proposed design attempts to receive or exchange credentials between Auth0 and Epic. Does the exact Auth0 tenant feature support this exchange? What issuer, audience, subject, and scopes does Epic require? Will Epic accept the resulting token? Who owns refresh, revocation, consent, and audit?
Auth0 token sent directly to Epic FHIR The application sends an Auth0-issued API token to an Epic FHIR endpoint. This is not established by the cited documentation. Do not implement it without explicit, tested agreement from the target Epic environment and a token contract that names Epic as the accepting resource server.

Auth0’s documentation describes its own Authorization Code Flow with PKCE, including an Auth0 API audience and requested scopes, and its OIDC enterprise connection requires at least openid while expecting user claims in the ID token rather than calling the provider’s /userinfo endpoint. Those are Auth0 component capabilities, not proof of a combined Epic topology. See Auth0’s PKCE authorization documentation and Auth0’s OIDC connection documentation.

Scopes, identity, and launch context

The SMART App Launch v2.2 guide separates scopes into access to FHIR resources, launch context, and identity. Its definitions are useful when designing the request:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Scope category Typical purpose Design treatment
launch Requests EHR launch context. Use for an EHR launch when required by the target implementation; preserve the returned context through the flow.
openid fhirUser Requests identity information for the current user. Ask only when the feature needs the user identity and validate the resulting claims.
FHIR resource scopes Authorize access to specified resources and operations, such as patient or user-level data. Choose the narrowest resource and operation set that supports the workflow.
offline_access or online_access Indicates whether a refresh-token request or online-only access is intended. Request according to the required session lifetime and the target server’s policy; acceptance and resulting privileges remain server- and user-dependent.

The authorization server decides which requested scopes are accepted, and the user context and app configuration affect the result. Epic says that the scopes returned depend on the requested scopes and the APIs selected in the app configuration. Its November 2024 documentation states that both SMART v1 and v2 scope formatting are supported, but that version marker should be confirmed against the customer environment.

Keep three questions separate in design reviews: “Can the app identify its user?”, “What EHR context was supplied?”, and “Which FHIR resources may this token access?” An Auth0 identity claim can answer the first without answering the other two.

PKCE, state, audience, and token custody

PKCE is leg-specific

PKCE protects an authorization-code transaction by binding the returned code to a verifier. If the app performs an Auth0 PKCE flow and a separate Epic PKCE flow, each flow has its own verifier, challenge, authorization endpoint, callback transaction, and code exchange. Completing one does not complete the other.

State protects the transaction

Use a cryptographically unpredictable, single-use state value and compare it in constant-time or an equivalent safe manner. Bind it to the browser or app session, issuer, redirect target, and launch transaction. Expire it quickly and invalidate it after use. An Auth0 callback state should not be treated as the Epic callback state unless the exact combined protocol defines that relationship.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Audience names the accepting resource server

Epic’s aud parameter identifies the FHIR resource server when the target configuration requires it. Auth0’s API audience identifies an Auth0 API. Similar-looking strings do not make the tokens interchangeable; verify issuer, audience, signing keys, scopes, and the receiving server’s acceptance rules.

Keep custody and refresh behavior explicit

Document where each access token and refresh token exists, which component can use it, when it expires, and how it is revoked. Prefer a server-side token store with encryption and strict access controls when a backend must call FHIR. Avoid exposing Epic bearer tokens to browser storage unless the client architecture and threat model specifically require it. Never place access tokens in URLs, analytics events, error messages, or ordinary application logs.

Testing plan that reaches production reality

Protocol tests in the sandbox

  • Use the Epic non-production client ID and the exact registered redirect URI.
  • Run both a standalone authorization and an EHR launch if both workflows are supported.
  • Verify that state mismatch, replay, missing launch values, and expired codes are rejected.
  • Verify the PKCE S256 challenge and verifier pair, including a deliberately incorrect verifier.
  • Inspect the token response for granted scopes, expiry, identity claims, patient context, and the expected audience.
  • Call only the APIs selected in the app configuration and confirm that the Epic-issued token is accepted at the intended FHIR base URL.
  • If Auth0 is present, test the Auth0 callback separately and then test the handoff between the Auth0 session and the Epic transaction without losing launch context.

Customer non-production validation

Epic’s simulators and harnesses do not remove the need to work with an Epic customer in a non-production environment. Customer IT may need to enable APIs, configure launch availability, create a MyChart menu item, and explain environment-specific consent or authentication behavior. Epic also notes that its sandbox is configured for OAuth 2.0; another authentication mode requires customer coordination. See Epic’s OAuth 2.0 tutorial and testing guidance.

Production cutover

Promote only after the customer confirms production client registration, redirect URI, selected APIs, launch placement, consent, and monitoring contacts. Keep production and non-production credentials and callback domains separate. Re-run the authorization, token, and first-FHIR-call tests after every customer-side configuration change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Validation checklist for a proposed broker design

  • Is Epic still the documented issuer of the token used at the Epic FHIR endpoint?
  • Are the Epic client ID and redirect URI correct for this environment?
  • Does the exact Epic authorization request include the required iss, launch, state, scopes, and conditional aud values?
  • Does launch context survive every Auth0 redirect and callback?
  • Is the PKCE S256 verifier matched to the correct authorization transaction?
  • Are Auth0 and Epic audiences, issuers, scopes, and token lifetimes kept distinct?
  • Are the APIs selected in Epic’s app configuration sufficient for the requested resources?
  • Is the Epic-issued token accepted by the exact FHIR base URL the app calls?
  • Does the customer’s IT team confirm the required EHR or MyChart launch configuration?
  • Are token storage, refresh, revocation, consent, audit, and incident response assigned to named components?

Troubleshooting by symptom

Symptom Checks to perform
Auth0 login succeeds but no FHIR call is authorized Confirm that the Epic authorization request and Epic code exchange actually ran. Inspect the issuer and audience of the token presented to FHIR; an Auth0 application token is not evidence of Epic authorization.
Epic returns an invalid redirect or callback error Compare the registered and sent redirect URIs character for character, including scheme, path, port, and trailing slash. Check that the environment’s client ID is being used.
The callback is rejected for state Check session affinity, single-use storage, expiration, parallel tabs, and whether an Auth0 callback overwrote the Epic transaction state.
PKCE exchange fails Confirm that the verifier belongs to the same transaction, that the challenge used S256, and that URL-safe base64 encoding was applied correctly. Do not reuse an Auth0 verifier for Epic.
Token is issued but FHIR returns 401 Check issuer, audience, expiry, FHIR base URL, client environment, and whether the token is the Epic-issued access token intended for that server.
FHIR returns 403 Check granted scopes, selected Epic APIs, user or patient context, consent, and customer configuration. A 403 can reflect app API selection or customer setup rather than an Auth0 identity failure.
Launch works in a simulator but not from the customer’s EHR Verify the customer’s launch URL, menu configuration, iss and launch values, production client registration, and any customer-specific authentication policy.
Identity claims are missing through an OIDC connection Confirm that openid was requested and that the provider places the required claims in the ID token. Auth0’s documented OIDC connection behavior does not rely on calling the provider’s /userinfo endpoint.

What the documented evidence does—and does not—establish

The standards and vendor documentation support the individual pieces: Epic’s SMART authorization-code flow, Epic’s versioned PKCE support, SMART scope and launch-context semantics, Auth0’s own PKCE flow, and Auth0’s OIDC connection behavior. An Auth0-authored 2021 SMART on FHIR guide discusses state, audience, restrictive scopes, PKCE, and consent as implementation practices; it is useful context, but it does not independently prove that Auth0 replaces Epic’s authorization server. The guide is available at Auth0’s 2021 SMART on FHIR best-practices document.

No cited source establishes a universal broker topology, comparative success rate, implementation cost, or clinical outcome. The practical conclusion is therefore architectural: keep Epic’s SMART leg intact, use Auth0 only for a role your tenant and customer explicitly support, and prove token acceptance and launch behavior in the target environment before calling the design production-ready.
