Asthma is diagnosed from a pattern of variable respiratory symptoms plus objective evidence that exhaled airflow varies. Treatment is individualized: controller therapy reduces future symptoms and attacks, while prescribed reliever medicine treats current symptoms. A written action plan explains what to do as symptoms change. Severe or worsening breathlessness, confusion, drowsiness, a “quiet chest,” or symptoms that do not improve with the prescribed reliever require emergency help using your local emergency number.

This guide explains the usual diagnostic tests, treatment principles, action-plan steps and emergency thresholds. It is general information, not a personal diagnosis or medication plan; follow your clinician’s instructions and your own written plan.

What is asthma?

Asthma is a respiratory condition in which symptoms and expiratory airflow vary over time and in intensity. Typical symptoms are wheezing, shortness of breath, chest tightness and cough. Symptoms may appear during exercise, at night, with infections or after exposure to an irritant, then improve for a period. Other illnesses can cause the same complaints, so symptoms alone do not establish asthma.

The 2025 Global Initiative for Asthma (GINA) summary guide says a diagnosis should document both a compatible symptom history and variable expiratory airflow. A physical examination can be normal between episodes. A clinician therefore combines your history, examination and lung-function evidence rather than relying on one symptom or one test.

What are the symptoms of asthma?

  • Wheezing or a whistling sound when breathing
  • Shortness of breath or difficult breathing
  • A feeling of chest tightness
  • Cough, sometimes the main or only symptom

The diagnostic clue is variability: symptoms can come and go, differ in severity, worsen at certain times or after particular exposures, and respond to treatment. Keep a record of timing, activity, infections, possible triggers and nighttime symptoms to discuss at an appointment. Severe breathing difficulty should be treated as an urgent problem, not observed at home while waiting for a diagnosis.

How is asthma diagnosed?

Diagnosis is made by a qualified health professional. The clinician asks when symptoms occur, how long they last, what makes them better or worse, and whether there is a pattern of recurrence. They listen to your chest and consider other possible causes. The US National Heart, Lung, and Blood Institute (NHLBI) diagnosis guidance describes lung-function testing as part of this assessment. The NHS also describes questions, a chest examination, possible blood testing and home peak-flow measurements over a period of time.

A normal result on one day does not answer every clinical question. Airflow can be normal between episodes, and the appropriate next test depends on your symptoms, age, current treatment and local clinical practice. Do not start, stop or change inhalers to “test” yourself; ask the clinician arranging the assessment how medicines should be handled before testing.

Spirometry

Spirometry measures how much air you can breathe out and how quickly you can exhale. The clinician uses the results to look for airflow limitation and whether it changes under specified conditions. It is generally performed in a clinic or pulmonary testing service and requires a reliable effort and correct technique.

Peak expiratory flow

A peak-flow meter measures how fast you can blow air out. A clinician may ask you to record readings at home over several days or weeks to look for variability. A peak flow meter for asthma can be useful for monitoring when it is incorporated into a clinician’s plan, but a single reading cannot diagnose asthma by itself. Your personal best and action-plan zones must be set with professional guidance.

FeNO and blood eosinophils

GINA’s 2025 FAQ notes that fractional exhaled nitric oxide (FeNO) or blood eosinophils may support a diagnosis of Type 2 asthma in someone with typical symptoms when lung-function testing is unavailable or normal. Raised values also occur in conditions other than asthma, and low values do not rule asthma out. These biomarkers are supporting evidence, not standalone confirmation.

How do the main diagnostic tools differ?

Tool What it measures Typical setting What it can and cannot show
Spirometry Volume and speed of exhaled air Clinic or lung-function service Objective evidence of airflow limitation and its variability; a normal test does not automatically exclude asthma.
Peak-flow meter Maximum speed of exhalation Clinic instruction with possible home monitoring Patterns over time can support assessment and monitoring; one reading does not diagnose asthma.
FeNO Exhaled nitric oxide associated with airway inflammation Where available through clinical assessment May support Type 2 asthma assessment; elevated or low results are not definitive alone.
Blood eosinophils A type of white blood cell associated with Type 2 inflammation Blood test May add supporting evidence; results can be elevated for non-asthma reasons and cannot confirm or exclude asthma alone.

How is asthma treated?

Treatment is tailored to age, symptom pattern, severity, other conditions, response and local guidelines. It normally combines medication with education and monitoring. Never change a prescribed dose or stop treatment because of an article.

Controller treatment

Controller medicines are taken on the schedule prescribed to reduce airway inflammation, prevent symptoms and lower the chance of attacks. GINA’s 2025 guidance states that treatment containing an inhaled corticosteroid (ICS) greatly reduces the risk of severe exacerbations. The exact medicine, device and schedule must be selected with your clinician.

Reliever treatment

Reliever medicines are used for current symptoms or an attack according to your prescription and action plan. Some people’s plans use a reliever in a particular combined regimen; others use a separate device. Because regimens differ by person and country, follow the written instructions supplied for your inhaler rather than copying someone else’s dose or timing.

Triggers and monitoring

Ask your clinician how to handle exposures that repeatedly precede symptoms, such as respiratory infections, smoke, allergens, exercise or workplace irritants. Trigger avoidance is not a substitute for prescribed controller treatment. Monitoring symptoms, reliever use and (when advised) peak-flow readings helps your clinician decide whether the plan is working.

Why inhaler technique matters

Each inhaler requires a particular sequence for preparing, breathing in and holding the breath. Incorrect technique can leave medicine in the mouth or device instead of the airways. Ask a doctor, nurse or pharmacist to watch you use the device and correct each step; the NHS specifically recommends a practical demonstration.

A spacer for a metered-dose inhaler may help some people use a compatible press-and-breathe device. Spacers are not universal: compatibility, cleaning and suitability depend on the inhaler and the person. Have a pharmacist or clinician confirm the combination and demonstrate its use.

What is an asthma action plan?

An asthma action plan is a written, individualized set of instructions for staying well, recognizing deterioration and obtaining help. The NHLBI treatment and action-plan guidance says it should cover:

  • Known or suspected triggers and practical avoidance steps
  • Your early warning signs and how to recognize an attack
  • Which medicines to use, how much and when, as prescribed
  • When to contact your healthcare provider
  • When to go to an emergency department or call emergency services
  • Emergency contact details and relevant medical information

Keep the plan where you and household members can find it, and provide copies to caregivers, school or work when appropriate. If you do not have one, or your symptoms, medicines or circumstances have changed, ask your clinician to create or review it. The plan—not a generic internet schedule—should determine your attack steps.

What should I do during an asthma attack?

  1. Use your personal action plan immediately. Take the prescribed reliever exactly as its instructions state. Do not exceed the plan’s maximum amount.
  2. Assess the response. Note whether breathing, speech, chest tightness and wheeze are improving within the timeframe your plan specifies.
  3. Get emergency help if the plan’s threshold is reached. Call the local emergency number when breathing remains very difficult, symptoms worsen, the prescribed reliever is not helping, or the plan tells you to do so.
  4. Do not drive yourself if severely unwell. Tell the dispatcher or responders what medicine was taken and when, and take your inhalers and action plan if it is safe to do so.

Do not wait for wheezing to become dramatic. A dangerous attack can present with very little air movement, sometimes called a “quiet chest.”

When should I go to the emergency room for asthma?

Seek emergency care for serious or worsening breathing difficulty, inability to speak normally because of breathlessness, symptoms that are not relieved by the prescribed reliever, or any severe sign identified in your action plan. GINA’s acute-care material includes drowsiness, confusion and a quiet chest among signs requiring urgent clinical response.

United States

The NHLBI asthma-attack guidance advises calling 9-1-1 if medicines are not relieving symptoms or breathing remains very hard. Follow the dispatcher’s instructions while waiting.

United Kingdom

NHS guidance says to call 999 if the person is getting worse, does not improve after the maximum dose in their own action plan, or has no inhaler. These instructions are specific to UK services and to the person’s prescribed plan.

Elsewhere

Use your country’s emergency number and local emergency advice. US 9-1-1 and UK 999 are examples, not interchangeable worldwide instructions. If breathing is seriously impaired, call rather than searching for more information online.

What should happen after an asthma attack?

Arrange clinical follow-up even if symptoms settle. The NHS advises an urgent GP appointment and review within two days after an attack in its UK guidance. Other health systems may set different timelines, but the purpose is the same: check recovery, review inhaler technique and triggers, assess whether controller treatment needs adjustment, and update the written action plan. Bring your medicines, device and symptom or peak-flow records.

How can I prepare before the next episode?

  • Know the names and roles of your controller and reliever medicines.
  • Check that inhalers are in date, available and used with the technique taught to you.
  • Keep the written action plan accessible and make sure family or caregivers know where it is.
  • Record changing symptoms, nighttime waking and increasing reliever use for your clinician.
  • Ask before buying or using monitoring equipment; a peak-flow meter is useful only when readings have a defined role in your plan.

Asthma control is an ongoing process. Regular review is especially important when symptoms are changing, attacks are recurring or treatment no longer provides the same relief.
